Congenital dislocation of the hip treated by total hip arthroplasty.

Thirty-two total hip arthroplasties (THAs) were performed on 26 severely incapacitated patients with congenital dislocation of the hip. The arthroplasty technique varied with the problems encountered, but the cup was always placed in the true acetabulum. In 19 cases, femoral head autografts were transplanted. The overall results were satisfactory. Roentgenologic evaluation of the bone grafts showed no nonunion or secondary displacement. Nineteen previously invalid patients now lead normal lives. Five patients are minimally incapacitated and only two were unimproved by the operation. THA with a…

Importance of the clicking hip in screening for congenital dislocation of the hip.

A birth cohort of 3289 babies was investigated prospectively for one year to assess the importance of risk factors for congenital dislocation of the hip (CDH). 426 (13%) babies had an identifiable risk factor when examined by paediatric senior house officers. On further examination in the first week of life at a screening clinic, 51 proved to have CDH, an incidence of 15.5 per 1000 births. The results confirmed the importance of established factors such as family history, caesarean section, and the breech position, and demonstrated the clinical significance of the presence of clicking hips on …

Congenital dysplasia of the hip: the lax (subluxatable) newborn hip.

During a 4-year period, 15,149 newborns were examined for hip abnormalities. Ninety-two infants (0.61%) were found to have subluxatable (lax, but not dislocatable) hips. The profile of this group was similar to patients with dislocated hips, with an increased frequency of females, breech presentations, left-sided involvement, and associated postural deformities. A significant number showed acetabular dysplasia or persistent hip laxity despite treatment with an abduction brace. We concluded that the newborn subluxatable hip arises from the same antecedents as frank dislocation, is not always be…

Hip dysplasia associated with abduction contracture of the contralateral hip.

Eighteen children with unilateral dysplasia of the hip were found to have asymmetrical gluteal folds and an apparent limb-length inequality secondary to pelvic obliquity caused by an abduction contracture of the contralateral hip. The dysplastic hip was on the left in all but one patient. As a result of the pelvic obliquity, the femoral head on the high side of the pelvis was directed superolaterally, stretching the relaxed capsule still farther. The femoral head was then not concentrically reduced, and the pressure it caused on the superior border of the labrum contributed to the development …

Surgical hip dislocation for treatment of synovial chondromatosis of the hip.

PURPOSE: In the present study, we aimed to evaluate the clinical outcomes of surgical hip dislocation in patients with synovial chondromatosis (SC) of the hip. METHODS: Seven patients with primary SC of the hip treated with open synovectomy and removal of loose bodies by surgical hip dislocation from 2016 to 2019 were retrospectively reviewed. All patients had numerous and widespread loose bodies based on pre-operative images, including routine radiographs, CT, and MRI. The visual analog scale (VAS) score and Harris hip score (HHS) were collected and analyzed before and after surgery. The pos…

Fever and Hip Pain: Not Always Due to a Septic Hip.

Hip pain in febrile children always raises concern for septic arthritis. Pyomyositis is a rare cause of hip pain. Because it involves muscles around the joint, it may have a similar presentation as a septic hip. This entity therefore has the potential for posing a diagnostic dilemma when physicians face a limping child with fever. We report a child who presented with fever and hip pain and has pyomyositis of the obturator internus muscle. This case underscores the need to consider the possibility of alternative etiologies in a limping child.

Imaging of the hip: a systematic approach to the young adult hip.

BACKGROUND: Great advances in knowledge and understanding of the biomechanics of the hip, both in arthroscopic procedures and imaging techniques, have expanded and improved the diagnosis of pathologies of the young adult hip. The anatomy of the hip joint is complex due to its morphology and orientation. The inter-pretation of the images requires deep knowledge of the osseous and soft tissue anatomy: muscles, tendons, ligaments, vessels and nerves. There are multiple imaging tools. Diagnostic techniques have different utilities and often are complementary. METHODS: In this article the various …

The Hyperflexible Hip: Managing Hip Pain in the Dancer and Gymnast.

CONTEXT: Dance, gymnastics, figure skating, and competitive cheerleading require a high degree of hip range of motion. Athletes who participate in these sports use their hips in a mechanically complex manner. EVIDENCE ACQUISITION: A search of the entire PubMed database (through December 2013) and additional searches of the reference lists of pertinent articles. STUDY DESIGN: Systematic review. LEVEL OF EVIDENCE: Level 3. RESULTS: Whether innate or acquired, dancers and gymnasts have some hypermobility that allows their hips to be placed in potentially impinging or unstable positions requir…

Hip problems and arthroscopy: adolescent hip as it relates to sports.

Injuries of the hip and pelvis in pediatric athletes are receiving increased attention, which may be due, in part, to the increased participation in competitive sports and focusing on a single sport at younger ages. With the advent of hip arthroscopy and the development of more advanced imaging of the hip through magnetic resonance imaging, internal derangements of the hip such as labral tears, loose bodies, femoroacetabular impingement, and chondral injuries are being diagnosed and treated with increased frequency. This article reviews the more common injuries of the hip and pelvis in pediatr…

[Total hip arthroplasty in the treatment of developmental dysplasia of the hip].

Total hip arthroplasty (THA) is the preferred treatment for patients with severe arthritis of the hip secondary to developmental hip dislocation or dysplasia. However, THA may be difficult due to bone and soft tissue problems that arise from hip dislocation or dysplasia. Another problem is that patients are usually young, which may affect the long-term survival of the prosthesis. Either cemented or uncemented components can be used depending on bone structure and bone stock. Uncemented components are more preferable because of the young age of the patients. From a biomechanical standpoint, the…

Anatomy of the dysplastic hip and consequences for total hip arthroplasty.

Total hip arthroplasty in dysplastic hips is challenging because of the modified anatomy of the proximal femur and acetabulum. We studied three-dimensional anatomic parameters in 247 dysplastic hips from 218 adult patients using radiographs and computed tomography, and analyzed the consequences for total hip arthroplasty. A cohort of 310 primary osteoarthritic hips was used as a control group. According to the classification of Crowe et al, 78 of the dislocated hips were graded Class I, 26 as Class II, 20 as Class III or IV, and 169 dysplastic hips had no subluxation. Compared with primary ost…

[Total hip arthroplasty for treatment of developmental dysplasia of the hip].

OBJECTIVE: To summarize the techniques of the total hip arthroplasty in the treatment of osteoarthritis secondary to developmental dysplasia of the hip joint (DDH). METHODS: Between February 1986 and November 2004, a total of 32 hips in 24 patients with advanced osteoarthritis secondary to DDH underwent the total hip arthroplasty. Among the patients, 4 were male and 20 were female, with their ages ranging from 33 to 59 years and an average age of 47 years. The bilateral arthroplasty was performed in 8 patients and the unilateral arthroplasty in 16 patients. The patients mainly suffered from p…
