OBJECTIVES: Natural microbial defence systems, such as bacteriocins, may be a novel means to prevent catheter-associated urinary tract infection. Suprapubic catheterization of the bladder is used as a short- or long-term alternative to urethral catheterization. As with any indwelling urinary catheter, correct insertion, care and removal are vitally important to minimize problems. A particular problem that affects suprapubic catheters is 'cuffing', which on its own or combined with encrustation can potentially cause a great deal of difficulty on removal or discomfort for the patient. This article discusses the causes of cuffing, and suggests using catheters with integral balloons to reduce the incidence of the problem.

Prolonged venous access devices are needed in cancer patients for central venous access. Catheter fragmentation leading to catheter malfunction represents a rare problem. Herein we present our experience in the management of fragmented catheters. Between 2001 and 2003, 183 catheters were placed via the subclavian vein, and five cases of fragmented catheters were observed. The aim of this study was to report catheter malposition during voiding cystourethrography. Eight hundred forty-three voiding cystourethrography (265 males and 578 females, aged 1 week to 12 years, mean age 2 years) were performed during a period of 4 years. We reviewed the medical records of 34 consecutive children with reflux who underwent simple ureteroneocystostomy at our institution and an affiliated hospital between 1991 and 1992. Fourteen patients were managed with ureteral and urethral catheters during the initial postoperative period, and 20 patients were managed with a "catheterless" technique employing neither of these devices. The latter group had a 50 percent decrease in length of hospital stay with 20 percent decrease in hospital costs when compared with the former. We have assessed the percutaneous insertion of Hickman catheters implanted directly into the subclavian vein; 116 catheters were inserted in 86 patients. The catheters were all inserted by members of the haematology staff. The authors present their experience with pneumatic nasal catheters in cases of epistaxis. Among the advantages of this method over tamponade with petroleum jelly-covered gauze are: Effective tamponade without causing hypoxia, carbon dioxide retention or additional loading of the cardiopulmonary system; Less need for sedation; and Easy application and removal. Conversely, its most important adverse effect is the possibility of pressure necrosis of the nasal mucosa and nasal septum. This can be greatly reduced by considering a few technical recommendations.

Two cases of fractured polyethylene double-pigtail ureteral catheters in pelvic cancer patients are reported. Current literature leaves a large void in guiding the management of these catheters. Surveillance techniques to help avoid complications in patients requiring these catheters are suggested.

The dual lumen catheter has recently been developed as a subclavian vascular access device for haemodialysis. This paper describes our preliminary experience with two currently available catheters of this design: the Shiley Dual Lumen Catheter, and the Quinton-Mahurkar Dual Lumen Catheter. The performance of both catheters, characterised by high blood flow rate capabilities, low venous resistances and minimal recirculation, is superior to that of other types of subclavian haemodialysis cannula.

During a 12-month period, the use of a subclavian vein Uldall catheter (UC) for hemodialysis or plasmapheresis in 27 patients was studied prospectively. Ten patients had ten UC site infections. Organisms associated with these infections included Staphylococcus epidermidis (five), Staphylococcus aureus (four), Proteus mirabilis (two), and Enterococcus (one).
