An infant born with preaxial polydactyly to a mother taking azathioprine throughout pregnancy is described. Experimental studies in animals reveal a teratogenic role for azathioprine and its main metabolite, 6-mercaptopurine. The skeletal system appears to be the primary tissue target for such action of these drugs. Although no direct cause-effect relationship can be established from the single case presented, the similarity of this anomaly to experimental azathioprine teratogenesis suggests the necessity for further surveillance.

Two groups of patients with polymyositis have been followed for approximately 3 years. One group was treated with prednisone alone and the other with prednisone plus azathioprine. Although the polymyositis of both groups has improved, no statistically significant difference was noted at the end of 3 months, as previously reported. Longer followup, however, has shown that the group given prednisone plus azathioprine has improved more with respect to functional disability; this group also requires less prednisone for disease control.

Three recipients of kidney allotransplants developed dysmyeloproliferative syndromes which were fully reversible after switching from azathioprine to cyclosporin A for immunosuppression. Similar bone marrow changes described in the literature progressed to leukemia. Whether the abnormalities observed in our patients could be early stages of the disease described in the literature and whether a fatal development can be prevented by changing the immunosuppressive therapy remains to be elucidated.

Severe anaemia is a rare, unexplained, side effect of azathioprine therapy. We report here such a case associated with a previously unreported abnormality in azathioprine metabolism. A 57 year old man on azathioprine therapy, for a presumed collagen vascular disease, developed severe megaloblastic anaemia. This resolved on cessation of azathioprine treatment. A very high concentration of an azathioprine metabolite, 6-thioguanine nucleotide, was found in the patient's red blood cells and this was confirmed by subsequent rechallenge with a single dose of 50 mg azathioprine.

There are only few studies with conflicting results on pregnancy outcome after paternal exposure to azathioprine or 6-mercaptopurine. In our study, pregnancy outcome of 115 prospectively followed pregnancies after paternal exposure to azathioprine or 6-mercaptopurine is compared to a control group of 341 pregnancies. The rate of major malformations was not increased (3.0% in exposed versus 2.2% in the control group). We report two cases of patients who developed Beau's lines while taking azathioprine. In each case, the patient developed a hypersensitivity reaction to the medication. These are the first reported cases of a hypersensitivity reaction to azathioprine leading to Beau's lines.

OBJECTIVE: Azathioprine (AZA) is frequently used in Crohn's disease (CD) therapy. This paper aimed to evaluate the frequency, evolution and management of AZA side effects in CD patients.
METHODS: One hundred and six CD patients under AZA therapy were evaluated prospectively from January 2002 to December 2006.
