Pediatric flexible laryngoscopy: Trends in diagnostic abilities throughout training.

OBJECTIVES: Our objectives were to evaluate the ability of residents to diagnose pathology of the pediatric larynx on laryngoscopy, to trend this ability throughout training, to compare their skills to pediatric otolaryngologists, and to determine whether reviewing digitally captured videos in slow motion, as opposed to a live exam, enhanced diagnostic abilities. In addition, we identified pathologies and anatomical sub-sites that posed diagnostic challenges. METHODS: Qualtrics was used to design and distribute an online test, which included 15 pediatric laryngoscopy videos. Participants sele…

Videolaryngoscopy versus direct laryngoscopy for tracheal intubation in neonates.

BACKGROUND: Establishment of a secure airway is a critical part of neonatal resuscitation in the delivery room and the neonatal unit. Videolaryngoscopy has the potential to facilitate successful endotracheal intubation and decrease adverse consequences of delay in airway stabilization. Videolaryngoscopy may enhance visualization of the glottis and intubation success in neonates. OBJECTIVES: To determine the efficacy and safety of videolaryngoscopy compared to direct laryngoscopy in decreasing the time and attempts required for endotracheal intubation and increasing the success rate at first i…

[Unexpectedly complicated laryngoscopy caused by a massive mandibular tori].

Here, we report a case of an unexpectedly complicated laryngoscopy caused by massive mandibular tori. A 64-year-old man with mitral regurgitation and aortic regurgitation was scheduled for a double valve replacement. Thyromental distance and the Mallampati score were used as predictive factors of difficult intubation, and both factors were within the normal range. Anesthesia with controlled ventilation was started with fentanyl, propofol and vecuronium. After the attainment of full muscle relaxation, an experienced anesthesiologist performed direct laryngoscopy. It was not possible to intubate…

Endotrol-tracheal tube assisted endotracheal intubation during video laryngoscopy.

Video laryngoscopes allow indirect visualization of the glottis and provide superior views of the glottis compared to direct laryngoscopes in patients with both normal and difficult airways, but it may be difficult to advance the endotracheal tube (ETT) through the vocal cords into the trachea, unless a stylet is used. We propose that the Endotrol(®) ETT may be an effective tool to facilitate video laryngoscope-assisted orotracheal intubation without the use of a stylet. After obtaining written and oral informed consent, 60-adult patients scheduled for elective surgery requiring general anesth

Cost Savings of Using Fluorescein Strips in Laser Laryngoscopy.

This technical report describes a simple, yet novel, approach to endotracheal cuff inflation in laryngological laser surgery that may result in significant cost savings for hospitals and patients. Pricing information was obtained from our tertiary care hospital's operating room pharmacy as of March 3, 2025. A common safety practice when using carbon dioxide (CO2) or potassium titanyl phosphate (KTP) lasers is to fill the endotracheal cuff balloon with normal saline and methylene blue. The cost of the smallest vial of methylene blue is $150.00, and the entire bottle must be discarded after each

Identification of difficult laryngoscopy using an optimized hybrid architecture.

BACKGROUND: Identification of difficult laryngoscopy is a frequent demand in cervical spondylosis clinical surgery. This work aims to develop a hybrid architecture for identifying difficult laryngoscopy based on new indexes. METHODS: Initially, two new indexes for identifying difficult laryngoscopy are proposed, and their efficacy for predicting difficult laryngoscopy is compared to that of two conventional indexes. Second, a hybrid adaptive architecture with convolutional layers, spatial extraction, and a vision transformer is proposed for predicting difficult laryngoscopy. The proposed adap…

Tongue symptoms, suspension force and duration during operative laryngoscopy.

PURPOSE: Suspension laryngoscopy is a commonly performed procedure in otolaryngology. During the procedure, the laryngoscope applies direct force to the tongue. Postoperative tongue symptoms include pain, swelling, numbness, taste disturbance, and rarely motor deficits. Duration and magnitude of force applied have previously been associated with post-operative throat and tongue pain, respectively. We sought to correlate postoperative tongue symptoms with magnitude of force applied and/or duration of suspension and investigate any risk factors for tongue morbidity. MATERIALS AND METHODS: A sam…

Continuous laryngoscopy quantitates laryngeal behaviour in exercise and recovery.

Exercise-induced laryngeal obstruction (E-ILO) causes exertional dyspnoea. There is no standardised methodology which characterises laryngeal obstruction in relation to exercise or links laryngeal obstruction and dyspnoea severity. Continuous laryngoscopy during exercise (CLE) may improve diagnostic sensitivity by enabling laryngeal visualisation at peak work capacity in patients with rapidly resolving obstruction. The time course of laryngeal obstruction across exercise and recovery has not been quantitated until this report.Adolescents and young adults referred for CLE were laryngoscopically…

[Optical and video-assisted laryngoscopy in difficult paediatric airways].

Despite ongoing technological advances, direct laryngoscopy still remains the golden standard for endotracheal intubation in children and infants. However, in some infants it is almost impossible to obtain a view of glottis and the vocal cords. In this paper we present an overview of the recent advances in optical and video-assisted laryngoscopy in difficult paediatric airways.

A two-dimensional model of anatomic relationships during laryngoscopy.

BACKGROUND: The view obtained during direct laryngoscopy is only seen by a single anesthesiologist. The inability of instructors to observe the view poses problems for teaching the technique. The anatomic interactions affecting laryngoscopy are largely internal, hampering efforts to understand why some patients are unexpectedly difficult to intubate. In response, we have constructed a full scale, adjustable, two-dimensional model showing the head and neck in the sagittal plane. In this article, we validate the mannequin and test how various conditions or changes in equipment affect the laryngo…

Laryngoscopy in thyroid surgery--essential standard or unnecessary routine?

BACKGROUND: Preoperative and postoperative laryngoscopy has been recommended for diagnostic and forensic reasons as a standard procedure in all patients who undergo thyroid surgery. The aim of this study was to find a more selective approach by defining patients at risk of developing vocal fold palsy (VFP). METHODS: The history of neck explorations, results of laryngoscopy, and histology were registered in all patients who underwent thyroid surgery at our institution between 1995 and 1999. Patients with pathologic findings at postoperative laryngoscopy underwent reassessment of voice and voca…

[Video laryngoscopy in the intensive care unit-A current assessment].

Video laryngoscopy (VL) and direct laryngoscopy (DL) are both intubation methods used on the intensive care unit although it is unclear if the routine use of VL is meaningful in this setting. A systematic literature search was carried out up to April 2024 in the data banks MEDLINE, CENTRAL and LIVIVO. Included were studies in which a comparison between VL and DL in adult patients on the intensive care unit was carried out. The primary endpoint was determined as the rate of the first attempt success (FAS). The secondary endpoints were visualization of the glottis according to Cormack-Lehane, th…
