Penile prosthesis implantation (PPI) is a definitive treatment option for patients with medically refractory erectile dysfunction (ED). OBJECTIVES: To assess the feasibility and results of 'valve-in-valve' implantation using the 23-mm CoreValve for the treatment of degenerated 19-mm and 21-mm Mitroflow bioprostheses.
METHODS: We retrospectively analysed all consecutive patients who underwent transcatheter aortic valve implantation for 19-mm and 21-mm Mitroflow bioprostheses. The height of implantation with respect to the ring of the Mitroflow prosthesis was targeted at -6 mm for the first 3 cases. In the following cases, the target was higher to promote free supra-annular movement of the leaflets.

Mitral annular calcification (MAC) is a chronic degenerative process at the fibrous base of the mitral valve. It is a feared diagnosis in the context of mitral valve operations because of the risk of severe adverse events such as atrioventricular disruption, injury to the circumflex artery during debridement, and difficult placement of annular sutures. We report a series of 3 consecutive female patients with severe circular MAC who underwent successful mitral valve replacement through a lateral minithoracotomy with use of an inverted transcatheter aortic valve.

Although penile implantation remains a final solution for patients with refractory impotence, undesirable postoperative effects, including penile size reduction and cold sensation of the glans penis, remain problematic. The use of tanscathether aortic valve implantation (TAVI) to treat severe aortic valve disease is increasing exponentially. Peripheral vascular access for TAVI is not always possible, and when transapical approach is contraindicated, other access options can be considered like the central transaortic access. The authors describe a successful implantation of a CoreValve prosthesis through direct aortic access via ministernotomy, addressing particular attention to the implantation procedure, potencial complications, advantages and limitations of the method.

During the period 1963--76, sixty-nine patients underwent aorto-iliac of aorto-femoral by-pass operation at the department. Fifty-two of the operations were performed through a retroperitoneal approach. The operative procedure is described. The approach is found gratifying and the statement of earlier authors concerning fewer cases of paralytic ileus and smoother convalescence is confirmed.
