Each year, millions of Americans suffer bone fractures, often requiring internal fixation. Current devices, like plates and screws, are made with permanent metals or resorbable polymers. Permanent metals provide strength and biocompatibility, but cause long-term complications and may require removal. Resorbable polymers reduce long-term complications, but are unsuitable for many load-bearing applications. To mitigate complications, degradable magnesium (Mg) alloys are being developed for craniofacial and orthopedic applications. Their combination of strength and degradation make them ideal for

Stable fixation of small proximal pole nonunions of the scaphoid by conventional techniques of bone grafting remains difficult. Because of this, long-lasting immobilisation is necessary. Treatment by retrograde Herbert-screw fixation offers the possibility of stable osteosynthesis resulting in shorter immobilisation. Even in case of avascular proximal fragments, it allows healing of the nonunion. We report on the 29 months follow-up of 23 patients treated in this manner showing bony consolidation in 17 cases (74%). Four patients demonstrated fibrous nonunion with minor symptoms, and in two pat

A biomechanical study was performed to investigate the relationship among the pullout strength of anterior vertebral screw fixation, bone mineral density (BMD) of the vertebral body, screw insertion torque, and width of the vertebral body. BMD was measured using a dual energy x-ray absorptiometry unit. Screw insertion torque and width of the lumbar vertebrae were also measured before the pullout test. Pullout strength was significantly correlated with BMD (r = 0.85) and screw insertion torque (r = 0.47), but not with width of the vertebral body. Multiple regression analyses demonstrated better

Twenty cases of Anderson and d'Alonzo type II and "shallow" type III fractures of the dens were treated by anterior screw fixation: the results were reviewed and compared with previously published results obtained in series of such fractures treated non-surgically, by posterior C1-C2 arthrodesis or anterior screw fixation. The complication rate of 25% in our series is comparable to those reported in a previously published review of studies on posterior wiring for C1-C2 arthrodesis. Three of our cases in which complications occurred (15%) were recognized in retrospect as inappropriate for the u

PURPOSE: Increasing numbers of spine surgery cases and an aging society led to challenges during revision surgery with osteoporotic bone. Safe and stable operative techniques are required.
METHODS: In this ex vivo study, human lumbar vertebrae were prepared and then screened for assessing bone density. Eighteen donors (65 vertebrae) with osteoporotic conditions were included. Primary dorsal instrumentation was performed using either standard pedicle screws (6.35 mm diameter; Group A, n = 50) or vertebroplasty screws (VT, 6.35 mm; Group B, n = 15). This was followed by a standardized pull-out

AIMS: Large bone defects resulting from osteolysis, fractures, osteomyelitis, or metastases pose significant challenges in acetabular reconstruction for total hip arthroplasty. This study aimed to evaluate the survival and radiological outcomes of an acetabular reconstruction technique in patients at high risk of reconstruction failure (i.e. periprosthetic joint infection (PJI), poor bone stock, immunosuppressed patients), referred to as Hip Reconstruction In Situ with Screws and Cement (HiRISC). This involves a polyethylene liner embedded in cement-filled bone defects reinforced with screws a

BACKGROUND: The BONEBRIDGE (MED-EL, Innsbruck, Austria) is a bone-conduction implant used in the treatment of conductive and mixed hearing loss. The BONEBRIDGE consists of an external audio processor and a bone-conduction floating mass transducer that is surgically implanted into the skull in either the transmastoid, retrosigmoid or middle fossa regions. The manufacturer includes self-tapping screws to secure the transducer; however, self-drilling screws have also been used with success. In cases where the skull is not thick enough to house the transducer, lifts are available in a variety of s

BACKGROUND CONTEXT: Dramatic increases in the average life expectancy have led to increases in the variety of degenerative changes and deformities observed in the aging spine. The elderly population can present challenges for spine surgeons, not only because of increased comorbidities, but also because of the quality of their bones. Pedicle screws are the implants used most commonly in spinal surgery for fixation, but their efficacy depends directly on bone quality. Although polymethyl methacrylate (PMMA)-augmented screws represent an alternative for patients with osteoporotic vertebrae, their

BACKGROUND: Irradiation results in impaired bone healing. Thus, osteosynthesis procedures are afflicted with increased failure rates. To improve osseointegration bone morphogenetic protein-2 (BMP-2) immobilized on nanocrystalline diamond (NCD)-coated implant surfaces might be 1 solution.
METHODS: By 4 weeks after irradiation of pig's mandible with a dose of 60 Gy a fracture was accomplished. Osteosynthesis was performed either with titanium osteosynthesis screws or NCD-coated screws with immobilized BMP-2. Nonirradiated animals served as control. After 1, 2, 4, and 8 weeks screws were evaluat

INTRODUCTION: Although intramedullary screw fixation likely leads to successful union of Jones fractures compared to that of nonoperative treatments, bony union disorder after surgical treatment remains to be elucidated.
METHODS: Intramedullary screw fixation was performed for the surgical treatment of proximal fifth metatarsal stress fractures in this series. Between January 2008 and December 2019, the feet of 222 patients were investigated regarding the effective factors for postoperative bony union between the normal union group and the bony union disorder group according to the patients'

PURPOSE: This study aimed to evaluate whether bone density-optimized (BDO) pedicle screw trajectories improve screw stability compared to standard trajectories in spinal fusion surgery. The central research question was whether preoperative planning based on bone mineral density (BMD) can enhance screw fixation strength and potentially reduce the risk of loosening.
METHODS: A biomechanical cadaveric study was conducted using thirty human vertebrae. Each vertebra was instrumented with one pedicle screw using a standard trajectory and another using a BDO trajectory. Two trajectory types were te
