A middle aged male had botryomycosis affecting the glans penis. There were macroscopic granules that on Gram staining showed masses of staphylococci.Staphylococcus aureus was isolated from the lesion. There was prompt response to surgical removal of the affected tissue in conjunction with systemic cloxacillin.

We describe a patient admitted to the intensive care unit following a penetrating head injury. This patient developed agranulocytosis and subsequently pancytopenia secondary to an idiosyncratic drug reaction. The possible causative drugs are phenytoin, cloxacillin, ceftriaxone and ceftazidime. Toxic epidermal necrolysis due to vancomycin is reported in a patient with human immunodeficiency virus infection. The same patient had anaphylaxis to cloxacillin but tolerated other penicillin derivatives. These reactions were documented using in vivo and in vitro tests. The role of human immunodeficiency virus infection in the pathogenesis of these reactions is discussed.

Although ampicillin has been suggested as a useful agent for the treatment of nocardiosis in man, little is known regarding the presence of beta-lactamase in Nocardia or its possible role in determining resistance to ampicillin and the other beta-lactam antibiotics. We describe a case of monoarthritis in an Inuit seal hunter which progressed to joint destruction of the distal interphalangeal joint despite early and aggressive sequential antibiotic therapy with ampicillin, cloxacillin and penicillin G. The incorporation of 2.5 micrograms/ml of cloxacillin into 5% bovine blood agar provided an inexpensive, easily prepared culture medium for the primary isolation of Moraxella bovis from bovine lacrimal and nasal secretions. With this medium, the time required to identify and isolate M. bovis from large numbers of field specimens was substantially reduced, whereas the sensitivity of isolation was increased by 60%.
