Verapamil has become a popular antiarrhythmic drug for the acute management of supraventricular tachycardia in infants and children. A full-term, 1-hour-old infant presented with supraventricular tachycardia and hypotension that did not respond to vagal maneuvers and direct current cardioversion. After intravenous verapamil, the heart rate slowed and the underlying rhythm was atrial flutter.

Verapamil, a calcium-inhibitory drug, was found to inhibit T-lymphocyte proliferation in mixed lymphocyte culture and the proliferative response to phytohaemagglutinin and mumps antigen. It also inhibited production of interleukin 2 (IL-2). To exert inhibition, verapamil had to be added early in the culture period. Verapamil also had a relatively small inhibitory effect on IL-2-dependent growth. The effects were clearly seen only at concentrations exceeding the therapeutic serum level of verapamil.

Verapamil, a calcium antagonist, was compared with propranolol, a widely used beta-adrenoreceptor blocker, in hypertensive patients with respect to its antihypertensive efficacy and incidence and severity of adverse effects. EDTA or verapamil was infused into the renal artery of the anesthetized dog, and the effects on renin release and renal function were examined in an attempt to elucidate the role and action site of calcium ion in the renin secretory system. We studied the long-term effects of oral verapamil (240-720 mg) daily on 10 patients with hypertrophic cardiomyopathy. Symptomatic improvement of chest pain or shortness of breath occurred in all the patients although 4 deteriorated after 1 year. Side effects occurred in 3 patients. Verapamil or placebo was administered as a bolus infusion in a double-blind fashion to 24 patients with either atrial fibrillation or flutter and to ten patients with paroxysmal supraventricular tachycardia. The effect of intravenous verapamil on the termination of supraventricular tachycardia (SVT) was studied by continuous electrocardiographic monitoring of 27 episodes of SVT. Digitalis preparations frequently fail to control heart rate in many patients who have chronic atrial fibrillation, particularly during physical exertion. The effects of orally administered verapamil, 160 to 240 mg/day, on the heart rate at rest and during mild exercise were studied in 23 digitalized patients with chronic atrial fibrillation of various causes. Verapamil substantially reduced the excessive heart rate response to exercise in well-digitalized patients who had chronic atrial fibrillation.

Rapid and slow venous infusion of various doses of Verapamil in a mixed series of 185 cases of arrhythmia since 1968 is reported. A 31-year-old multiparous woman presented with shortness of breath, palpitations and new-onset, wide complex tachycardia at approximately 28 weeks' gestation.
