An Independent Department of IR: Rationale, Conception, and Implementation.

On April 12, 2016, the University of Miami established its Department of Interventional Radiology (IR), completing a 3-year process initiated by the Chief of IR and supported by the Chair of Radiology. This transition reflects IR's maturity and its distinction from diagnostic radiology. Historically, IR evolved within radiology, similar to other specialties like radiation oncology. Advancements in minimally invasive procedures and increasing direct patient care have driven IR's growth, formalized in 2012 by the American Board of Medical Specialties and the approval of the Dual Primary Certific

Multimodality diagnostic imaging and role of interventional radiology in pancreas transplantation.

Pancreas transplantation remains a key therapeutic option for patients with type 1 diabetes mellitus. Despite advances in medical management, post-transplant complications continue to pose significant risks to graft survival, making early detection essential for optimizing patient outcomes. Given the complexity of pancreas transplantation, multimodality imaging plays a central role in evaluating graft function and identifying complications. Each modality provides pertinent information based on the complication suspected. This review outlines imaging strategies essential for assessing graft via

[Is there a curative treatment for oligometastases? Lung radiofrequency ablation].

Percutaneous ablation allows to treat tumours with temperature modifications. These non-invasive techniques are useful to treat metastatic lung tumours in patients with a slowly evolving disease, which requires multiple local treatment. Ablation is therefore proposed as a locoregional treatment because it has a minimal impact on the lung. Other advantages are its efficacy and a low morbidity. Limits may be the tumour size and its location.

Intraosseous Varix of the Left Tibia Causing Severe Lower Extremity Pain: A Report of a Rare Case.

Varicose veins are venous anomalies of the peripheral venous system that commonly present with lower extremity pain and swelling. Intraosseous varices are a very rare subtype of varicose veins with an atypical presentation and thus pose a significant diagnostic challenge, leading to prolonged patient morbidity. Only a handful of cases have been reported of this rare entity. We report a case of a 73-year-old male with a history of hyperlipidemia who presented with a 4-week history of worsening left lower extremity pain. Initial workup, including labs, radiography, and ultrasound Doppler, ruled

Multimodality imaging findings in image-guided biopsy proven splenic littoral cell angioma: series of three cases.

Littoral cell angioma (LCA) is a rare primary splenic vascular tumor arising from the littoral cells that line the splenic sinuses of the red pulp. Definitive diagnosis of this entity depends on histology and immunochemical studies, for which splenectomy is usually performed. We present three image-guided percutaneous core biopsy proven cases of splenic LCA and also discuss imaging features of these rare tumors on CT, MR, and US findings.

[Usefulness of POCUS in Orthopedic and Trauma Fields].

Recently, musculoskeletal ultrasonography has been becoming more common for diagnosis and treatment by ultrasound-guided intervention in the field of orthopedics. Because musculoskeletal ultrasound technology has rapidly advanced in recent years, and has many advantages, including no exposure to radiation, non-invasive, wide availability, cost-effectiveness, and the ability to be used in real-time in the general outpatient clinic. Traditional radiography was not able to detect soft tissue injury, but musculoskeletal ultrasonography enables the diagnosis of not only musculoskeletal disorders in

X-ray Doses in Relation to Body Mass, Indication, and Substrate During Pediatric Electrophysiological Procedures on the Heart.

The main goal of this study is to determine typical values of dose area product (DAP) and difference in the effective dose (ED) for pediatric electrophysiological procedures on the heart in relation to patient body mass. This paper also shows DAP and ED in relation to the indication, the arrhythmia substrate determined during the procedure, and in relation to the reason for using radiation. Organ doses are described as well. The subjects were children who have had an electrophysiological study done with a 3D mapping system and X-rays in two healthcare institutions. Children with congenital hea

Implementing a Trauma Hybrid Operating Room from an Interventional Radiology Perspective.

Hybrid operating rooms have been increasingly adopted in trauma centers to enable integrated surgical and endovascular care in hemorrhagic emergencies, but practical guidance on their operational implementation from the interventional radiology perspective remains limited. We describe our experience implementing a trauma hybrid operating room at an American College of Surgeons-verified Level I trauma center, addressing room design, zoning for rapid transitions between laparotomy and angioembolization, a mobile supply cart, and a pre-embolization checklist. We outline a training approach center

Fundamental Surgical Principles for Interventional Radiology and Vascular Practice.

Interventional radiology (IR) and vascular procedures are minimally invasive in approach, yet complications such as bleeding, infection, pseudoaneurysm formation, and delayed wound healing remain closely linked to fundamental surgical principles. Despite increasing procedural complexity, structured training in tissue handling, suturing, and infection prevention remains variable across IR practice. This narrative review synthesises established surgical literature, IR guidance, and procedural evidence to present a practical, principle-based framework applicable to routine IR and vascular interve

Use of pre-procedure electronic questionnaire to enhance scheduling and safety in interventional radiology procedures with anesthesia care.

The growth of interventional radiology (IR) procedures with anesthesia team care in increasingly medically complex populations points to the need for effective and efficient pre-procedure screening. We present an ongoing quality improvement project involving a brief online questionnaire disseminated to patients three to ten days before the day of their scheduled IR procedures. The questionnaire was developed by anesthesiologists and a nurse practitioner to increase pre-procedure awareness of relevant medical concerns, guide scheduling of procedures at outpatient versus inpatient locations, and

Echocardiography-fluoroscopy fusion imaging: The essential features used in congenital and structural heart disease interventional guidance.

Increased catheter-based interventions in congenital and structural heart disease require imaging modalities to be oriented in the same visual perspective. The use of echocardiography-fluoroscopy fusion (EFF) imaging has been developed for better characterization of complex anatomy and to facilitate key steps in interventional procedures. This review will detail the technology behind EFF, the differences between the two ultrasound fusion systems, and essential features of EFF imaging in congenital and structural heart disease interventions.

A systematic review of diagnostic accuracy of vertebral fracture assessment (VFA) in postmenopausal women and elderly men.

UNLABELLED: This systematic review was performed to compare the diagnostic accuracy of vertebral fracture assessment (VFA) with that of spinal radiography for identification of vertebral fractures (VFs). VFA appeared to have moderate sensitivity and high specificity for detecting VFs when compared with spinal radiography. INTRODUCTION: VFs are recognized as the hallmark of osteoporosis, and a previous VF increases the risk of a future fracture. Therefore, the timely detection of VFs is important for prevention of further fractures. This systematic review examined the diagnostic accuracy of VF

Adjuvant MRI-guided percutaneous cryoablation treatment for aneurysmal bone cyst.

Aneurysmal bone cysts are benign, expansile, lytic bone lesions that behave in a locally aggressive manner. Although radiography and computed tomography (CT) can detect the lesion, magnetic resonance imaging (MRI) is ideal for the demonstration of characteristic fluid-fluid levels, extent, and margins. Treatment typically consists of open surgical curettage with the addition of local adjuvants and bone grafting. Residual or recurring lesions may be treated using percutaneous cryoablation. Although CT guidance is often employed for image guidance, visualization and targeting of smaller clusters

Unoperated tetralogy of Fallot in a 68-year-old patient.

Tetralogy of Fallot (TOF) is the most common cyanotic congenital heart disease, generally treated with total correction within the first two years of life. Occasionally, some unoperated cases can reach older ages. A 68-year-old woman with diabetes mellitus presented with swelling in legs and abdomen, weakness, exertional dyspnea, and orthopnea. On physical examination, she had mild cyanosis with clubbing. Her blood pressure was 110/60 mmHg and pulse rate was 79 beat/min. She had a systolic ejection murmur and bilateral rales on basal lung areas. Massive edema was noted in both lower limbs. Ele

The widened mediastinum in trauma patients.

Mediastinal widening is a frequent radiological finding in the emergency department patient. The causes of mediastinal widening can be divided into traumatic and nontraumatic mediastinal widening. An important association of moderate to high velocity trauma is the mediastinal haematoma. It may be the result of traumatic transsection of the aorta, or it may be due to bleeding from other mediastinal vessels. Before the era of multidetector spiral CT, angiography was the gold standard for the evaluation of patients with a widened mediastinum. Meanwhile, angiography as a risk-carrying invasive exa
