Rett females: patterns of characteristic side-asymmetric neuroimpairments at long-term follow-up.

From a follow-up series of 24 classical Rett syndrome females (13-59 years of age), side asymmetries of neuroimpairments were systematically investigated. A characteristic right-sided dominance in terms of dysfunction and neurology was revealed. Differences in the perinatal/early postnatal development of finer brain net structures between brain halves are suggested as a possible explanation.

Acute transverse myelitis in a 7-month-old boy.

Acute transverse myelitis (ATM) is an acute intramedullary spinal cord disease characterized by an abrupt onset of rapidly progressive weakness of the extremities accompanied by a loss of sensation and sphincter control. Although ATM has an important place in childhood spinal cord disorders, its occurrence remains extremely uncommon in the infant population (0 to 12 months old). This report of a 7-month-old boy with ATM is followed by a discussion of the case and review of the literature regarding etiology, diagnosis, and prognosis for neurological recovery.

Intramedullary sarcoidosis presenting as incomplete paraplegia: case report and literature review.

Sarcoidosis is a chronic, multisystem noncaseating granulomatous disease of unknown etiology. Sarcoidosis typically presents clinically in individuals between the ages of 20 and 40 years. Although sarcoidosis most commonly affects the respiratory system, nearly any organ system can be involved. Spinal cord involvement by sarcoidosis is a rare event, occurring in less than 1% of patients with systemic disease. The case presented is that of a 29-year-old black male with benign past medical history who presented with a 6-week history of progressive incomplete paraplegia and bowel dysfunction. Mag

The Kleine-Levin syndrome. Report of a case and review of the literature.

Kleine-Levin syndrome is a rare self-limited disorder which usually affects adolescent males and is characterized by episodic hypersomnia, increased appetite, and behavioral/psychiatric disturbances. Individuals are normal between the attacks. The case of an adolescent boy is presented who suffered from recurrent sleepiness, hyperphagia, and behavioral disturbances such as rocking, punching and pacing, and was originally misdiagnosed as suffering from encephalitis. Before the diagnosis of Kleine-Levin was given, the patient underwent unnecessary investigations and treatment which, in turn, com

Multiple cranial neuropathy: a common diagnostic problem.

Syndrome of multiple cranial palsies is a common clinical problem routinely encountered in neurological practice. Anatomical patterns of cranial nerves involvement help in localizing the lesion. Various infections, malignant neoplasms and autoimmune vasculitis are common disorders leading to various syndromes of multiple cranial nerve palsies. A large number of diffuse neurological disorders (e.g. Gullian-Barre syndrome, myopathies) may also present with syndrome of multiple cranial nerve palsies. Despite extensive biochemical and radiological work-up the accurate diagnosis may not be establis

Differential diagnosis of Parkinson's disease.

The differential diagnosis of PD includes other neurodegenerative disorders; hereditary disorders; and symptomatic causes, such as structural lesions, infections, metabolic abnormalities, hydrocephalus, and drugs or toxins. A good history of symptom evaluation, drug use, and family illness is just as essential as a careful neurologic examination when evaluating a patient with parkinsonism. Although there is no definitive diagnostic test for PD at this time, tests to rule out other causes should be considered and then treatment started.

Study on relationship between carbon disulfide exposure and symptoms and signs of nervous system in workers.

The relationship between carbon disulfide (CS2) and the symptoms and signs of nervous system in rayon workers was investigated. In a cross sectional study of CS2-exposed workers from the viscose industry and controls, data on neurological symptoms and clinical neurological examination were evaluated. The exposed subjects includes 326 workers and 105 workers from the same factory, without exposure to CS2 or to any other neurotoxic agents, served as controls. The median exposure to CS2(8 h time weighted) in the low-level exposure group was 4.6 mg/m3, and 18.3 mg/m3 in high-level exposure group.

Proximal neuropathies of the upper extremity.

This article provides a comprehensive review of the diagnosis and management of mononeuropathies of the long thoracic, suprascapular, axillary, and musculocutaneous nerves. Although these nerves are frequently injured in conjunction with other portions of the brachial plexus, this discussion focuses on isolated injury to each of these nerves. The anatomy, clinical presentation, differential diagnosis, findings on electrodiagnostic evaluation, origin, management, and prognosis of each mononeuropathy is discussed.

[The quantitative assessment of functional disorders of the hand and upper limb].

Methods have been developed for calculation of quantitative indicators of the hand and upper limb function, which rely on measurement of sensitivity of fingers, the pick-objects-up test (a modified Moberg test), measurement of active and passive movements of joints, hand grips. The following quantitative indicators are computable from formulae: sensitivity, motor function of joints, grips/grasps, coordination of movements. Based on the above four indicators is computation of the integral function of the hand and upper extremity. Computation procedure for quantitative determinants of the upper

[Spontaneous subdural hematoma in a 18-day-old male newborn infant with severe hemophilia A].

Most intracranial bleedings in hemophiliacs occur in patients more than 6 months of age. In the neonatal period, this complication is rare and almost always observed in the first week of life. Based on a review of the literature, intracranial hemorrhage is an exceedingly rare occurrence in infants with hemophilia aged 2 weeks-6 months. We report on a male infant with hemophilia A who was referred to our hospital on day 18 because of pallor and jaundice. The neurological examination was normal. A cerebral ultrasound showed a left sided subdural hematoma with a shift of the midline structures to

Controversies in pallidal surgery.

Posteroventral pallidotomy (PVP) has gained a worldwide acceptance after its reintroduction by Laitinen et al. in 1992 (56) and many studies have since been published. A review of the recent literature reveals that there is variation in the clinical indications for this procedure, the surgical technique used and the assessment of results. There is no uniform practice in the choice of the anatomical target point within the globus pallidus, the imaging of the target structure, the intraoperative assessment of the physiological target and the mode of evaluation of the surgical results. Although s

[MDMA ("Ecstasy") use--an overview of psychiatric and medical sequelae].

Epidemiological research and Substance Abuse Warning Systems point to a sharp increase in the use of "Ecstasy" (MDMA), as well as to structural changes in the drug scene in and outside Europe. For some consumers, "Ecstasy" opens the door to the abuse of other illegal substances. Since the mid-eighties psychiatric complications and consequences of the abuse of MDMA have been reported in at least 48 cases. It is necessary to differentiate between acute psychiatric complications, which subside completely when the level of intoxication comes down, toxic psychoses and long-term psychiatric diseases

[Cataplexy in type C Niemann-Pick disease].

Cataplexy usually occurs as a part of the tetrad of clinical phenomena of idiopathic narcolepsy. Symptomatic cases are rare. A 4 years old girl from consangineous parents had recurrent loss of muscle tone and fell to the ground, when she laughed. The EEG was normal. Prolonged neonatal jaundice with cholestasis, hepatosplenomegaly, mental regression, supranuclear ophthalmoplegia, and foam cells led to the diagnosis of Niemann-Pick disease type C with symptomatic cataplexy. Symptomatic forms of the narcolepsy-cataplexy complex should be considered, when there is an early onset before puberty, ca

Muscle pain, fatigue, and fasiculations.

This article discusses muscle pain, fatigue, and fasiculations. Muscle pain and fatigue are common problems in general medicine and in neurology, while fasiculations raise concern about a potentially ominous disease. The author reviews the conditions that cause pain and similar conditions arising from nonmuscular soft tissues. The article includes a general evaluation to be used for each of these clinical problems.

[Differential sciatica pain diagnosis from the neurologic viewpoint].

Low back pain and sciatica with or without neurological deficits represent frequent disorders in clinical practice. Both, history and neurological examination are essential for localizing underlying lesions and initiating appropriate investigations. Pain location, sensory symptoms, motor signs and reflexes are extremely reliable in indicating which motor root(s) or peripheral nerve(s) are affected. Sometimes, pain and/or paraesthesia are the only presenting symptoms. Therefore differential diagnosis of low back pain and sciatica is various. This article summarizes the differential diagnosis of
