Preterm infants are particularly vulnerable to bacterial infections that may rapidly progress to life-threatening sepsis, a state of excessive inflammation and organ dysfunction. To investigate disease mechanisms and advance therapeutic development, robust translational animal models that replicate the complex pathophysiology and clinical presentation of neonatal sepsis are critically needed. This protocol presents a clinically relevant preterm piglet model of neonatal sepsis, induced via intra-arterial infusion of live Staphylococcus epidermidis shortly after birth or on postnatal day three. …

Neonatal sepsis remains a major cause of preventable newborn deaths worldwide despite advances in maternal screening, antibiotic use, and neonatal care. Persistent diagnostic delays, rising antimicrobial resistance (AMR), and unequal access to resources highlight the urgent need for new approaches. This review summarizes the primary pathogens responsible for early- and late-onset neonatal sepsis, along with their transmission routes, virulence factors, and clinical presentations. We examine promising innovations in rapid molecular diagnostics, next-generation sequencing, artificial intelligenc…

Infections are among the leading causes of neonatal mortality, and about 75% of the burden occurs in developing countries. Diagnosis of neonatal sepsis in these countries is dependent on the recognition of a set of nonspecific clinical signs that maximize sensitivity because staff making initial assessments may not have specialist pediatric training. Accurate diagnosis is usually limited by the unavailability of reliable microbiological investigation. The World Health Organization recommends ampicillin (or penicillin; cloxacillin if staphylococcal infection is suspected) plus gentamicin for em…

Protein toxins are important virulence factors contributing to neonatal sepsis. The major pathogens of neonatal sepsis, group B Streptococci, Escherichia coli, Listeria monocytogenes, and Staphylococcus aureus, secrete toxins of different molecular nature, which are key for defining the disease. Amongst these toxins are pore-forming exotoxins that are expressed as soluble monomers prior to engagement of the target cell membrane with subsequent formation of an aqueous membrane pore. Membrane pore formation is not only a means for immediate lysis of the targeted cell but also a general mechanism…

BACKGROUND: There is limited literature on neonatal bacterial sepsis in New World (NW) camelids.
HYPOTHESIS: Bacterial culture-positive crias have clinical differences based on the specific bacterial genera isolated.
ANIMALS: Bacterial culture-positive NW camelid crias <21 days of age from 1990 to 2005 were included.
METHODS: Historic physical examination and cliniopathologic data were retrieved from medical records as were the identity and antibiograms of bacterial isolates. Cases were categorized by outcome (survival versus nonsurvival) and type of sepsis (gram-negative or gram-positive).

Salmonella enterica serovar Weltevreden is an uncommon cause of gastroenteritis occurring worldwide. For the first time, we report 2 cases of neonatal sepsis caused by S. Weltevreden from Hubli, India. In the first case, the neonate had features of septicemia and S. Weltevreden was isolated from a blood culture. The other neonate had omplalitis and clinical features of septicemia. S. enterica serovar Weltevreden was isolated from the umbilical swab culture of this neonate. Even though extensive investigations were conducted, the source of infection could not be identified. Both neonates recove…

The plasmid profile of 25 hospital strains of Gram-negative bacteria that are known to be originators of neonatal sepsis was studied by the authors. Of these, E. coli, Enterobacter spp., and Citrobacter spp. proved to be the most frequently seen causative agents in the above health problem, with 13, 5, and 7 strains respectively having been recovered in the studied material. There have been isolated 26 plasmids from the studied 25 bacterial strains. Of these, fifty percent were classified as Citrobacter spp. The molecular mass of the plasmids recovered has been found to vary among 30 and 120.

96 consecutive inborn neonates with blood culture proven bacterial sepsis during the period January to June 1997 were studied. Lethargy with refusal of feeds (28%), fever (28%) and respiratory distress (31.3%) were the major presenting features. Half of them (n = 48) were of early onset (< 48 hours) and the remaining half were of late onset (> 48 hours). Staphylococcus aureus (n = 59, 61.5%) was the predominant pathogen and 66% of them were methicillin resistant followed by Klebsiella pneumoniae (n = 24, 21.9), Escherichia coli (n = 13, 13.5%) and streptococci (n = 3, 3.1%). Antibiotic resista

This study was conducted to determine the value of superficial cultures in the diagnosis of neonatal sepsis in our hospital. Sixty three babies, younger than 2 weeks who were admitted with suspected sepsis were investigated. A total of 369 cultures were obtained from these babies--252 (68.29%) superficial and 171 (31.70%) deep cultures. External ear canal swab, umbilical cord swab and throat swab culture accounted for the superficial cultures. Blood culture, cerebrospinal fluid culture and i.v. catheter culture accounted for deep cultures. Of the 369 cultures, 225 (60.97%) were positive for pa…

An unusual case of infarction of the tibia in a neonate who presented with sepsis following poor cord management at birth is reported. He was managed as for necrotizing enterocolitis and had limited debridement of infarcted limb because the parents refused amputation of the limb. This patient was a diagnostic problem. Necrotizing fasciitis and in retrospect purpura fulminans are considered as possible diagnoses and are discussed briefly.

The pattern of neonatal bacterial infection, its management and the types of infection control policy were ascertained in 20 neonatal intensive care units (NICU) in Australia by questionnaire survey. Group B Streptococcus and Escherichia coli were the predominant organisms responsible for perinatally acquired infection for which the most common antibiotic combination used was Penicillin and Gentamicin. Staphylococcus epidermidis and aureus (majority Methicillin resistant) were the predominant organisms responsible for nosocomial infection for which the most common antibiotic combination used w

Despite the development of newer generation of antibiotics, mortality from neonatal sepsis remains high. In a prospective, randomized study, we investigated the use of IgM-enriched immunoglobulin therapy in neonatal sepsis. Two groups of 30 infants each (matched for gestational age, sex, weight, and other variables) were randomly allocated to receive either antibiotics alone (control group) or antibiotics plus 5 mL/kg/d for four days of IgM-enriched immunoglobulin intravenously (immunotherapy group). Mortality from sepsis in the control group was 20% (6/30), while in the immunotherapy group it…

Neonatal sepsis remains one of the most pressing public health challenges in Africa, threatening progress towards the UN Sustainable Development Goal target 3.2 of reducing neonatal mortality to 12 deaths or less per 1000 livebirths. Beyond the well recognised barriers to diagnosis, treatment, and prevention, additional and often overlooked determinants, including sociocultural norms, fragmented health systems, inequities in care, and low political will, continue to stall progress and sustain unacceptably high mortality rates. Antimicrobial-resistant infections further compound this crisis: ne…

Preterm and extremely low-birth-weight newborns are at a higher risk for early-onset neonatal sepsis (NN-SP). This condition is often linked to intrapartum fever, PROM, and sepsis occurring within the first day of life. The causal pathogen can be acquired through intrauterine transmission or from the mother's flora. Therefore, our study aimed to evaluate PC and PI in cases involving NN-SP. Therefore, we can conclude that analyzing the PC and its indices can improve the prognosis of sepsis in NN. Studying NN using this method is a cost-effective approach that helps prevent mortality and morbidi…
