We describe a case of erythema multiforme in a 2-week-old boy. He had no remarkable antecedents, and a polymerase chain reaction-based technique failed to detect herpes simplex virus DNA in the skin biopsy specimen. To our knowledge, only one previous biopsy-proven case of erythema multiforme during the neonatal period has been reported.

Oral erythema multiforme (EM) is considered as a third category of EM other than EM minor and major. Patients present with oral and lip ulcerations typical of EM but without any skin target lesions. Classically described as target-like, the erythema multiforme lesions can be isolated, recurrent, or persistent. Erythema multiforme (EM) is an acute self-limited eruption with multiple possible precipitators including drugs, infections, and in a small percentage, vaccinations. The lesions of EM can vary in presentation and are frequently misdiagnosed. With the reinstitution of the smallpox vaccination program for deploying service members, there is an increased need for military providers to be aware of the adverse reactions to be expected with this vaccination. We present the case of a 19-year-old active duty male who developed EM 14 days after receiving vaccination for smallpox.
