In previous studies, the use of thyroid cartilage needle electrodes (TCN) was defined as an inexpensive method for intra-operative nerve monitoring (IONM) in thyroid surgery. This multi-center retrospective study aims to determine the effectiveness and reliability of TCN in thyroid surgery. Patients operated on between January 2018 and August 2023 from five centers were included in this study. Demographic data, indications, type of surgery, IONM recording system, pre-post-resection vagus nerve (V1-V2), pre-post-resection recurrent laryngeal nerve (R1-R2) amplitudes and latency values, type of

BACKGROUND: Hemodynamic disturbances cause half of the perioperative strokes following carotid endarterectomy (CEA). Guidelines strongly recommend strict pre- and postoperative blood pressure (BP) monitoring in CEA patients, but do not provide firm practical recommendations. Although in the Netherlands 50 centres perform CEA, no national protocol on perioperative hemodynamic, and cerebral monitoring exists. To assess current monitoring policies of all Dutch CEA-centres, a national survey was conducted.
METHODS: Between May and July 2017 all 50 Dutch CEA-centres were invited to complete a 42-q

BACKGROUND: Chronic noncancer pain is a global public health challenge. It is imperative to identify biological markers ("biomarkers") to understand the mechanisms underlying chronic pain and to monitor pain over time and after interventions. Transcranial magnetic stimulation (TMS) is a promising method for this purpose.
OBJECTIVES: To examine differences in TMS-based outcomes between persons with chronic pain and healthy controls (HCs) and/or before versus after pain-modulating interventions and relationships between pain measures and TMS outcomes; To summarize the neurophysiological mechani

OBJECTIVE: The surgical resection of intramedullary spinal cord tumors (IMSCTs) is associated with significant morbidity. Intraoperative neuromonitoring (IONM) with transcranial myogenic motor evoked potentials (m-MEP) and D-wave aims to reduce new postoperative motor deficits. Still, their comparative ability to predict short- and long-term outcomes is incompletely defined. We evaluated the diagnostic performance of m-MEP and D-wave monitoring at multiple postoperative time points and examined their influence on intraoperative decision-making.
METHODS: In this single-institution observationa

OBJECTIVE: The objective of this study was to evaluate the prognostic value of intraoperative blink reflex (BR) monitoring in predicting postoperative facial nerve outcomes following vestibular schwannoma (VS) resection. Given the limitations of existing intraoperative neuromonitoring (IONM) techniques, this study aimed to determine whether BR loss correlates with worsened facial nerve function, and to assess its potential as an adjunctive tool for surgical decision-making.
METHODS: The authors conducted a retrospective review of adult patients who underwent VS resection between January 2021

BACKGROUND: The re-organization of motor functions is a sign of adaptive brain plasticity to early brain damage and understanding its mechanisms may play a key role in the future treatment modalities. This study provides direct evidence of neural plasticity in four patients with early brain damage, who underwent epilepsy surgery due to drug-resistant epilepsy.
METHODS: In this retrospective analysis, four patients with early brain damage who underwent resective or disconnective epilepsy surgery were examined. Intraoperative neuromonitoring was employed to preserve motor function during surger

BACKGROUND: Transcranial color Doppler (TCD) is currently the only noninvasive bedside tool capable of providing real-time information on cerebral hemodynamics. However, being operator dependent, TCD monitoring is not feasible in many institutions. Robotic assisted TCD (ra-TCD) was recently developed to overcome these constraints. The aim of this study was to evaluate the safety and feasibility of cerebral monitoring with a novel ra-TCD in acute neurovascular care.
METHODS: This is a two-center prospective study conducted between August 2021 and February 2022 at Padua University Hospital (Pad

Deep brain stimulation (DBS) is a therapy for Parkinson's disease (PD) and essential tremor (ET). The mechanism of action of DBS is still incompletely understood. Retrospective group analysis of intra-operative data recorded from ET patients implanted in the ventral intermediate nucleus of the thalamus (Vim) is rare. Intra-operative stimulation tests generate rich data and their use in group analysis has not yet been explored.Objective.To implement, evaluate, and apply a group analysis workflow to generate probabilistic stimulation maps (PSMs) using intra-operative stimulation data from ET pat

OBJECTIVE: We examined the feasibility of using cortico-cortical evoked potentials (CCEPs) to monitor the major cortical white matter tract involved in language, the arcuate fasciculus (AF), during surgery under general anaesthesia.
METHODS: We prospectively recruited nine patients undergoing surgery for lesions in the left peri-sylvian cortex, for whom awake surgery was not indicated. High angular resolution diffusion imaging (HARDI) tractography was used to localise frontal and temporal AF terminations, which guided intraoperative cortical strip placement.
RESULTS: CCEPs were successfully

BACKGROUND: In conus medullaris and cauda equina surgery, identification of the sacral nerve roots may be uncertain in spite of their anatomical/radiological landmarks. Mapping the sacral roots by recording the muscular responses to their stimulation may benefit from EMG recording of the External Anal sphincter (EAS) in addition to the main muscular groups of the lower limbs.
METHOD: In a consecutive series of 27 lumbosacral dorsal rhizotomy (DRh), authors carried out a prospective study on the reliability of the EMG recording of the EAS for identification of the S1 and S2 sacral roots.
