Malignant Hyperthermia-Associated Rhabdomyolysis After Succinylcholine: A Case Report.

Malignant hyperthermia-associated rhabdomyolysis is a condition that lies on the spectrum of malignant hyperthermia (MH). Unlike MH, malignant-hyperthermia-associated rhabdomyolysis is a condition which most commonly is recognized in the postoperative period by the development of discolored brown urine with notably absent rigidity, tachypnea, tachycardia, and hyperthermia. The presented case describes the subtle onset of malignant hyperthermia-associated rhabdomyolysis, along with a review of relevant literature.

Volatile anesthetics and succinylcholine in cardiac ryanodine receptor defects.

Familial polymorphic (catecholaminergic) ventricular tachycardia is an arrhythmogenic cardiac disorder caused by mutations of the myocardial isoform of the ryanodine receptor gene (RyR2). Mutations of the corresponding gene in the skeletal muscle (RyR1) predispose its carriers to malignant hyperthermia upon use of volatile anesthetics or succinylcholine, which further deteriorate the inherited intracellular calcium release disorder. We report a series of patients with cardiac RyR defects who underwent general anesthesia without complications. Succinylcholine and volatile anesthetics did not ha…

Jaw relaxation after a halothane/succinylcholine sequence in children.

BACKGROUND: Lack of complete jaw relaxation after a halothane-succinylcholine sequence has been described in the literature. To date, however, most existing studies are retrospective, and lack agreement on the magnitude and incidence of this phenomenon. This prospective study examined the incidence and degree of incomplete jaw relaxation in 500 children who were given intravenous succinylcholine during halothane anesthesia. METHODS: Five hundred consecutive unmedicated children received a minimum dose of 2 mg/kg intravenous succinylcholine after induction of anesthesia with halothane. The deg…

Isoflurane inhibits muscle fasciculations caused by succinylcholine in children.

The incidence and intensity of muscle fasciculations as well as the occurrence of cardiac arrhythmias following succinylcholine were evaluated in 36 premedicated children (1.0-5.7 years) after intravenous induction with thiopentone or after inhalation induction with isoflurane (3.75 vol-% in 70% nitrous oxide in oxygen). The study was randomized. In the thiopentone group, fasciculations were seen in all children and in the isoflurane group in 5 of 18 children (P < 0.001). The median of the duration of fasciculations was 15 s with a minimum of 5 s and maximum of 36 s (1st quartile 9 s and 3rd q…

Succinylcholine-vecuronium (Org NC 45) sequence for cesarean section.

Vecuronium (Org NC 45) was used in 27 parturients undergoing elective cesarean section under general anesthesia. One of the parturients had a twin pregnancy. Neuromuscular transmission was assessed clinically by stimulating the ulnar nerve and observing the contraction of the fingers. An initial bolus of 0.05 mg . kg-1 of vecuronium was injected after recovery from succinylcholine block. Complete neuromuscular block resulted in all cases and lasted for a mean duration of 19.3 +/- 6.02 min. Muscular relaxation was maintained by incremental doses of 0.01 mg . kg-1 of vecuronium. Administration o…

Anaphylaxis due to succinylcholine. Immunoallergological study of thirteen cases.

The antigen non-specific release of histamine by synthetic neuromuscular blocking agents is well known, though our review of thirty-two cases reported in the literature of anaphylactoid reactions to succinylcholine, shows that there is insufficient evidence to determine whether the signs are due to anaphylactic or to anaphylactoid reactions. We have examined thirteen cases in whom evidence of anaphylactic sensitization was examined by direct skin tests (in eleven cases) Prausnitz--Küstner tests (in five), rabbit basophil degranulation, Shelley test (in twelve) and human basophil degranulation …

Influence of succinylcholine on middle component auditory evoked potentials.

Auditory evoked potentials in the middle component time domain (post-stimulus, 8 to 50 msec) were recorded in response to 1,000-Hz tone pips in a normal-hearing adult subject. Electromyographic (EMG) responses in response to ulnar nerve shocks were recorded from the ipsilateral hypothenar muscles. With the assistance of an anesthesiologist, data were collected during a normal resting state, a state of light sedation, and a state of complete skeletal muscle paralysis from succinylcholine administration. During the paralyzed state, there was abolition of the normal EMG responses seen in the rest…

The Effect of Pregabalin on the Prevention of Succinylcholine-Induced Fasciculation and Myalgia.

PURPOSE: This study evaluates the effect of pregabalin on fasciculation and myalgia after using succinylcholine. DESIGN: This randomized double-blind prospective study was conducted among 100 patients aged 20 to 60 years old. METHODS: Pregabalin (300 mg) and placebo (in capsule form) were placed in similar containers. The results were analyzed by SPSS 23 software, and statistical analysis consisted of χ2 test and t test, and a P value less than .05 was considered significant. FINDINGS: The mean pain score in the group receiving pregabalin was lower than the placebo group. According to the χ…

Estimate of the relative risk of succinylcholine for triggering malignant hyperthermia.

BACKGROUND: Facilities with volatile anesthetic agents stock dantrolene for the treatment of malignant hyperthermia (MH). The availability of dantrolene at these facilities satisfies cost-utility norms even for sites with as few as 1 anesthetic per workday, based on the overall incidence of MH per anesthetic. We considered the stocking of dantrolene at facilities with succinylcholine alone (i.e., where volatile anesthetics are not available), by using registry data and estimates of the frequency of administration of succinylcholine during anesthesia. We determine the magnitude of the relative …

Pretreatment with remifentanil is associated with less succinylcholine-induced fasciculation.

BACKGROUND: Succinylcholine is a popular muscle relaxant and one of its most common side effects is muscle fasciculation. The purpose of this study was to evaluate the efficacy of remifentanil in preventing succinylcholine-induced fasciculation in patients undergoing general anesthesia. METHODS: In aprospective, double blind study, 60 ASA I & II patients were randomly assigned into two groups (30 each) to receive either remifentanil 1 microg/kg (Group R), or saline 3 ml (Group S) as a pretreatment agent, one minute before induction of general anesthesia by propofol, fentanyl, and 1.5 mg/kg su…
