Evaluation of sit-to-stand motion using a pressure distribution measurement system--effect of differences in seat hardness on sit-to-stand motion.

PURPOSE: This study examined differences between standing from an ordinary seat and standing from a low-repulsion mat using a pressure distribution measurement system (BIG-MAT) and identified parameters to determine sit-to-stand (STS) motion difficulty. METHOD: Ten healthy male volunteers (aged 30-38 years) participated. During STS motion from an ordinary seat and from a low-repulsion urethane mat, plantar surface pressure changes of both feet and the centre of pressure (COP) trajectory were recorded for 7  s. This series of tests was performed four times in each subject. Left and right press

Is there a relationship between forefoot alignment and ingrown toenail? A case-control study.

Biomechanical imbalance between the hallux and the second toe and external compression of the lateral nail fold are proposed as the main reasons for ingrown toenail. Therefore, any anatomical predisposition facilitating this compression may play a role in the occurrence of ingrown toenail. The purpose of this study is to investigate the relationship between forefoot alignment and ingrown toenail. The authors retrospectively reviewed radiographs of 81 patients with ingrown toenails and compared them with 100 healthy subjects regarding forefoot alignment. Hallux valgus angle (HVA), intermetatars

Surgical approaches to the forefoot for common sports-related pathologies: a review of the literature and cadaveric dissection.

This article reviews the published literature regarding the surgical approaches to pathologies encountered within the forefoot, including arthroscopic and open techniques, and their treatment. We have demonstrated these surgical approaches with cadavers, to identify the key anatomical landmarks and safe zones for these surgical techniques, to prevent the complications associated with their treatment.

Freiberg's disease as a rare cause of limited and painful relevé in dancers.

Freiberg's disease, or osteonecrosis of the second metatarsal head, is an uncommon cause of forefoot pain that can severely limit a dancer's relevé. Dancers may be predisposed to the condition due to repetitive microtrauma to the ball of the foot during routine dance movements. Freiberg's disease is diagnosed by history, physical examination, and plain film radiographs. Conservative treatment in dancers is disappointing, and surgical options fail to produce uniformly good results. Previously published reports of successful surgical outcomes would, for a dancer, result in an unacceptable loss o

Reliability of the Maestro radiographic measuring tool.

INTRODUCTION: Maestro et al. presented a detailed preoperative measuring and classification technique for the forefoot. The purpose of this paper was to determine if the PACS system will allow the Maestro measuring technique and classification system to be reliable and precise. MATERIALS AND METHODS: This radiographic study was conducted on 73 subjects (36 females, 37 males, age 30.4 ± 9.9) who had given informed consent. The geometrical progression was measured for each foot of each subject by the two observers according to the measuring methodology of Maestro. The intraclass correlation coe

The transverse forefoot arch demonstrated by a novel X-ray projection.

OBJECTIVE: Metatarsalgia is often treated by metatarsal osteotomy. Exact knowledge of the normal anatomy of the forefoot is essential for pre-operative planning. The objective of this study was to investigate the forefoot arch during maximal loading in a randomly selected population sample. METHODS: Two hundred subjects randomly selected from a municipality representative of Denmark were invited to interview and forefoot X-ray examination, including a novel horizontal X-ray projection by which the height of each metatarsal from the floor can be measured under maximal loading. RESULTS: One hu

Effect of rearfoot posts in reducing forefoot forces. A single-subject design.

The purpose of this study was to assess the effectiveness of a semirigid foot orthosis with a varus wedge on forefoot vertical forces in a 24-year-old female with a compensated rearfoot varus deformity. The results of this study indicate that the use of total contact semirigid foot orthoses reduces the forefoot force-time integral during walking, whether a rearfoot varus wedge was or was not used. The authors recommend that total contact construction of the foot orthoses be considered when a reduction of the forces acting on the forefoot is the goal of treatment.

Solid-Cystic Hidradenoma: A Dorsal Foot Presentation.

Solid-cystic hidradenoma is a benign cutaneous tumor with eccrine sweat gland origins that is most commonly found in the head, neck, trunk, and upper extremity regions of patients in the middle to older age groups. These lesions are generally asymptomatic, slow-growing, solitary, and nonulcerative in presentation. Degenerative malignant transformation of this tumor is very rare. In this case report, the authors describe the marginal surgical excision and subsequent microscopic pathologic diagnosis of a moderate sized solid-cystic hidradenoma of the dorsolateral forefoot in a middle-aged male p

[Degenerative forefoot : Diagnostics and treatment].

The hallux valgus deformity is the most common toe deformity of the forefoot and is often associated with a splayfoot. Malpositioning of the small toes may be isolated but are more common in other foot deformities. The understanding of the complex pathoanatomy of the foot is necessary for orthopedic treatment. Conservative treatment is reserved for the early stages. The indications for surgery should be based on clinical and radiographic findings. Countless surgical procedures are available and minimally invasive surgical techniques are also increasingly being used.

Ankle and knee kinetics between strike patterns at common training speeds in competitive male runners.

The purpose of this study was to investigate the interaction of foot strike and common speeds on sagittal plane ankle and knee joint kinetics in competitive rear foot strike (RFS) runners when running with a RFS pattern and an imposed forefoot strike (FFS) pattern. Sixteen competitive habitual male RFS runners ran at two different speeds (i.e. 8 and 6 min mile(-1)) using their habitual RFS and an imposed FFS pattern. A repeated measures analysis of variance was used to assess a potential interaction between strike pattern and speed for selected ground reaction force (GRF) variables and, sagitt

Effect of a steel toe cap on forefoot injury pattern in a cadaveric model.

BACKGROUND: Crush injuries to the foot are a common workplace injury and a significant source of morbidity, disability and lost wages. Many regulatory bodies including the Occupational Safety and Health Administration (OSHA) recommend the use of safety shoes in certain occupations to help protect against these occupational hazards. However there remains controversy and paucity of published data regarding the protection afforded by a steel toe cap in regards to clinical injury pattern. This study looks to investigates the protective influence of a steel toe cap on crush injuries of the forefoot

[Biomechanics of the foot--forces in the forefoot during walking and their clinical relevance].

Pathologic conditions of the toe and metatarsophalangeal (MP) joints, such as the commonly encountered hallux valgus but also the relatively rare idiopathic necrosis of the metatarsal heads (Freiberg-Koehler's disease) and many other disorders of the foot, call for precise investigation of the kinematic and dynamic behavior of the foot during walking. The success of surgery performed on bone or soft tissues in the forefoot depends on an understanding of the biomechanics of the region. Therefore, the plantar forces acting under the metatarsal heads of the 1st, 2nd and 5th rays and under the pad

Methods for providing vascularized tissue protection of microanastomoses.

Perianastomotic thrombosis following free tissue transfer must be systematically prevented if even routine successes are to be expected. One possible culprit that must be considered is the method of soft tissue closure around the recipient site pedicle that frequently poses a challenge to avoid tension and the resultant compression that may then predispose to such thrombosis. One hundred sixteen consecutive free tissue transfers from 1982 to 1990 were analyzed; simple local soft tissue manipulations to provide adequate coverage were satisfactory in 67% of cases. Local flaps (5%) or extensions

Rethinking Morton's neuroma: a sonographic study of bilateral prevalence and symmetry.

BACKGROUND: Morton neuroma (MN) is commonly regarded as a focal, unilateral condition. Whether sonographically detectable lesions also occur contralaterally to the symptomatic side has not been defined systematically. We aimed to determine the bilateral sonographic prevalence, anatomic distribution, symmetry, and size characteristics of lesions consistent with MN in patients referred with clinical suspicion of MN. METHODS: In this cross-sectional study, we included all consecutive patients referred between January 2023 and December 2024 for suspected MN. All underwent standardized bilateral h

Foot pronation during walking is associated to the mechanical resistance of the midfoot joint complex.

BACKGROUND: The demonstration of the relationship between midfoot passive mechanical resistance and foot pronation during gait may guide the development of assessment and intervention methods to modify foot motion during gait and to alter midfoot passive mechanical resistance. RESEARCH QUESTION: Is foot pronation during the stance phase of gait related to the midfoot passive mechanical resistance to inversion? METHODS: The resistance torque and stiffness provided by midfoot soft tissues of 33 participants (21 females and 12 males) with average of 26.21 years were measured. In addition, the p

Post-operative stress fractures complicating surgery for painful forefoot conditions.

A stress fracture is caused by repetitive or unusual loading of a bone leading to mechanical failure. Fatigue type stress fractures occur in normal bone exposed to abnormally high repetitive loads, whereas insufficiency type stress fractures occur in abnormal bone exposed to normal loads. We describe three cases of insufficiency stress fractures that have complicated surgery for painful forefoot conditions. The diagnosis and management of these cases are discussed. Stress fractures should be included in the differential diagnosis of any patient who continues or develops pain after surgery to t
