Directional preponderance, 1942-1974. A review.

The subject reviewed is the phenomenon of Directional Preponderance (DP), a term first proposed in 1942 by Fitzgerald & Hallpike for an abnormality in the pattern of the caloric responses found to occur in association with a variety of unilateral lesions, both central and peripheral, of the vestibular system. On the basis of observations carried out upon a series of human subjects, Fitzgerald, Hallpike & Cawthorne (1942) argued that the DP towards the unaffected labyrinth which follows unilateral labyrinthectomy is subserved in part by a unilateral loss of utricular function--a utricular pares

Persistent positional perceptual dizziness in clinical practice: a scoping review.

INTRODUCTION: Dizziness is an important symptom presenting in routine practice and the life time prevalence ranges from 17 to 30%. Persistent positional perceptual dizziness (PPPD) is a common cause of chronic dizziness and has often been labeled as psychogenic dizziness in the past. DISCUSSION: PPPD is diagnosed based on clinical criteria laid down by the Barany society. The vestibular function tests and imaging of the brain and the inner ear are often normal. Most of the patients have an underlying anxiety trait and most cases of PPPD arise following an attack of acute vertigo like Meniere'…

Utility of a novel perceptual-motor control test for identification of sport-related concussion beyond current clinical assessments.

Clinically feasible metrics, that can inform the concussion recovery decision making process by evaluating a unique domain beyond current testing domains (e.g., balance, neurocognition, symptoms, vestibular/ocular function) are still in need. The purpose of this study was to compare perceptual-motor control in adolescent athletes ≤21 days of sport-related concussion and healthy controls and evaluate the association of perceptual-motor control to the outcomes of commonly-used sport-related concussion clinical assessments. Athletes (age: 12-18 years) with sport-related concussion (n = 48) and he

Pre-habilitation Before Vestibular Schwannoma Surgery-Impact of Intratympanal Gentamicin Application on the Vestibulo-Ocular Reflex.

Background: Patients with vestibular schwannoma that show residual peripheral-vestibular function before surgery may experience sudden and substantial vestibular loss of function after surgical resection. To alleviate the sudden loss of peripheral-vestibular function after vestibular-schwannoma (VS) resection, pre-surgical intratympanic gentamicin application was proposed. Objective: We hypothesized that this approach allows for a controlled reduction of peripheral-vestibular function before surgery but that resulting peripheral-vestibular deficits may be canal-specific with anterior-canal spa

Video head impulse in comparison to caloric testing in unilateral vestibular schwannoma.

CONCLUSIONS: Although there was a statistically significant relationship between the results of the vHIT and the caloric test, the limited strength of this relationship suggests that, for unilateral vestibular schwannoma (UVS), caloric testing and vHIT may provide complementary information on vestibular function. OBJECTIVE: There is limited information that can be used to determine which of the video head impulse test (vHIT) and caloric test might be better used in the diagnosis and management of UVS. In this study, a group of participants with un-operated UVS was studied using both methods.

Vestibular involvement in cognition: Visuospatial ability, attention, executive function, and memory.

A growing body of literature suggests the inner ear vestibular system has a substantial impact on cognitive function. The strongest evidence exists in connecting vestibular function to the cognitive domain of visuospatial ability, which includes spatial memory, navigation, mental rotation, and mental representation of three-dimensional space. Substantial evidence also exists suggesting the vestibular system has an impact on attention and cognitive processing ability. The cognitive domains of memory and executive function are also implicated in a number of studies. We will review the current li

Steroids for Acute Vestibular Neuronitis-the Earlier the Treatment, the Better the Outcome?

OBJECTIVE: To present findings that suggest steroid treatment within 24 hours of onset of vestibular neuronitis results in better restitution of vestibular function than treatment between 25 and 72 hours. PATIENTS: Thirty-three consecutive patients (17 men, 16 women, mean age 57 yr, range 17-85 yr) with acute vestibular neuronitis and treated with steroids within 72 hours after symptom onset. Patients were divided into two groups depending on if they were treated within the first 24 hours or not. INTERVENTIONS: Oral prednisolone 50 mg/d for 5 days with tapering of doses for the next 5 days,

Brainstem hyperintensity in patients with vestibular schwannoma is associated with labyrinth signal on magnetic resonance imaging but not vestibulocochlear tests.

OBJECTIVES: Focal hyperintensity in the dorsal brainstem (HDB) has been described in large cerebellopontine angle tumours and is thought to represent vestibular nuclei degeneration, but its functional significance has not been thoroughly investigated. Our aim was to analyse its relationship to imaging characteristics of the tumour and inner-ear structures and to vestibulocochlear functional tests. METHODS: We retrospectively reviewed 54 patients with a histological diagnosis of vestibular schwannoma (VS). Magnetic resonance imaging tumour characteristics (size, cystic composition and distance

[An experimental herpes simplex virus infection in the vestibular nerve].

The author succeeded in producing vestibular ganglionitis by inoculating herpes simplex virus type 1 (HSV-1) into the back of the auricle of a new-born mouse. Postural deviation to the inoculated side was observed in 14 of 275 mice (5.1%) 6 to 10 days after the inoculation. In 6 of the 14 mice (42.9%) with postural deviation, HSV antigens were demonstrated immunohistochemically in the vestibular ganglion cells of the inoculated side. No antigens were identified in the animals with normal posture or in controls. Vestibular functions were evaluated in 5 animals suffering from postural deviation
