Are there recognized prognostic factors for patients undergoing pulmonary metastasectomy for colorectal carcinoma?

A best evidence topic in thoracic surgery was written according to a structured protocol. The question addressed was whether there is a specific subgroup of patients that would benefit from pulmonary metastasectomy for colorectal carcinoma (CRC). A total of 524 papers were identified using the reported search, of which 1 meta-analysis, 1 systematic review and 17 retrospective studies represented the best evidence to answer the clinical question. The authors, date, journal, country, study type, population, outcomes and key results are tabulated. Single pulmonary metastasis (PM) was identified a

Impact of COPD on Outcomes After Pulmonary Metastasectomy: A Propensity-Score Matched Analysis.

BACKGROUND: Pulmonary metastasectomy (PM) is an established component of multimodal oncological treatment, but its perioperative safety and long-term outcomes in patients with chronic obstructive pulmonary disease (COPD) remain insufficiently defined. We aimed to evaluate the impact of COPD on perioperative morbidity, mortality and long-term survival after PM. METHODS: We retrospectively analysed 692 patients who underwent PM with curative intent. COPD severity was graded using the GOLD spirometric classification. Patients were classified according to their postbronchodilator FEV1/FVC ratio i

Prognostic Factors for Pulmonary Metastasectomy for Colorectal Cancer: A Propensity Score Matching Analysis.

BACKGROUND/AIM: This study aimed to investigate prognostic factors in pulmonary metastasectomy for colorectal cancer (CRC), considering the location of the primary tumor. PATIENTS AND METHODS: We collected information from patients who underwent pulmonary resection for CRC at two high-volume centers from January 2010 to December 2023. Using a multivariate Cox proportional hazards model, patient background factors that affected the prognosis after pulmonary metastasectomy were identified. RESULTS: A total of 289 patients were included in the study. The multivariate analysis revealed the prima

Stereotactic ablative radiotherapy for pulmonary metastasis from sarcoma: a retrospective comparison with metastasectomy.

Recent studies report excellent local control (LC) and favorable toxicities of stereotactic ablative radiotherapy (SABR) for pulmonary metastasis (PM) from sarcoma. This study compared the LC and survival of SABR and metastasectomy for sarcoma PM. We analyzed the LC rates of 54 PMs treated with SABR between 2008 and 2022. For survival analysis, we compared 14 patients who received SABR as first-line treatment with 61 patients who underwent metastatectomy. For SABR-treated PMs, a median total dose of 55 Gy (range, 48-60) was administered over 3-10 fractions. Median follow-up for LC in SABR-trea

Does solitary- and organ-confined metastasectomy really improve survival in advanced urologic malignancies?

The role of metastasectomy on survival in advanced/urologic malignancies still remains unclear. Renal cell carcinoma (RCC) is the most common solid tumor within the kidney. 25-30 % of patients have metastases at manifestation. Urothelial carcinoma (UC) consists of bladder carcinoma, upper urinary tract urothelial cell carcinoma and urethral carcinoma. Bladder cancer is the ninth most common cancer worldwide. Half of patients with muscle invasive bladder cancer have lymph node or distant metastases. In metastatic disease first-line treatment is multi-agent platinum-based systemic chemotherapy.

Pulmonary metastasectomy in colorectal cancer patients with previously resected liver metastasis: pooled analysis.

BACKGROUND: Data addressing the outcomes and patterns of recurrence after pulmonary metastasectomy (PM) in patients with colorectal cancer (CRC) and previously resected liver metastasis are limited. METHODS: We searched the PubMed database for studies assessing PM in CRC and gathered individual data for patients who had PM and a previous curative liver resection. The influence of potential factors on overall survival (OS) was analyzed through univariate and multivariate analysis. RESULTS: Between 1983 and 2009, 146 patients from five studies underwent PM and had previous liver resection. The

Long-term survival and occasional regression of distant melanoma metastases after adrenal metastasectomy.

BACKGROUND: Human melanoma cells express high-affinity glucocorticoid receptors, and adrenalectomy has been shown to have antimelanoma effects in animal models. Long-term regression of distant metastatic melanoma was observed in one patient after bilateral adrenalectomy, prompting a review of adrenalectomy for melanoma metastases performed at this center. METHODS: A retrospective study in which all patients treated at the Sydney Melanoma Unit and recorded as having adrenal gland metastases between January 1987 and January 2004 were identified and their survival analyzed. RESULTS: One hundred

Pulmonary metastasectomy for malignant melanoma prolongs survival significantly compared to non-surgical therapy.

OBJECTIVES: Pulmonary metastasectomy (PM) is an established treatment for selected patients with pulmonary metastases. While systemic therapies such as immune checkpoint inhibitors, and stereotactic radiation have become standard for unresectable or metastasized melanoma patients, no randomized controlled trials directly assess the benefit of PM by itself in oligometastatic melanoma patients with lung metastases. This retrospective study compares long-term survival outcomes in patients undergoing PM versus those receiving non-surgical therapy for pulmonary metastatic melanoma. METHODS: We ret

Survival Outcomes of Complete Pulmonary Metastasectomy for Head and Neck Squamous Cell Carcinomas.

PURPOSE: Metastatic head and neck squamous cell carcinoma (HNSCC) is relatively poor; however, depending on the selected cases, pulmonary metastasectomy can be a practical therapeutic option. This study aimed to identify the outcomes of complete metastasectomy based on each primary site and to investigate unfavorable prognostic factors. PATIENTS AND METHODS: We used the database from the Metastatic Lung Tumour Study Group of Japan. Between November 1980 and April 2017, 231 patients were deemed eligible. According to anatomy and the current epidemiology of HNSCC, the patients were divided into

Pulmonary metastasectomy for thyroid cancer as salvage therapy for radioactive iodine-refractory metastases.

OBJECTIVES: Distant metastasis arising from thyroid cancer is rare but has been associated with significantly reduced long-term survival, especially when refractory to radioactive iodine ablation. We provide one of the largest studies worldwide reporting the outcome after salvage pulmonary metastasectomy for this entity, aiming to identify prognostic factors and to analyse surgical indication. METHODS: We retrospectively analysed the medical records of 43 patients who had undergone pulmonary metastasectomy for radioactive iodine-refractory thyroid cancer from 1985 to 2016. RESULTS: The media

Metastatic angiosarcomas: doxorubicin-based regimens, weekly paclitaxel and metastasectomy significantly improve the outcome.

BACKGROUND: Angiosarcomas are a rare but aggressive form of soft tissue sarcoma. At metastatic stage, the clinical benefit of therapeutic intervention remains debatable. PATIENTS AND METHODS: We have carried a retrospective analysis of 149 cases treated between 1996 and 2009 in the French Sarcoma Group. RESULTS: The median age was 60; the sex ratio was 0.80. Sixty-two percentage of cases presented with metastasis at the diagnosis. About 20% arose in irradiated fields. The median overall survival was 11 months. Treatment consisted in metastasectomy (5.4%), doxorubicin-based regimen (46.9%), w

A case of metastasectomy following multiple courses of radionuclide therapy for distant metastatic pheochromocytoma.

Pheochromocytomas and paragangliomas (PPGLs) are neuroendocrine tumors derived from chromaffin cells that produce catecholamines. The primary treatment for PPGLs is surgical resection, and even in cases with distant metastases, metastasectomy may be considered. Other therapeutic options for distant metastatic disease include radionuclide therapy, such as 131I-metaiodobenzylguanidine (MIBG) and 177Lu-1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid-(Tyr3)-octreotate (DOTATATE). However, clear evidence is lacking regarding which patients with distant metastases are the best candidates fo

Pulmonary Metastasectomy for Head and Neck Squamous Cell Carcinoma in the Modern Treatment Era.

OBJECTIVES: Pulmonary metastasectomy (PM) has been a viable treatment option for metastatic head and neck squamous cell carcinoma (HNSCC). In recent years, advances in systemic therapy and multidisciplinary cancer management, including immune checkpoint inhibitors (ICIs), have changed the treatment landscape of advanced or recurrent head and neck cancer. METHODS: We retrospectively reviewed 44 patients who underwent complete resection for pulmonary metastases from HNSCC between 2008 and 2021. To minimize the risk of including primary lung cancer, 8 patients were excluded based on predefined c

A Case of Large Cell Neuroendocrine Carcinoma of the Bladder With Long-Term Control by Metastasectomy.

INTRODUCTION: Large cell neuroendocrine carcinoma (LCNEC) of the bladder is rare and aggressive. Reports of metastasectomy for single distant metastases are even rarer. CASE PRESENTATION: A 49-year-old man was admitted to our hospital with gross hematuria. Cystoscopy, magnetic resonance imaging, and computed tomography revealed invasive bladder cancer without metastasis. He underwent transurethral resection of the bladder tumor, and histopathological examination confirmed LCNEC. The patient received neoadjuvant chemotherapy followed by partial cystectomy. Nineteen months postoperatively, a so
