Correction of pediatric cervical kyphosis: our experience and systematic-literature review.

BACKGROUND: Pediatric cervical kyphosis is a distinct entity with diverse etiology (congenital, syndromic, traumatic, metabolic or neoplastic). Surgical correction in pediatric population is challenging due to their growing spine and low blood volume. PURPOSE: To analyse their presentation, surgical techniques and outcome of pediatric cervical kyphosis and systematically review the pertinent literature. DESIGN: Retrospective study. PATIENT SAMPLE: 16 patients aged ≤ 18 years who underwent correction for cervical kyphosis between 2009 and 2021. OUTCOME MEASURES: Nurick's grading, mJOA score

Incomplete cervical limited dorsal myeloschisis with membranous sac: a case report and literature review.

Limited dorsal myeloschisis (LDM) is caused by a primary neurulation defect resulting from incomplete disjunction of the cutaneous and neural ectoderm, and manifests as a fibroneural tethering stalk linking the skin lesion to the underlying spinal cord. We present a case of cervical LDM associated with a multi-lobular cystic lesion suspected during the prenatal ultrasonographic check-up. A diaphanous membranous cystic sac without neuronal components inside was extruded from the dorsal cervical vertebrae at the C6/7 portion. The preoperative diagnosis was confirmed by computed tomography (CT), …

[Fractures of the cervical spine].

A gentle traction is often sufficient to repose unstable lesions of the cervical spine, when a retention by means of conservative methods is not possible. Therefore a ventral, intracorporal fusion with autologous, corticospongious chip material combined with a plate fixation is an adequate solution to obtain a stable, movable blocking of the lower six cervical vertebrae. In case of a completely ruptured dorsal ligamentous apparatus, a ventral bone-chip blocking is generally insufficient. Ventral spondylodesis should be combined with a dorsal traction belt in these cases. Fresh, instable fractu…

Is Shigella an under-recognized pathogen? A case of pyogenic cervical spondylitis caused by Escherichia coli and Shigella flexneri infection.

Shigella typically causes gastrointestinal infections, and extra-intestinal manifestations are rare. We report the first known case of pyogenic cervical spondylitis co-infected with Escherichia coli and Shigella flexneri, highlighting the diagnostic challenges and clinical implications. A 53-year-old woman presented with neck pain for one month. MRI revealed C6 and C7 vertebrae abscesses. The patient underwent anterior cervical debridement and bone-graft fusion. Intraoperative pus culture grew Escherichia coli, while metagenomic next-generation sequencing detected both Escherichia coli and Shi

[A Patient with Klippel-Feil Syndrome Having Difficulties in Inserting and Placing an Endotracheal Tube under General Anesthesia].

A patient with Klippel-Feil syndrome had difficulties in inserting and placing an endotracheal tube under general anesthesia. Klippel-Feil s … y (3D-CT) images after the operation, showed tracheal stenosis at the level of the fourth and fifth cervical vertebrae. In cases where airway difficulties are expected, evaluating 3D-CT images might be useful in airway management.

Multiple Major and Minor Anomalies Associated With Klippel-Feil Syndrome: A Case Report.

Klippel-Feil syndrome is defined as congenital fusion of two or more cervical vertebrae. In this article, we report a 55-year-old male patient with one-year history of neck pain, headaches, and one episode of syncope after a severe trauma. X-rays and magnetic resonance imaging of cervical spine revealed fused vertebral bodies of C2-C5. The major anomalies associated with Klippel-Feil syndrome (small stature, thoracic kyphoscoliosis, lumbar scoliosis, restricted opening mouth, and bilateral sensorineural hearing loss) as well as multiple minor anomalies (mild face asymmetry, high arched palate,…

Calcium carbonate apatite deposition in the cervical spine with associated vertebral destruction. Case report.

This 52-year-old woman developed crystal deposition disease involving the cervical vertebrae. She presented with symptomatic spinal cord compression secondary to extensive calcified lesions in the posterior elements of the cervical spine. Surgical decompression with posterior fusion was performed. Histological examination showed hardened deposits of calcium carbonate involving the soft tissue, and dissolution of the vertebral bone trabeculae. There was no inflammatory response to these deposits. One year postoperatively the patient developed severe pulmonary disease associated with the collage…

An Unusual Pattern of Skull Base Pneumatization.

OBJECTIVE: Our aim is to describe an unusual pattern of craniocervical pneumatization resulting in a spontaneous fracture in a patient, who presented with tinnitus. METHODS: Temporal CT was ordered in a patient with tinnitus lasting over a year. His audiogram was normal. No history of previous trauma, recent flight, diving, or weight lifting was present. Further questioning revealed his habitual Valsalva maneuvers to relieve symptoms of nasal congestion. RESULTS: Temporal CT showed abnormal pneumatization of the occipital bone and C1 vertebra. A definite cortical defect resulted with the pre

A Comprehensive Review of Congenital Lumbar Synostosis and Associated Findings.

Congenital vertebral synostosis (CVS) is a rare developmental condition due to failure of vertebral segmentation. Vertebrae and their intervertebral discs differentiate and resegment at the time of organogenesis during fetal life. Failure of this embryological process can result in the limitation of mobility of the involved segment. This inappropriate segmentation thus results in vertebral fusion or a block vertebra with subsequent vertebral synostosis. Long-term, such segmental fusion can increase osteoarthritis at levels below and above the fused segment due to excessive wear on these joints

Rosai-dorfman disease in thoracic spine: a rare case of compression fracture.

Sinus histiocytosis with massive lymphadenopathy known as Rosai-Dorfman disease is characterized by painless bilateral cervical lymphadenopathy. Extranodal manifestations are uncommon and spinal involvement is rare. A 15-year-old man presented with intermittent midthoracic back pain only. He had no specific findings on neurologic examinations, hematologic and biochemical laboratory tests. Radiological examination of thoracic spine revealed collapse of T6 vertebrae with thoracic kyphosis and osteolytic lesion of T12 vertebra body. He underwent a removal of bone tumor, anterior reconstruction wi

The contribution made by a single somite to the vertebral column: experimental evidence in support of resegmentation using the chick-quail chimaera model.

The somitic involvement in the formation of the vertebral column was examined using the chick-quail chimaera model. Single cervical somites from quail donor embryos were transplanted into similarly staged chick host embryos. Following further incubation, serial sections of variously staged embryos were stained with the Feulgen reaction to distinguish the two cell populations. Quail cells were generally located within a delimited region in one half of each of the two adjacent vertebrae, as well as in the intervening disc. The horizontal plane of division through each vertebra passed approximate

Surgical Correction of Distal Junctional Kyphosis.

Distal junctional kyphosis (DJK) is defined as the development of a kyphotic angulation over 10 degrees below a fusion construct and has been described as a complication of the treatment of adolescent idiopathic scoliosis, Scheuermann kyphosis, adult spinal deformity, and cervical deformity. There are some inherent risk factors to DJK: multilevel fusions, damage to the midline soft tissues including interspinous/supraspinous ligaments, T5-T12 thoracic kyphosis, T11-L2 thoracolumbar kyphosis, and increased mismatch between cervical lordosis and T1 slope. A 53-year-old male presented with cervic

[Myxopapillary ependymoma of the filum terminale with a holocord cyst: a case report].

This paper describes a case of myxopapillary ependymoma of the filum terminale with a holocord cyst. A fifteen-year-old boy was admitted because of dysarthria, dysphagia and tetraparesis. He also suffered from severe lumbago and ankle pain for three years. On admission his vital signs were stable and neurological examination revealed lower cranial nerve palsies (from 9th through 12th), tetraparesis, bilateral impairment of vibratory and position sense of the lower extremities and urinary incontinence. All deep tendon reflexes were hypoactive bilaterally. Babinski reflex and Lasègue sign were n

Postural Consequences of Cervical Sagittal Imbalance: A Novel Laboratory Model.

STUDY DESIGN: A biomechanical study using human spine specimens. OBJECTIVE: To study postural compensations in lordosis angles that are necessary to maintain horizontal gaze in the presence of forward head posture and increasing T1 sagittal tilt. SUMMARY OF BACKGROUND DATA: Forward head posture relative to the shoulders, assessed radiographically using the horizontal offset distance between the C2 and C7 vertebral bodies (C2-C7 [sagittal vertical alignment] SVA), is a measure of global cervical imbalance. This may result from kyphotic alignment of cervical segments, muscle imbalance, as well

[Arterial vascularization of the vertebral column. Injection-corrosion-fluorescence technic].

The authors have used the technique of injection-corrosion-fluorescence for a better knowledge of the arterial vascularization of the vertebral column of dogs. The vascular molds have been observed under ultraviolet light, and the magnificent fluorescence allowed easy reading of the results. At the cervical level they are the vertebral and the deep cervical arteries as the main arterial sources, at the thoracic level it is the intercostal, and in the lumbar region the lumbar arteries.

Transpedicular Approach for Ventral Epidural Abscess Evacuation in the Cervical Spine.

OBJECTIVE: Spinal epidural abscess may require prompt surgical intervention. Ventral cervical abscesses pose a particular challenge regarding the approach for surgical evacuation. The aim of this article was to describe the technical nuances of a posterior transpedicular cervical approach for evacuation of a ventral epidural abscess. METHODS: After a standard laminectomy, a foraminotomy was performed to identify the exiting nerve root. Then the medial aspect of the pedicle below the nerve was drilled. This allowed the insertion of a dissector to reach the ventral epidural space and drain the
