The association between having a medical home and vaccination coverage among children eligible for the vaccines for children program.

BACKGROUND: The Vaccines for Children (VFC) program is designed to reduce the cost of vaccines for vulnerable children, including Medicaid-eligible children, American Indian/Alaska Native children, uninsured children, and underinsured children whose health insurance does not cover the cost of vaccinations. A desired consequence of the program is to promote comprehensive continuous medical care within a medical home for these children. OBJECTIVES: To explore how having a medical home is associated with vaccination coverage among children eligible for the program. PARTICIPANTS: A total of 2451

A mammogram coupon program: a collaborative effort to provide mammograms to uninsured women in Connecticut.

A collaborative effort to provide free screening mammograms to uninsured women throughout Connecticut using monies raised by the Connecticut RACE FOR THE CURE was launched during National Breast Cancer Awareness Month, October 1994. The program involved 55 American College of Radiology (ACR) accredited mammography facilities and 48 social service agencies. Eligibility criteria stipulated that participants be age 40 or older, be asymptomatic, have no health insurance, and have not had a mammogram within the last year. A total of 531 women received screening mammograms with four breast cancers d

Patterns of medication adherence and health care utilization among patients with chronic disease who were enrolled in a pharmacy assistance program.

BACKGROUND: Poor medication adherence due to high drug costs is a barrier to optimal health outcomes among low-income uninsured patients with chronic conditions. Charitable pharmacy assistance programs provide medications to such patients, but little is known about the utilization patterns of program participants. METHODS: We used a retrospective cohort design to investigate 6-month outcomes for participants in the University of North Carolina (UNC) Health Care Pharmacy Assistance Program (PAP) who received medications indicated for hypertension, diabetes, and/or hyperlipidemia from 2009 thro

Breast cancer screening in underserved women in the Bronx.

This article reports the results of mammography screening among socioeconomically disadvantaged women in Bronx, NY using a federally funded low-cost or no-cost cancer screening service. The New York State Department of Health provided funds for the uninsured through the Bronx Breast Health Partnership. All women < or = 40 years underwent screening mammography using both a mobile van unit and hospital-based mammographic x-ray unit, both American College of Radiology (ACR) accredited. Return visits were coordinated by a follow-up clinic at Montefiore Medical Center using a patient navigator who

Improving Engagement With Breast Cancer Screening Through Structured Telephone Outreach Among Uninsured Women.

Background Financial barriers, limited healthcare engagement, and gaps in preventive care contribute to delayed breast cancer screening among uninsured women. This study evaluated mammography completion within 60 days and secondary measures of breast cancer screening engagement following structured telephone outreach combined with facilitated scheduling through a free state-sponsored screening program among uninsured women overdue for mammography. Hypothesis We hypothesized that structured telephone outreach offering facilitated access to free mammography would support patient engagement acros

Infants' Non-Enrollment in Medicaid after Medicaid-Financed Birth: an Analysis of Pregnancy Risk Assessment Monitoring System (PRAMS) Data.

OBJECTIVES: To characterize the proportion of Medicaid-eligible infants experiencing gaps in Medicaid coverage during early infancy and to determine whether infants without Medicaid coverage were covered by other plans or not at all. STUDY DESIGN: Infants with Medicaid-financed births from three states participating in the 2016-2020 Pregnancy Risk Assessment Monitoring System (PRAMS) were included (N=3,658). Infant insurance coverage (Medicaid, non-Medicaid, none) was assessed at the time of the PRAMS survey, typically around four months of age. RESULTS: Fifteen percent of infants had non-Me

Timeliness of Initial Therapy in Multiple Myeloma: Trends and Factors Affecting Patient Care.

Multiple myeloma (MM) treatment has advanced significantly over the last 2 decades. In most patients, the disease course has been altered from early fatality to chronic morbidity with multiple lines of treatment. The MM treatment paradigm has shifted toward treating patients before end-organ damage occurs. Thus, timeliness of treatment initiation in this era might improve patient outcomes. This is the first report to our knowledge analyzing disparities and trends in treatment timeliness of patients with MM using the National Cancer Database. Multiple factors affected the timing of treatment in

Racial Disparities in Breast Cancer Presentation and Diagnosis in COVID-Era Central Texas.

INTRODUCTION: The COVID-19 pandemic has significantly impacted the diagnosis of breast cancer (BC). With a large Hispanic/Latinx population, early revocation of mask mandates, and lower vaccination rate than many other states, this study explores the relationship between COVID-19 and the presentation and diagnosis of BC patients in the unique socio-politico-economic context of Central Texas. METHODS: This study is a retrospective review of the Seton Medical Center Austin tumor registry for BC patients from March 1, 2019 to March 2, 2021. We compared demographics, insurance status, clinical an

Political determinants of US states' screening-amenable cancer stage at diagnosis and premature cancer mortality.

BACKGROUND: Political determinants of cancer risk are largely unexplored, conceptually and empirically. METHODS: Observational analysis of associations present in 2017-2021 between 5 state-level political metrics and 4 age-standardized cancer outcomes (regional and distant stage at diagnosis for breast, cervical, and colorectal cancer among screening-age adults and premature cancer mortality), overall and in standardized linear regression models adjusting for state-level poverty and medical uninsurance. RESULTS: In fully adjusted models (adjusted for state-level poverty and state-level medic

Emergency department usage by uninsured patients in Galveston County, Texas.

The number of uninsured Texas residents who rely on the medical emergency department as their primary health care provider continues to increase. Unfortunately, little information about the characteristics of this group of emergency department users is available. Using an administrative billing database, we conducted a descriptive study to examine the demographic and clinical features of 17,110 consecutive patients without medical insurance who presented to the emergency department of the University of Texas Medical Branch in Galveston over a 12-month period. We also analyzed the risk of multi

Surgical Care Through a Community Free Clinic-Ambulatory Surgical Center Partnership.

BACKGROUND: Disparities secondary to underinsurance present throughout the surgical care continuum. Community free clinics are uniquely capable to provide health care services to the medically underserved, but surgery often falls outside their scope of care. METHODS: Retrospective chart review was conducted on consecutive community free clinic patients receiving free surgical services via referral to a partnering ambulatory surgery center between March 2016 and September 2021. Those with documented contact information were recruited 1-3 years post-procedure for long-term quality-of-life (LTQO

Tuberculosis Elimination Efforts in the United States in the Era of Insurance Expansion and the Affordable Care Act.

The Patient Protection and Affordable Care Act can enhance ongoing efforts to control tuberculosis (TB) in the United States by bringing millions of currently uninsured Americans into the health-care system. However, much of the legislative and financial framework that provides essential public health services necessary for effective TB control is outside the scope of the law. We identified three key issues that will still need to be addressed after full implementation of the Affordable Care Act: (1) essential TB-related public health functions will still be needed and will remain the responsi

Pushing for Equity, Pulling at Heartstrings: Perinatal Care Access for Uninsured Migrants.

Despite widely accepted clinical care guidelines, uninsured pregnant migrants in Canada face health inequalities and there are ethical implications of denying them publicly-funded care. Uninsured migrants face multiple barriers (financial, systemic, and cultural) to their perinatal care access, which can negatively impact their health and psychosocial outcomes, as well as those of their families. Drawing on interviews with 10 uninsured migrant women in Montreal, Canada, we explore the factors that enabled their access to perinatal care. Our findings underline the importance of health professio
