Artificial intelligence-enabled cytopathological interpretation of fluorescence microscopy images via a multimodal large language model.

Quantitative analysis of fluorescence microscopy images remains a significant challenge in cytopathological assessments, as subjective interpretation and inter-observer variability hinder reproducibility and standardization of classifications. In this study, an artificial intelligence-assisted analytical framework utilizing a multimodal large language model (MLLM) was employed to classify cytotoxic responses in acridine orange/propidium iodide-stained MCF-7 cells subjected to doxorubicin treatment for 24 h. A total of 500 fluorescence images, collected from five experimental groups including a

Improving fairness of large language model-based ICU mortality prediction via case-based prompting.

Accurately predicting mortality risk in intensive care unit (ICU) patients is critical for clinical decision-making. Large language models (LLMs) are increasingly explored for clinical prediction using structured medical data, but their outputs may exhibit demographic disparities. Mitigating such disparities without degrading predictive performance remains challenging. We systematically investigate demographic bias in LLM-based ICU mortality prediction and propose Case-Based Prompting (CAP), a training-free framework designed to improve the empirical balance between predictive performance and …

Multimodal large language model versus emergency physicians for burn assessment: a prospective non-inferiority study.

BACKGROUND: Accurate burn size and depth assessment at first contact guides fluid resuscitation, referral, and operative planning, yet both tasks show meaningful inter-clinician variability. General-purpose multimodal large language models may offer scalable, image-based decision support in emergency care, but prospective benchmarking against clinicians and a robust reference standard remains limited. METHODS: We conducted a prospective, single-centre diagnostic accuracy and agreement study in a tertiary emergency department (22 July-8 September 2025). Consecutive acute burn presentations (< …

Evaluating GPT-4 Responses on Scars or Keloids for Patient Education: Large Language Model Evaluation Study.

BACKGROUND: Scars and keloids impose significant physical and psychological burdens on patients, often leading to functional limitations, cosmetic concerns, and mental health issues such as anxiety or depression. Patients increasingly turn to online platforms for information; however, existing web-based resources on scars and keloids are frequently unreliable, fragmented, or difficult to understand. Large language models such as GPT-4 show promise for delivering medical information, but their accuracy, readability, and potential to generate hallucinated content require validation for patient e…

Development and pilot evaluation of a large language model-based clinical simulation system for medical education in gastroenterology.

BACKGROUND: Medical education relies on experience to develop clinical reasoning skills, yet patient access is often limited. Large language models (LLMs) offer an accessible alternative for simulating patient encounters. We aimed to develop and evaluate an LLM-based clinical simulation system for gastroenterology that simulates entire clinical encounters in natural language, from history-taking through to treatment and follow-up planning. METHODS: The study was conducted in three phases. First, a system comprising an LLM-based clinical case generator, an LLM-based patient simulator, and a we…

xBind: an integrated webserver for large language model-enabled cross-molecular protein binding site prediction.

xBind is an interactive, freely accessible, and fully configurable webserver for large language model (LLM)-enabled cross-molecular protein binding-site prediction. xBind leverages LLM embeddings from the ESM-2 model together with sequence- and structure-derived features to predict protein-protein, protein-DNA, and protein-RNA binding sites using symmetry-aware deep graph neural networks. The input to xBind is either a single-chain protein sequence in FASTA format or a monomer protein structure in PDB or mmCIF format and it outputs predicted residue-level binding sites of the input protein wit

When collaboration fails: persuasion driven adversarial influence in multi agent large language model debate.

Recent developments have made Large Language Model (LLM) multi-agent systems a promising paradigm for enhancing reasoning via collaborative debate and collective deliberation. Prior work has demonstrated that coordinated LLM agents tend to perform better than single models in terms of accuracy, robustness, and reasoning depth. But these benefits depend on a rarely questioned assumption: that all actors act honestly. In this paper we subvert this assumption by identifying one of the most critical weaknesses: a persuasion-induced adversarial influence in LLM-to-LLM debate. Here we show that a si
