BACKGROUND: Inositol is an essential nutrient required by human cells in culture for growth and survival. Inositol promotes maturation of several components of surfactant and may play a critical role in fetal and early neonatal life.
OBJECTIVES: To assess the effectiveness and safety of supplementary inositol in preterm infants with or without respiratory distress syndrome (RDS) in reducing adverse neonatal outcomes.
SEARCH METHODS: The Cochrane Central Register of Controlled Trials (CENTRAL) in The Cochrane Library, MEDLINE, EMBASE, CINAHL, Clinicaltrials.gov and Controlled-trials.com were

In recent years, interest has been focused to the study of the two major inositol stereoisomers: myo-inositol (MI) and d-chiro-inositol (DCI), because of their involvement, as second messengers of insulin, in several insulin-dependent processes, such as metabolic syndrome and polycystic ovary syndrome. Although these molecules have different functions, very often their roles have been confused, while the meaning of several observations still needs to be interpreted under a more rigorous physiological framework. With the aim of clarifying this issue, the 2013 International Consensus Conference

BACKGROUND: Management of problem drinking presents the general practitioner with similar challenges and rewards to those associated with the management of other chronic conditions.
OBJECTIVE: This article presents a framework for managing alcohol problems in general practice based on national guidelines for the treatment of alcohol problems.
DISCUSSION: General practitioners are well placed to undertake the management of drinking problems following an assessment of the amount of alcohol taken and the risks this poses for the individual and the people around them. This assessment starts the

Brown-Vialetto-Van Laere syndrome (BVVLS) is a genetic condition caused by a mutation in the C20orf54 gene, which also codes for an intestinal riboflavin transporter. We report the case of a female who presented at 22 months with acute-onset stridor and generalized muscle weakness, in whom a genetic diagnosis of BVVLS was made, and whose symptoms improved on therapy with high-dose riboflavin. She had previously been developing normally and was able to walk at 11 months, then developed progressive muscle weakness at 22 months, and within 2 weeks was unable to sit without support. She also demon

Medium-chain Acyl-CoA dehydrogenase deficiency (MCAD) is one of the most common fatty acid oxidation disorders. Clinical manifestations can be serious and lead to death if unrecognized. They are not specific and can mimic meningitis or an acute intestinal intussusception in its neurological form. Early recognition of MCAD and presymptomatic treatment of intercurrent illness improve the prognosis over the short- and long-term. MCAD deficiency satisfies the major criteria for newborn screening. We report the cases of 2 patients whose presentation was typical and severe. Early diagnosis of MCAD d

Neural tube defects (NTDs) are serious birth defects that result from the failure of the neural tube to close in the cranial region (anencephaly) or more caudally along the spine (spina bifida) by the 28th day of gestation. Infants born with anencephaly usually die within a few days of birth, and those with spina bifida have life-long disabilities with varying degrees of paralysis. Currently, identified risk factors for NTDs include a mother who previously had an NTD-affected pregnancy, maternal diabetes, obesity, hyperthermia, certain antiseizure medications, genetic variants, race/ethnicity,

The objectives of this study were to examine the effect of thiamine immersion of fish from a population known for compromised survival as a result of early mortality syndrome (EMS) and to investigate the cause-response relationship between thiamine concentration and lesions in tissues in swim-up-stage lake trout Salvelinus namaycush alevins. Lake trout eggs from 14 fish from Lake Michigan were artificially fertilized and the progeny divided into two groups based on the thiamine concentration (low [< 0.73 nmol/g] or high [> 0.85 nmol/g]) in the unfertilized eggs. Progeny were treated or not wit

The Health Council of the Netherlands recommends fortifying bread with folic acid to ensure that women have a basic level of folic acid intake around the time of conception in order to prevent neonatal neural tube defects. This advice should not be followed because Dutch children who already have adequate intake may easily be overdosed. Sixteen million people will be subjected to lifelong folic acid intake to prevent neural tube defects in 30 of the 200,000 babies born each year. A long-term investment in education and care targeting immigrant women is a much more effective and efficient alter

Treatment of the genetic metabolic myopathies remains generally unsatisfactory with the exception of a select few. Multiple Acyl Co-A Dehydrogenase Deficiency (Glutaric Aciduria type II), in particular, has been shown to respond well to riboflavin supplementation. Recently, studies have also confirmed the effectiveness of recombinant enzyme replacement therapy for Acid Maltase Deficiency (Pompe's Disease). Accurate and early diagnosis of these diseases is vital to prevent serious complications and impaired recovery following delayed treatment.

Hypoglossal nerve paralysis is a rare complication of endotracheal intubation for general anesthesia. Anesthesiologists should be aware of the possible causes and should take extreme care to prevent nerve injuries and other complications. We present a case of postoperative unilateral hypoglossal nerve palsy after general anesthesia for laparoscopic cholecystectomy.

This case report describes the diagnostic trial of an inspiratory wheeze in a 1.5-year-old Swiss Alpine goat. Left grade 4 laryngeal hemiplegia was diagnosed via laryngoscopy, whereas the severity of the hemiplegia was assessed according to the grading system used in horses. The results of clinical, radiographic, sonographic and endoscopic examinations as well as haematological, biochemical and serological analyses did not reveal the cause of the hemiplegia. Treatment with an antibiotic and vitamin B complex resulted in only slight improvement. A postmortem examination four months later reveal…

BACKGROUND: Dietary supplementation with folic acid and vitamin B12 lowers blood homocysteine concentrations by about 25% to 30% in populations without routine folic acid fortification of food and by about 10% to 15% in populations with such fortification. In observational studies, 25% lower homocysteine has been associated with about 10% less coronary heart disease (CHD) and about 20% less stroke.
METHODS: We reviewed the design and statistical power of 12 randomized trials assessing the effects of lowering homocysteine with B-vitamin supplements on risk of cardiovascular disease.
RESULTS:

Dietary Reference Intakes (DRIs) represent the new approach adopted by the Food and Nutrition Board to providing quantitative estimates of nutrient intakes for use in a variety of settings, replacing and expanding on the past 50 years of periodic updates and revisions of the Recommended Dietary Allowances (RDAs). The DRI activity is a comprehensive effort undertaken to include current concepts about the role of nutrients and food components in long-term health, going beyond deficiency diseases. The DRIs consist of 4 reference intakes: the RDA, which is to be used as a goal for the individual;

Rice is the staple food in many countries of Asia. Recent nutrition surveys in eight states, conducted by the National Nutrition Monitoring Bureau of India, show that though the average energy intake is adequate, more than 50% of the households surveyed consumed less than the Recommended Dietary Allowance (RDA) of energy. These households generally had per capita incomes of less than Rupees 2/- (US+ 0.25) per day. The average intake of vitamin A was only 42% of the RDA and that of riboflavin, 70% of the RDA. The average intake of other nutrients such as thiamin, niacin, ascorbic acid, iron and
