Malignant hyperpyrexia.

Malignant hyperpyrexia is a dangerous complication of general anesthesia occurring in individuals with an underlying disease of muscle. The … exia is to carry out an in vitro muscle test in which the muscle is exposed to caffeine, halothane, succinylcholine, and potassium chloride. Susceptible individuals should be given local, regional, or spinal anesthesia if an operation is needed. If this form of anesthesia is unsuitable, barbiturates such as thiopentone, tranquilizers such as diazepam, narcotics s …

The roles of striated and smooth muscle in the movement of the cat's nictitating membrane.

1. When the movement of the nictitating membrane is recorded in the usual way, pulled out under load in cat, dog, or rabbit, end-plate-depolarizing drugs such as succinylcholine, decamethonium, nicotine, and 2268F, cause a retraction.2. This movement is due to activation of orbital striated muscle, as evidenced by the resistance of the movement to ganglion block or excision of the superior cervical ganglion and to administration of phentolamine or atropine, and by its great sensitivity to dimethyltubocurarine.3. End-plate-depolarizing drugs produce contractions of the superior and inferior obl…

Cardiac arrest in adult patients following burn injury: a scoping review with expert recommendations on management.

BACKGROUND: Cardiac arrest following severe burn injury is rare but devastating, reflecting profound systemic compromise from hypovolemia, h … pected carbon monoxide poisoning, empirical hydroxocobalamin for enclosed-space fires, avoidance of succinylcholine after 24 h, and individualized decisions on cardiopulmonary resuscitation. CONCLUSIONS: Burn-associated cardiac arrest remains poorly characterized with uniformly poor outcomes except in electrical injury. These findings and consensus recommendatio …

Chronic inflammatory demyelinating polyradiculoneuropathy and anesthesia: a case series.

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an acquired autoimmune demyelinating polyneuropathy characterized by sym … laxants. Seventeen patients with CIDP underwent general anesthesia, of whom 16 had muscle weakness. Succinylcholine was used in 5 cases (29.4%) and nondepolarizing muscle relaxants in 11 cases (64.7%). Two patients required postoperative mechanical ventilation; one was critically ill and the other had open heart surgery. One patient had aspiration on the second p …

[Hemodynamic effects of the induction of general anesthesia after low thoracic epidural anesthesia].

Sixteen ASA 1 or 2 patients scheduled for abdominal surgery were included in the study after they had given their informed consent. Thirty minutes after starting a low-thoracic epidural anaesthesia (median level of sensitivity loss: T5), the patients were randomly given an intravenous bolus injection of either thiopentone (4 mg.kg-1, n = 8) or etomidate (0.5 mg.-1, n = 8), associated with succinylcholine 1 mg.kg-1. One minute after induction of general anaesthesia, the patients were intubated and mechanically ventilated (V(T) 8 ml.kg-1, rate 12 c.min-1). Mean arterial blood pressure (MAP) (osc…

[Circulatory collapse in bodybuilder during anaesthesia].

The use of anabolic steroids is a growing problem in Denmark. The effects and side effects caused by anabolic steroids in relation to anaesthesia are poorly described. This article describes a case of circulatory collapse in a young bodybuilder during anaesthesia. The primary cause of the collapse remains undetermined, but different hypotheses are proposed based on previously published literature.

Cardiovascular actions of common anesthetic adjuvants during desflurane (I-653) and isoflurane anesthesia in swine.

To determine the cardiovascular actions of drugs commonly combined with inhalation anesthetics, we administered one drug from each of several classes of adjuvants to seven swine already anesthetized with equipotent concentrations (1.2 MAC) of desflurane, formerly I-653, a new inhaled anesthetic, or isoflurane. Succinylcholine (1 and 2 mg/kg), atracurium (0.6 mg/kg), and atropine (5 micrograms/kg) plus edrophonium (5 mg/kg) had no cardiovascular effects. Fentanyl was given in amounts that decreased MAC for the inhaled anesthetics by 25%-35%. A dose of 50 micrograms/kg IV had no cardiovascular e…

[Anaphylactic shock during elective cesarean section. Sensitizing mechanisms and follow-up].

Anaphylactic reactions during anaesthesia are increasing. Women experience these reactions more often than men. In about 50% of the cases the reaction is due to a muscle relaxant drug. Recently, Australian and French researchers have described these reactions as possibly mediated by IgE in most of the patients. As regards Ig-antibody binding, the quaternary ammonium ions seem to be potential determinants in the muscle relaxant molecule. Patients may have been sensitized by exposure to cosmetics, disinfectants or solvents, and cross-reactivity between different neuromuscular blocking agents has

Pathophysiology of malignant hyperthermia.

Malignant hyperthermia (MH) is a pharmacogenetic disease in man and animals. It primarily involves skeletal muscle tissue, but other tissues might be affected to a lesser degree. Calcium homeostasis in muscle cells is upset in susceptible individuals, so that various agents and circumstances can increase the free, ionised intracellular calcium concentration to damaging levels. The primary defect is not known at present, but is believed to involve an abnormally sensitive calcium-induced calcium release mechanism. Thus small, localised increases in calcium concentration releases more calcium so

Anaesthesia and progressive muscular dystrophy.

The presentation and features of Duchenne's progressive muscular dystrophy (Duchenne's PMD) are described and the increased risks associated with anaesthesia are considered. Hazards associated with induction of anaesthesia and immediate postoperative recovery have been stressed in recent case reports, and these are summarized. Features of a hyperpyrexia-like response including cardiac arrest, increased serum creatine phosphokinase concentration, myoglobinuria and metabolic acidosis following suxamethonium or halothane, or both, have been described in patients with Duchenne's PMD. Subsequent in

[Risk factors in anaphylactoid reactions to muscle relaxants. A retrospective study of 103 cases of shock].

A homogeneous series of 103 cases of shock due to muscle relaxants has been used to identify the risk factors in anaphylactoid shocks due to either true anaphylaxis or to non-specific histamine release. Clinical atopy (asthma) and sub-clinical atopy as shown up by skin tests with mites and pollens, as well as a history of IgE-dependent drug allergy, were present with a significantly greater frequency in the history of patients presenting with anaphylaxis. Abnormal histamine release and reactivity to histamine, as assessed by skin tests with 48/80 and histamine, were often found in those patien

Comparison of the frequency of muscle pain associated with suxamethonium in pre- and post-ovulatory women and in those taking a combined oral contraceptive pill.

The frequency of muscle pain following the use of suxamethonium was compared in pre- and post-ovulatory women and in those taking a combined oral contraceptive pill. There were no statistically significant differences between the groups. It is suggested that serum concentrations of progestogen were too low to produce the protection against suxamethonium pains reported in late pregnancy.

Effect of competitive myoneural blockade and fentanyl on muscle fasciculation caused by suxamethonium in children.

The effects of tubocurarine 0.06 mg kg-1, alcuronium 0.03 mg kg-1, pancuronium 0.01 mg kg-1, and fentanyl 1 or 2 micrograms kg-1 on the muscle fasciculations associated with suxamethonium were studied in 171 children undergoing otolaryngological surgery. The mean fasciculation index in all pretreatment groups was significantly smaller than in the control group. The most effective pretreatment was fentanyl 2 micrograms kg-1 followed, in order, by alcuronium, fentanyl 1 microgram kg-1, tubocurarine and pancuronium. The rate of the onset of the fasciculations after the injection of suxamethonium

Neuromuscular blocking properties of suxamethonium and decamethonium in normal and myasthenic rat muscle.

Patients with myasthenia gravis (MG) have increased tolerance to the neuromuscular blocking properties of suxamethonium (SCh) and decamethonium (C10) and exhibit a reversal of the C10-induced block by neostigmine. The effects of these drugs were compared in forelimb flexor digitorum longus muscle from normal rats and from rats with experimental autoimmune myasthenia gravis (EAMG) to investigate the similarity of EAMG to MG. The depolarization induced by 1, 5, 10 and 25 microM SCh or C10 at the motor end-plates was significantly higher in normal than in EAMG muscle. However, both normal and EAM

Influence of barbiturate anesthetic induction on circulatory responses to morphine.

Circulatory responses to morphine (1 mg/kg, 10 mg/min) following a barbiturate anesthetic induction (thiamylal 4 mg/kg) were determined in 7 patients with coronary artery disease. Compared with awake measurements, the mean arterial pressure and cardiac index were decreased 16 and 42 percent, respectively (p less than 0.05) and systemic vascular resistance increased 35% (p less than 0.05) at the end of morphine infusion. The same dose of morphine, but without prior barbiturate, has been previously reported not to significantly alter these measurements. Therefore, the assumption that morphine is

Effect of pregnenolone-16alpha-carbonitrile (PCN) on drug response in man.

Pregnenolone-16alpha-carbonitrile (PCN), which alters drug responses and induces hepatic microsomal drugs-metabolizing enzymes in experimental animals, is well-tolerated in man. There were no marked changes in the concentrations of blood sugar and urea; marked changes in the concentrations of blood sugar and urea; bromsulphophthalein (BSP) clearance; serum alkaline phosphatase activity; hematocrit, erythrocyte and leukocyte counts; as well as in ecg patterns. When the steroid was given by mouth for 4 days to patients treated with electroshock for psychiatric disorders, it decreased suxamethoni
