T-type channels control the opioidergic descending analgesia at the low threshold-spiking GABAergic neurons in the periaqueductal gray.

Endogenous opioids generate analgesic signals in the periaqueductal gray (PAG). However, because cell types in the PAG are difficult to identify, its neuronal mechanism has remained poorly understood. To address this issue, we characterized PAG neurons by their electrical properties using differentially labeled GABAergic and output neurons in the PAG. We found that GABAergic neurons were mostly fast-spiking cells and could be further divided into two distinct classes: with or without low-threshold spikes (LTS) driven by T-type channels. In contrast, the PAG output neurons lacked LTS and showed

Antigen-specific IgG antibodies in feather duvet lung.

BACKGROUND: Feather duvet lung (FDL) is a rare subgroup of bird fancier's lung (BFL). We were interested in determining antigen-specific IgG antibodies in patients with FDL and comparing them with those with BFL. MATERIAL AND METHODS: Specific IgG antibodies against goose and duck feathers, analysed with an automated fluorimetric enzyme immunoassay, were measured in healthy subjects (group A, n = 30), in patients with FDL (group B, n = 10) and with BFL (group C, n = 35); typical specific IgG antibodies of BFL in groups B and C. RESULTS: An optimal threshold value for antibodies against goose

Effects of duodenal lipids on gastric sensitivity and relaxation in patients with ulcer-like and dysmotility-like dyspepsia.

BACKGROUND/AIMS: Functional dyspepsia (FD) according to Rome II is divided into ulcer-like dyspepsia (ULD) and dysmotility-like dyspepsia (DLD). The rationale behind this is the assumption that the underlying pathophysiology is different, but this is largely unexplored. Our aim was to elucidate the differences in gastric sensorimotor function in these subgroups of FD. 13 healthy controls (HC) and 20 patients with FD, 10 ULD and 10 DLD, were included. METHODS: A feeding tube was placed fluoroscopically in the proximal duodenum and a barostat balloon was placed in the proximal stomach. Sensory

Parasympathetic overactivity and its evaluation in patients with sinus nodal dysfunction.

In 17 controls, and 17 patients with sinus nodal dysfunction, an electrophysiologic study was made of sinus nodal function and atrioventricular nodal conduction in the basal state. The study was then repeated in all patients after atropine. The heart rate, mean sinus cycle length, variations of sinus cycle length, sinus node recovery times, sinuatrial conduction time, AH interval, and atrioventricular nodal Wenckebach threshold were significantly different in patients from those of controls. All these parameters changed significantly in patients after atropine, and were comparable to those of

A transient outward current in NG108-15 neuroblastoma x glioma hybrid cells.

Outward currents were recorded from voltage-clamped NG108-15 mouse neuroblastoma X rat glioma hybrid cells, differentiated with prostaglandin E1. Depolarising voltage steps from -70 mV, evoked a transient outward current from a threshold of -30 mV. The outward current showed complete inactivation at potentials positive to -10 mV. Inactivation was removed by hyperpolarisation with half-inactivation at -53 mV. The time course of the inactivation could be best fitted by two exponentials with mean time constants of 280 ms and 1.6 s at +80 mV. Tail current measurements showed a shift in the reversa

Effects of noise, hypothermia and barbiturate on cochlear electrical activity.

Pentobarbital has been reported as both increasing and decreasing the effects of noise trauma on the inner ear. In the C57BL/6 laboratory mouse, the effects depend upon which electrical events are measured and whether or not the hypothermic effects of pentobarbital are counteracted. Pentobarbital-induced hypothermia per se increased the latency of both the high-intensity-threshold, short-latency (H) and low-intensity-threshold, long-latency (L) waves by approximately 60 mus per degrees C decrease, had no significant effect on H-wave amplitude, but caused a severe reduction of the L-wave amplit

The basaloid-spectrum of salivary gland tumors: A focused review with practical diagnostic considerations.

Basaloid salivary gland neoplasms are a diverse and varied group of benign and malignant tumors. The term 'basaloid' is broadly used in reference to cells with elevated nuclear to cytoplasmic ratio, sparse cytoplasm, and hyperchromatic nuclei. However, a subset may also fit within the "small round blue cell tumor" morphologic category or the "biphasic" salivary gland tumor category. Furthermore, there are no established thresholds for the proportion of basaloid tumor cells needed to consider a tumor within the basaloid spectrum. Given the implicit variability in what is considered a basaloid s

Are surgical masks manufactured from sterilisation wrap safe?

BACKGROUND: Due to regional shortages some health services have proposed using surgical masks manufactured from sterilisation wrap. However, there has been little assessment of the safety of this practice. Therefore, we developed our own prototypes and evaluated whether they met regulatory standards. METHODS: Surgical mask prototypes were manufactured from two thickness grades of commercial sterilisation wrap. Safety was assessed in the context of regulatory standards. As it was not previously reported, we developed and performed differential pressure and synthetic blood penetration resistanc

Simultaneous detection and quantitation of Chikungunya, dengue and West Nile viruses by multiplex RT-PCR assays and dengue virus typing using high resolution melting.

Chikungunya (CHIKV), Dengue (DENV) and West Nile (WNV) viruses are arthropod-borne viruses that are able to emerge or re-emerge in many regions due to climatic changes and increase in travel. Since these viruses produce similar clinical signs it is important for physicians and epidemiologists to differentiate them rapidly. A molecular method was developed for their detection and quantitation in plasma samples and a DENV typing technique were developed. The method consisted in performing two multiplex real-time one-step RT-PCR assays, to detect and quantify the three viruses. Both assays were c

[Quantitative vertebral x-ray computed tomography. Results in 105 women consulting for osteoporosis].

Quantitative vertebral CT scan imaging is a method developed to provide direct measurements of mineralization of vertebral body spongy tissue, and is presently the most precise procedure for the early detection of spinal osteoporosis. A fracture threshold has been defined below which are found 95% of patients with a crushed vertebra: it is situated at 70% of the value for mineralization normal for the age of patients. Patients with marked reductions in their level of mineralization can be kept under surveillance before the onset of fracture. In patients with vertebral collapses the density is

Differential diagnosis of the small renal masses: role of the apparent diffusion coefficient of the diffusion-weighted MRI.

INTRODUCTION: Renal cell carcinoma (RCC) accounts for approximately 3% of adult malignancies and more than 90% of neoplasms arising from the kidney. Uninformative percutaneous kidney biopsies vary from 10 to 23%. As a result, 7.5-33.6% of partial nephrectomies in patients with small renal masses (SRM) are performed on benign renal tumors. The aim of this study was to assess the feasibility of the apparent diffusion coefficient (ADC) of the diffusion-weighted imaging (DWI) of MRI, as RCC imaging biomarker for differentiation of SRM. METHOD: Adult patients (n = 158) with 170 SRM were enrolled i

MOSIC: Model-Agnostic Optimal Subgroup Identification with Multi-Constraint for Improved Reliability.

Current subgroup identification methods typically follow a two-step approach: first estimate conditional average treatment effects (CATEs) and then apply thresholding or rule-based procedures to define subgroups. While intuitive, this decoupled approach fails to incorporate key constraints essential for real-world clinical decision-making-such as subgroup size and propensity overlap. These constraints operate on fundamentally different axes than CATE estimation and are not naturally accommodated within existing frameworks, thereby limiting the practical applicability of these methods. We propo

UBA1 as a proteostasis-innate immunity checkpoint: lessons from vexas syndrome and therapeutic implications.

Ubiquitin-like modifier activating enzyme 1 (UBA1) occupies the apex of the canonical ubiquitination cascade and supplies activated ubiquitin to a broad network of E2 conjugating enzymes and E3 ligases. Although traditionally regarded as an essential housekeeping enzyme, human genetics has revealed a more instructive function for UBA1: it determines whether proteostatic stress is buffered, converted into inflammatory adaptation, or progresses to cell death. Somatic UBA1 mutations causing VEXAS syndrome provide a natural experiment in this threshold biology. Partial and compartment-selective lo

Lag-induced critical transitions to extinction in replicating systems: the τ-tipping mechanism.

Replicating systems sustained by error-prone enzymatic amplification can undergo critical transitions between persistence and extinction. In RNA viruses, such transitions are classically governed by mutation rates and fitness landscape topographies, giving rise to error thresholds and lethal mutagenesis. Motivated by experimental evidence that polymerase-targeting antivirals limit replication, we analyze replicating systems with explicit time lags in replication-enzyme availability by means of delay differential equations. Our approach is not equivalent to a simple renormalization of the rate

The Role of Quantitative PCR in Evaluating the Clinical Significance of Human Bocavirus Detection in Children.

Human bocavirus (HBoV) has emerged as a significant pathogen primarily associated with respiratory infections in children. This study aimed to evaluate the clinical relevance of HBoV infection by quantifying viral loads in nasopharyngeal swabs from hospitalized children with acute respiratory infections (ARIs) and investigating correlations with clinical outcomes. A total of 957 children were tested, with HBoV detected in 73 cases (7.6%), either as a sole infection or co-infection with other respiratory viruses. Quantitative polymerase chain reaction (qPCR) was employed to measure viral load,

Iatrogenic adrenal atrophy following oral corticotherapy is not reliably identified ultrasonographically in cats.

BACKGROUND: In dogs, corticosteroid administration is known to decrease adrenal gland height when measured ultrasonographically. However, comparable information is lacking in cats. OBJECTIVES: i) Validate that the adrenal height of our control population without corticosteroid administration was similar to previous data, ii) determine effects of dose and duration of oral corticosteroid therapy on adrenal height, and iii) determine an adrenal size threshold to differentiate cats receiving corticosteroids or not. ANIMALS AND PROCEDURES: Adult cats (N = 308) that received abdominal ultrasonogra

Rasch analysis to evaluate the motor function measure for patients with facioscapulohumeral muscular dystrophy.

Patient-relevant outcome measures for facioscapulohumeral muscular dystrophy (FSHD) are needed. The motor function measure (MFM) is an ordinal-based outcome measure for neuromuscular disorders, but its suitability to measure FSHD patients is questionable. Here, we performed Rasch analyses on MFM data from 194 FSHD patients to assess clinimetric properties in this patient group. Both the total scale and its three domains were analyzed (D1: standing position and transfers; D2: axial and proximal motor function; D3: distal motor function). Fit to the Rasch model, sample-item targeting, individual
