Correction of malunited intra-articular distal radius fractures with an inside-out osteotomy technique.

PURPOSE: To present an inside-out osteotomy technique under arthroscopic guidance to correct an intra-articular malunion. METHODS: The joint is explored with a 2.7-mm arthroscope through the standard portals without infusing any water. To allow room to introduce the curettes and the osteotomes (4-mm wide), the instrumentation portals are made slightly larger than usual. Malunited fragments are cut with the osteotomes from inside the joint and advanced out. Fragments are mobilized, and granulating tissue and/or new bone is removed with curettes and synoviotomes. After disimpaction and reductio

A randomised clinical trial of activity-focussed physiotherapy on patients with distal radius fractures.

INTRODUCTION: Physiotherapy is considered by some authorities to be an important aspect of management in patients following distal radius fractures. There is evidence of improvement in impairment with physiotherapy; however, there is no evidence to support early return of functional activity. Traditional physiotherapy management has focussed on improving impairment; however, there are no trials with emphasis on skill acquisition via motor re-learning principles. MATERIALS AND METHODS: Forty-one participants with conservatively managed distal radius fractures were studied in a randomised, sing…

The primary Sauve-Kapandji procedure--for treatment of comminuted distal radius and ulnar fractures.

We have performed primary Sauve-Kapandji procedures on four patients with severe open comminuted fractures of both the distal radius and ulna. The fragmented distal ulna was fixed to the sigmoid notch in order to stabilize the ulnar side of the carpus, and a proximal pseudoarthrosis was maintained for forearm rotation. All the distal radial fractures united without major complications. The mean wrist flexion/extension arc was 76 degrees , the mean pronation/supination arc was 135 degrees, and grip strength was 64% of the contralateral side. All patients returned to their work or daily activiti

Outcome after surgery of distal radius fractures: no differences between external fixation and ORIF.

The purpose of this study was to determine the most appropriate surgical treatment from three procedures used for fractures of the distal radius. In a retrospective comparative study, 166 out of 237 patients who underwent surgery for AO/ASIF A3 or C2 distal radius fractures were evaluated. Surgical procedures were external fixation or open reduction and internal fixation with either palmar or dorsal plates. Radiological outcomes were palmar tilt, radial angle, radial length and ulnar variance. Functional outcomes were range of motion and grip strength. In addition, outcomes according to Gartla…

Results of autogenous trephine biopsy needle bone grafting in fractures of radius and ulna.

Cortico-Cancellous bone graft harvested from the anterior iliac crest by the conventional open method is associated with more morbidity and is more time consuming as compared to the percutaneous method using trephine biopsy needle. The aim of the study was to determine whether cancellous bone graft harvested from anterior iliac crest using trephine biopsy needle consistently achieved bone union in comminuted fractures and fractures of more than 3 weeks duration of radius and ulna and also to determine the morbidity at the donor site. Autogenous cancellous bone graft was harvested percutaneousl

Pseudo-rupture of Extensor Pollicis Longus following Kischner wire fixation of distal radius fractures.

We report two cases of extensor pollicis longus tendon tethering following K-wire insertion to treat unstable distal radius fractures, an unusual complication. In both cases, extensor pollicis longus function returned with conservative management. The structures considered "at risk" during K-wire insertion are described, and the common complications are discussed. Operative approaches to minimise soft tissue damage during K-wire insertion are reviewed. This case report is the first to demonstrate the potential for ultrasound scanning to aid in the diagnosis of extensor pollicis longus tetherin…

Volar fixed-angle plate fixation for unstable distal radius fractures in the elderly patient.

PURPOSE: Increased incidence of falls and osteoporosis combine to make distal radius fractures a major cause of morbidity for the elderly patient. This report presents our experience treating distal radius fractures in the elderly population using a volar fixed-angle internal fixation plate. METHODS: We reviewed retrospectively all patients older than 75 years treated during a period of 4 years and 7 months at our centers for unstable distal radius fractures using a volar fixed-angle plate. Postoperative management included immediate finger motion, early functional use of the hand, and a wris…

Palmar tilt changes due to distal radius fractures and radiocarpal instability: a biomechanical study.

OBJECTIVE: To investigate the impact of dorsally angulated malunited distal radius fractures on the stability of the radiocarpal joint. METHODS: Eight fresh upper limb specimens from 4 human cadavers were obtained, in which radial osteotomy was performed to prepare models of Frykman I distal radius fracture. Pressure-sensitive film was applied to study the changes in the contact area distribution and the pressure load across the radiocarpal joint in response to palmar tilt variation, and the lateral radiograph of the wrist was taken at the end of pressure loading. RESULTS: When the dorsal ti…

Arthroscopically-assisted reduction of intra-articular fractures and soft tissue management of distal radius.

Arthroscopy was used to help to reduce intra-articular fractures of the distal radius and treat soft tissue injuries in 33 acute cases. The fractures were treated by reduction under arthroscopic control and percutaneous fixation either with or without external fixation. The triangular fibrocartilage complex (TFCC) was torn in 18 of 33 patients (54%). All tears were peripheral and were repaired with arthroscopic procedures. Scapholunate (SL) ligament injuries prevailed in six (18%) patients; most of them exhibited instability in the SL joint. They received SL ligament debrided and transfixed wi

Conservative treatment of intra-articular fractures of the distal radius--factors affecting functional outcome.

One hundred and eleven intra-articular fractures of the distal radius in young adults that were conservatively treated were studied. At two years, 80% of fractures were rated as excellent or good by the modified Green and O'Brien scoring system. The rate of re-displacement within casts was 28%. Sixty five per cent of re-displaced fractures were treated with surgical fixation. Those re-displaced fractures treated with repeated manipulation had only 33% excellent or good result. Moreover, their initial alignment could not be preserved. Axial compression (>2 mm) and dorsal angulation (>15 degrees

[Transfixation upper arm plaster bandage for treatment of comminuted fractures of the distal radius].

Thirty-five comminuted fractures of the distal radius in 34 patients were reduced by manipulation and pins, then immobilised with plaster. The Kirschner wires were inserted into the first metacarpal and the olecranon. In five cases, problems with the proximal pin occurred. A clinical and radiological examination could be done in 21 patients three to nineteen years after the accident. Using the demerit-point-system of Gartland and Werley, the result was excellent in three patients, good in nine, and fair in nine; there were no poor results. After careful retrospective consideration of treatment

[Initial experiences with a new wrist joint fixator in treatment of distal radius fractures].

Between January 1989 and October 1991, 56 patients with 57 distal radius fractures were treated with the new Orthofix wrist fixator. This review of 43 patients consists of cases with follow-ups of at least 12 months. One patient had died, and 38 (90.5%) of the remaining 42 fractures were examined 20.3 months after surgery, on average. Radiological findings were classified according to Lidström, and the functional results according to Gartland and Werley in the Sarmiento modification. Functional analysis demonstrated excellent and good results in 73% of the fractures. Although the number of cas…

Changes in the incidence and treatment of distal radius fractures in adults - a 22-year nationwide register study of 276,145 fractures.

INTRODUCTION: The incidence of distal radius fractures (DRFs) is reported, with conflicting results. To ensure evidence-based practice, the variation in treatment over time should be monitored. Treatment in the elderly population is particularly interesting because newer guidelines find little indication for surgery. Our primary aim was to assess the incidence and treatment of DRFs in the adult population. Secondly, we assessed the treatment stratified on non-elderly (aged 18-64 years) and elderly (aged 65+ years) patients. METHODS: This is a population-based register study comprising all adu…

Conservative treatment of ulnar styloid fractures following volar-plate fixation of distal radius fractures: incidence of nonunion evaluated by computed tomography.

PURPOSE: The first aim of this study was to investigate the incidence of ulnar styloid fractures (USFs) accompanied by distal radius fractures (DRFs), treated with volar locking plates. The fracture type of DRFs was evaluated by the classifications, based on computed tomography (CT) scan findings. The second aim was to investigate the bone union rate of USFs, depending on the fracture type of DRFs, by comparing union and nonunion groups in the USFs groups. METHODS: Between May 2012 and December 2019, 239 consecutive patients with DRFs were treated. Of these patients, 177 DRFs met inclusion cr…

Radiographic Healing and Functional Outcomes of Untreated Ulnar Styloid Fractures Following Volar Plate Fixation of Distal Radius Fractures: A Prospective Analysis.

Background: Ulnar styloid fractures (USFs) are common concomitant injuries associated with distal radius fractures (DRFs). Recent studies have found conflicting evidence on whether these fractures treated or untreated effect pain and functional outcomes. The purpose of this study was to prospectively evaluate pain and function outcomes of consecutively untreated USFs in surgically repaired DRFs. The study hypothesis was that there would be no difference in outcomes whether an USF is present or not, with all cases left untreated. Methods: A prospective study at a single institution of consecuti…

[Carpal lesions associated with dislocated fractures of the distal radius. A systematic screening of 104 fractures using preoperative CT and MRI].

BACKGROUND: The functional outcome of surgically treated dislocated fractures of the distal radius is limited and does not correlate with radiographic results. Additional carpal lesions are assumed to be the cause. This study has evaluated which carpal lesions are associated with dislocated fractures of the distal radius. MATERIAL AND METHODS: A total of 104 consecutive patients with dislocated fractures of the distal radius were included in the study. The injured wrist was examined by radiography, computed tomography (CT) and magnetic resonance imaging (MRI) to determine additional carpal le
