King's syndrome with malignant hyperthermia. Potential outpatient risks.

King's syndrome is a sporadic genetic syndrome exhibiting characteristic facial features, short stature, and subclinical myopathy. Affected individuals are susceptible to malignant hyperthermia (MH) induced by anesthesia, stress, or trauma. The incidence of King's syndrome is uncertain as all reported cases were first recognized following an episode of MH. We report a case of King's syndrome without the cryptorchidism and pectus carinatum associated with the syndrome. King's syndrome's possible link to Noonan's syndrome is discussed herein, and the diagnosis and management of MH are briefly re

Pattern of CNS recovery following reversal of neuromuscular blockade. Comparison of atropine and glycopyrrolate.

Recovery from anaesthesia was compared, in a group of patients (n = 25) receiving a mixture of glycopyrrolate and neostigmine (to reverse non-depolarizing neuromuscular blockade), with recovery in a group of patients (n = 25) receiving an atropine-neostigmine mixture. Recovery following anaesthesia was more rapid in the patients receiving the glycopyrrolate-neostigmine mixture.

Failure of two benzodiazepines to prevent suxamethonium-induced muscle pain.

In a randomised double-blind trial carried out on fit, unpremedicated patients undergoing standard minor operations with early postoperative mobility, using a standard form of anaesthesia, pretreatment with diazepam 0.15 mg/kg or midazolam 0.1 mg/kg failed to reduce significantly the incidence of postoperative muscle pains following suxamethonium 1 mg/kg. By contrast, tubocurarine 0.05 mg/kg proved to be effective as a pretreatment. Neither benzodiazepine influenced the incidence or severity of fasciculations seen with suxamethonium or the duration of neuromuscular block. Tubocurarine virtuall

Current Practices and Safety of Medication Use During Pediatric Rapid Sequence Intubation.

OBJECTIVES: This study aimed to characterize medication-related practices during and immediately -following rapid sequence intubation (RSI) … not associated with development of adrenal insufficiency. Rocuronium was the most used NMBA (78%). Succinylcholine use was low (11%) and administered despite hyperkalemia in 2 patients. Postintubation sedation and analgesia were not used or inadequate based on timing of initiation in many patients who received a non-depolarizing NMBA. CONCLUSIONS: Medication pr …

Iatrogenic pediatric hydroxocobalamin overdose.

INTRODUCTION: Hydroxocobalamin, a precursor molecule to vitamin B12, has emerged as the preferred empiric treatment for patients rescued fro … rom direct laryngoscopy, and sinoatrial muscarinic receptor stimulation caused by repeated doses of succinylcholine. In all, we were unable to appreciate any complications due to excess hydroxocobalamin administration.

Utility of basophil activation testing to assess perioperative anaphylactic reactions in real-world practice.

INTRODUCTION: Perioperative anaphylactic reactions due to drugs and substances associated with general anesthesia can potentially be life-th … isatracurium, etomidate, neostigmine, midazolam, mivacurium, pancuronium, propofol, pyridostigmine, succinylcholine, sufentanil, thiopental, vecuronium), and other individual substances. RESULTS: Three patients showed positive results in the BAT: One to metamizole, one to PPL, and one to pancuronium. BATs with these substances were negative in controls. CONCLUS …

[Atypical reaction to anesthesia in Duchenne/Becker muscular dystrophy].

BACKGROUND AND OBJECTIVES: Duchenne/Becker muscular dystrophy affects skeletal muscles and leads to progressive muscle weakness and risk of atypical anesthetic reactions following exposure to succinylcholine or halogenated agents. The aim of this report is to describe the investigation and diagnosis of a patient with Becker muscular dystrophy and review the care required in anesthesia. CASE REPORT: Male patient, 14 years old, referred for hyperCKemia (chronic increase of serum creatine kinase levels – CK), with CK values of 7,779–29,040 IU.L−1 (normal 174 IU.L−1). He presented with a discrete…

An RYR1 mutation associated with malignant hyperthermia is also associated with bleeding abnormalities.

Malignant hyperthermia is a potentially fatal hypermetabolic disorder triggered by halogenated anesthetics and the myorelaxant succinylcholine in genetically predisposed individuals. About 50% of susceptible individuals carry dominant, gain-of-function mutations in RYR1 [which encodes ryanodine receptor type 1 (RyR1)], though they have normal muscle function and no overt clinical symptoms. RyR1 is predominantly found in skeletal muscle but also at lower amounts in immune and smooth muscle cells, suggesting that RYR1 mutations may have a wider range of effects than previously suspected. Mild bl…

Stable gastric pentadecapeptide BPC 157 and bupivacaine.

Bupivacaine toxicity following accidental overdose still lacks therapeutic solution. However, there are major arguments for testing BPC 157 against bupivacaine toxicity in vivo in rats, in particular, and then finally, in vitro. These are: the lack of any known BPC 157 toxicity, a lifesaving effect via the mitigation of arrhythmias in rats underwent hyperkalemia or digitalis toxicity, the elimination of hyperkalemia and arrhythmias in rats underwent succinylcholine toxicity and finally, the reduction of potassium-induced depolarization in vitro (in HEK293 cells) in severe hyperkalemia. Most im…

Residual neuromuscular blockade in a real life clinical setting: correlation with sugammadex or neostigmine administration.

BACKGROUND: The aim of this study was to identify the frequency of residual neuromuscular blockade (RNMB) in the postanesthesia care unit (P … Five hundred twenty patients were studied; 90.4% received rocuronium, 9.2% cis-atracurium, and 0.4% succinylcholine. The prevalence of RNMB was 10.8%. A significant difference (P=0.0006) was detected between patients who received neostigmine versus sugammadex, with the latter showing higher TOF values postoperatively, although the actual incidence of RNMB did not …

Hemodynamic improvement of a LUCAS 2 automated device by addition of an impedance threshold device in a pig model of cardiac arrest.

INTRODUCTION: The combination of the LUCAS 2 (L-CPR) automated CPR device and an impedance threshold device (ITD) has been widely implemented in the clinical field. This animal study tested the hypothesis that the addition of an ITD on L-CPR would enhance cerebral and coronary perfusion pressures. METHODS: Ten female pigs (39.0 ± 2.0 kg) were sedated, intubated, anesthetized with isofluorane, and paralyzed with succinylcholine (93.3 μg/kg/min) to inhibit the potential confounding effect of gasping. After 4 min of untreated ventricular fibrillation, 4 min of L-CPR+an active ITD or L-CPR+a sham…
