Downstream thoracic endovascular aortic repair following the frozen elephant trunk procedure.

The frozen elephant trunk technique has become a well-established treatment option for patients presenting all thoracic aortic pathologies including acute and chronic dissection, aortic aneurysms and even penetrating aortic ulcers involving the aortic arch and descending aorta. Nevertheless, there is a significant incidence of and risk for distal aortic reinterventions after the frozen elephant trunk. Indications mainly include a planned staged approach, diameter progression of downstream aortic segments and the development of distal stent-graft induced new entries (dSINEs). Endovascular stent

Delayed Endovascular Repair With Procedural Anticoagulation: A Safe Strategy for Blunt Aortic Injury.

BACKGROUND: Blunt aortic injury (BAI) and traumatic brain injury (TBI) are the leading causes of death after blunt trauma. The purposes of this study were to identify predictors of mortality for BAI and to examine the impact of procedural heparinization during thoracic endovascular aortic repair (TEVAR) on neurologic outcomes in patients with BAI/TBI. METHODS: Patients with BAI were identified over an 8 year period. Age, gender, severity of injury and shock, time to TEVAR, morbidity, and mortality were recorded and compared. Multivariable logistic regression (MLR) was performed to determine i

Patterns of procedure-specific radiation exposure in the endovascular era: impetus for further innovation.

OBJECTIVE: Although patient preference and outcome data support continued development and use of minimally invasive endovascular therapies, only a few studies have documented radiation exposure to the patient. This report summarizes patient radiation exposure by endovascular procedure at Cleveland Clinic. METHODS: A retrospective review was undertaken of all endovascular procedures during a 30-month period. Procedures were categorized as infrarenal aortic aneurysm (IAA), isolated thoracic aneurysm (TA), visceral occlusive intervention, renal artery intervention, cerebrovascular intervention, …

Carbon dioxide as a promising alternative to iodinated contrast media for lower extremity endovascular procedures (Review).

Carbon dioxide (CO2) angiography has emerged as an alternative contrast agent for arterial endovascular procedures, particularly in patients at increased risk of complications from iodinated contrast media (ICM). The present review evaluates the clinical circumstances in which carbon dioxide (CO2) angiography can reduce the use of iodinated contrast media (ICM) during arterial endovascular procedures, with particular attention to renal-risk reduction, image quality, anatomical limitations, and procedure-related complications. A narrative literature review was performed using PubMed, EMBASE, th…

Staging Strategies During Complex Endovascular Aortic Procedures to Minimize Spinal Cord Ischemia Rates: A Narrative Review.

Endovascular repair of thoracoabdominal aortic aneurysms (TAAAs) requires multidisciplinary expertise to minimize mortality and disabling complications. Despite satisfactory outcomes with this approach being common knowledge, extensive aortic coverage occurring in fenestrated/branched endovascular aortic repair (F/B-EVAR) carries a non-negligible risk of spinal cord ischemia (SCI). Recently, many authors have proposed different endovascular strategies to mitigate the risk of SCI; however, the real effectiveness of these maneuvers is not universally recognized due to a lack of standardized prot

Post-Implantation Syndrome Incidence is Higher After Complex Endovascular Aortic Procedures Than After Standard Infrarenal Repair.

OBJECTIVE: Post-implantation syndrome (PIS), characterised by malaise, fever, and increased inflammatory markers, is a common occurrence after endovascular aneurysm repair (EVAR), causing prolonged hospitalisation and increased cost. This study aimed to determine the incidence and short-term outcomes of PIS after fenestrated or branched procedures in aorto-iliac aneurysms compared with standard EVAR. METHODS: A retrospective, comparative study from a tertiary academic institution was undertaken. All patients who underwent elective EVAR with polyester stent grafts from January 2015 to June 202

[Aspects of antithrombotic and anticoagulant therapies in patients undergoing endovascular procedures for lower extremity arterial disease].

The aging of the population, the increasing prevalence of important risk factors of atherosclerosis, like diabetes, and the declining mortality of acute cardiovascular conditions lead to increased peripheral arterial disease incidence. At the same time, cases showing a severe decline in quality of life or danger of limb loss get more prevalent that demands vascular interventions. In such cases, revascularization is recommended, and consequently, in the last decade, an expansion in endovascular procedures can be seen. Considering these procedures, besides a technical success, the risk of new st…

EVALUATION OF RADIATION DOSES USING CONE BEAM COMPUTED TOMOGRAPHY IN ENDOVASCULAR AORTIC REPAIR AND SCOLIOSIS PROCEDURES.

The aim of this project was to evaluate the radiation dose to patients for cone beam computed tomography (CBCT) in endovascular aortic repair (EVAR) and scoliosis procedures and to compare radiation doses between CBCT and computed tomography (CT). An anthropomorphic phantom and Siemens and General Electric X-ray equipment were used. Default protocol settings were used for comparison of protocols and modalities. The ratio between the highest and lowest CBCT effective dose, for each equipment, had a maximum of 13 (Artis Pheno) for EVAR and 1.8 (Artis Zeego) for scoliosis. It is difficult to pred

Standard procedures of endovascular treatment for vascular access stenosis in our facility - clinical usefulness of ultrasonography.

In Japan, the number of patients receiving dialysis is 314,180 at the end of 2013 and 97% are treated with hemodialysis. And the mean age of patients and the percentage of diabetes have been increasing. For this reason, preparations of a new vascular access (VA) and its long-term maintenance have become difficult. In the guidelines by the Japanese Society for Dialysis Therapy (JSDT), endovascular treatment (ET) is positioned as the first line for VA stenosis. The procedure of ET itself is very simple. The revision of Japanese health insurance set an expensive technical fee for ET in 2012. It a

Prediction of endovascular leaks after thoracic endovascular aneurysm repair though machine learning applied to pre-procedural computed tomography angiographs.

To predict endoleaks after thoracic endovascular aneurysm repair (TEVAR) we submitted patient characteristics and vessel features observed on pre- operative computed tomography angiography (CTA) to machine-learning. We evaluated 1-year follow-up CT scans (arterial and delayed phases) in patients who underwent TEVAR for the presence or absence of an endoleak. We evaluated the effect of machine learning of the patient age, sex, weight, and height, plus 22 vascular features on the ability to predict post-TEVAR endoleaks. The extreme Gradient Boosting (XGBoost) for ML system was trained on 14 pati

3D CT to 2D X-ray image registration for improved visualization of tibial vessels in endovascular procedures.

PURPOSE: During endovascular revascularization interventions for peripheral arterial disease, the standard modality of X-ray fluoroscopy (XRF) used for image guidance is limited in visualizing distal segments of infrapopliteal vessels. To enhance visualization of arteries, an image registration technique was developed to align pre-acquired computed tomography (CT) angiography images and to create fusion images highlighting arteries of interest. METHODS: X-ray image metadata capturing the position of the X-ray gantry initializes a multiscale iterative optimization process, which uses a local-v

Indications for the Performance of Intracranial Endovascular Neurointerventional Procedures: A Scientific Statement From the American Heart Association.

Intracranial endovascular interventions provide effective and minimally invasive treatment of a broad spectrum of diseases. This area of expertise has continued to gain both wider application and greater depth as new and better techniques are developed and as landmark clinical studies are performed to guide their use. Some of the greatest advances since the last American Heart Association scientific statement on this topic have been made in the treatment of ischemic stroke from large intracranial vessel occlusion, with more effective devices and large randomized clinical trials showing strikin
