Acute and chronic influences of obstructive sleep apnoea on the pulmonary circulation.

Obstructive sleep apnoea (OSA) produces immediate effects on pulmonary haemodynamics during sleep in all subjects. In addition, in some subjects, OSA is accompanied by chronic abnormalities of the pulmonary circulation. During sleep, pressure in the main pulmonary artery oscillates within each apnoea, in synchrony with intrathoracic pressure changes; in addition, it may increase progressively as a consequence of prolonged severe hypoxaemia. Pulmonary capillary wedge pressure may increase during inspiratory efforts, possibly reflecting a mechanical limitation of left ventricular function. Cardi

Oxygen Therapy Lowers Right Ventricular Afterload in Experimental Acute Pulmonary Embolism.

OBJECTIVES: To investigate if oxygen could unload the right ventricle and improve right ventricle function in a porcine model mimicking intermediate-high risk acute pulmonary embolism. DESIGN: Controlled, blinded, animal study. SETTING: Tertiary university hospital, animal research laboratory. SUBJECTS: Female, Danish pigs (n = 16, approximately 60 kg). INTERVENTIONS: Acute autologous pulmonary embolism was induced until doubling of baseline mean pulmonary arterial pressure. Group 1 animals (n = 8) received increasing Fio2 (40%, 60%, and 100%) for time intervals of 15 minutes returning to

Peri-operative myocardial performance in infants with Down syndrome undergoing CHD repair.

BACKGROUND: We aimed to characterise the impact of Down syndrome on myocardial performance and loading conditions in infants with Down syndrome and CHD over the peri-operative period by comparing them with infants matched for cardiac lesion with a normal microarray. METHODS: Left ventricular global longitudinal strain, right ventricular free wall longitudinal strain, left ventricular end-systolic wall stress, and right ventricular systolic pressure were measured in the two groups over the peri-operative period. RESULTS: Fifty-five infants had a diagnosis of Down syndrome and these were compa

Inhibition of Sodium/Hydrogen Exchanger-1 in the Right Ventricle and Lung Dysfunction Induced by Experimental Pulmonary Arterial Hypertension in Rats.

BACKGROUND: Life-threatening pulmonary arterial hypertension (PAH) still lacks a direct therapeutic approach targeted to the molecular defects associated with the disease. Here, we focus on the impaired regulation of intracellular acidity and sodium/calcium overload by testing the hypothesis that inhibiting NHE-1 (sodium/hydrogen exchanger isoform 1) with rimeporide enables the recovery of pulmonary and right ventricular dysfunctions in the Sugen5416/hypoxia PAH model in rats. METHODS AND RESULTS: Adult Sprague-Dawley male rats (n=44) rats were divided into 2 broad groups: control and Sugen54

Short-Term Cardiac Functional Change Following Transcatheter Atrial Septal Defect Closure: A Prospective Echocardiographic Study.

BACKGROUND: Atrial septal defect (ASD) is a common congenital heart anomaly causing a significant hemodynamic burden if untreated. Transcatheter closure is a minimally invasive standard treatment, but prospective data on short-term cardiac remodeling are limited. AIMS: To evaluate short-term echocardiographic changes in cardiac structure and function following transcatheter ASD closure in adults. METHODS: This prospective observational cohort study included 112 patients aged 15-68 years who underwent transcatheter ASD closure. Comprehensive echocardiography was performed pre-procedure and at

Atrial and ventricular pressures in atrial flutter.

The hemodynamic effects of atrial flutter (AF) are unknown. The purpose of the present study was to investigate the changes in atrial and ventricular pressures after induction of AF. In 23 patients with paroxysmal AF (age 59 +/- 9 years), a hemodynamic study was performed both during sinus rhythm and after induction of the tachyarrhythmia. During AF, 13 patients showed a fixed 2:1 AV conduction and 10 patients showed variable conduction. Mean right and left atrial pressures increased (P < 0.001) and right and left ventricular end-diastolic pressures decreased (P < 0.001) after induction of AF.

Subclinical cardiomyopathy and long QT syndrome: an echocardiographic observation.

OBJECTIVE: Long QT syndrome (LQTS) is a cardiac channelopathy predisposing to syncope and sudden death secondary to LQT-triggered ventricular arrhythmias. Long QT syndrome has been regarded as a purely electrical disease. Recent reports have shown by echocardiography that LQTS patients have contraction abnormalities that are associated with cardiac arrhythmias. The purpose of this study was to determine the spectrum and prevalence of echocardiographic anomalies in a large cohort of patients diagnosed genetically and/or clinically with LQTS. OUTCOME MEASURES: Two-dimensional and Doppler echoca

Acute effects of balloon valvuloplasty and pacing on left ventricular performance in children with moderate pulmonary valve stenosis, analysed by systolic and diastolic pressure-volume relationships.

Right ventricular overload of volume and/or pressure type may affect left ventricular systolic and diastolic function. This has been shown in animal studies and has been suggested in non-invasive studies in man. Altered geometry of the left ventricle, myocardial hypertrophy and changes in contractile state may be responsible for the change in function. Balloon valvuloplasty is an effective treatment for isolated valvular pulmonary stenosis in children, and results in an immediate decrease of right ventricular systolic pressure. Whether this results in immediate changes in left ventricular perf

Metabolic dysfunction-associated fatty liver disease and cardiac remodeling in non-ischemic dilated cardiomyopathy.

BACKGROUND AND AIM: Metabolic dysfunction-associated fatty liver disease (MAFLD) represents systemic metabolic dysfunction and is significantly associated with cardiovascular disease. However, its specific impact on cardiac remodeling in non-ischemic dilated cardiomyopathy (NIDCM) remains unclear. This study aimed to investigate the impact of MAFLD and liver fibrosis on biventricular remodeling in patients with NIDCM. METHODS AND RESULTS: We evaluated 293 NIDCM patients using comprehensive echocardiography. Liver fibrosis was assessed via the NAFLD Fibrosis Score (NFS). Associations were anal

Cardiovascular Magnetic Resonance and Sport Cardiology: a Growing Role in Clinical Dilemmas.

Exercise training induces morphological and functional cardiovascular adaptation known as the "athlete's heart" with changes including dilatation, hypertrophy, and increased stroke volume. These changes may overlap with pathological appearances. Distinguishing athletic cardiac remodelling from cardiomyopathy is important and is a frequent medical dilemma. Cardiac magnetic resonance (CMR) has a role in clinical care as it can refine discrimination of health from a disease where ECG and echocardiography alone have left or generated uncertainty. CMR can more precisely assess cardiac structure and

Imaging features of desmoplakin arrhythmogenic cardiomyopathy: A comparative cardiovascular magnetic resonance study.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) related to Desmoplakin (DSP) mutations is a distinct condition associated with particularly severe outcomes, more frequent left ventricular (LV) involvement, including fibrosis, dysfunction, and inflammatory episodes. Whether DSP-ACM is associated with specific imaging features remains elusive. This study aims to provide a comprehensive description of cardiovascular magnetic resonance (CMR) findings in patients with DSP-ACM and to compare them to RV-dominant ACM with LV involvement (LV+ right-dominant-ACM). METHODS: Patients with DSP-ACM matched

Bendopnea Predicts Right Ventricular Energy Failure in Patients with Pulmonary Hypertension.

OBJECTIVE: The development of right ventricular failure has a significant adverse prognostic impact on the course of pulmonary hypertension. Right ventricular energy failure has been shown to double the mortality of pulmonary hypertension even after correction for many established risk predictors. We hypothesize that bendopnea may indicate right ventricular energy failure in patients with pulmonary hypertension. METHODS: We prospectively enrolled patients with pulmonary hypertension who were admitted to our pulmonary hypertension outpatient clinic between January 2021 and June 2021. Bendopnea

Right Ventricular Failure and Congenital Heart Disease.

Survivorship into adulthood of patients with congenital heart disease is due to improvements in prenatal detection, novel surgeries, and specialized adult congenital heart disease care. As patients survive further into adulthood, long-term complications of congenital and repaired physiology have been more clearly elucidated. The overall mortality of patients with adult congenital heart disease with heart failure is around 4%. Congenital malformations, palliations, residual defects, and resultant physiology impact the right ventricle. This relationship influences morbidity and mortality. For th

[Clinical significance of incomplete right bundle branch block].

In authors' opinion the majority of the physicians speak about an incomplete right bundle branch block in cases of a terminal r'wave in lead V1 of the ECG, when the QRS complex is not wider than 0.12 sec. These ECG anomalies are not properly separated in the literature, either. Authors define 4 groups whose separation is possible by the analysis of leads V1 and V2. 1. Right ventricular conduction defect--the ECG width is 0.09-0.13 sec. This picture generally signs normal or only slight higher right ventricular pressure. 2. Real incomplete right bundle branch block--the terminal vector directs
