[Parental representations of children's acute appendicitis and impact on its surgical management in three departments].

INTRODUCTION: Appendicitis is well known by the general and medical population. Yet, the number of appendectomies varies by a factor of one to four depending on French departments.Purpose of research: This study aimed to compare, according to French schooling departments, the representations and attitudes of parents facing abdominal pain of their children, and their use of health care system. We selected three departments which have different proportions of use of appendectomy according to the DRESS's rapport of 2014. Meurthe-et-Moselle's rank is between 15.65 and 23.48/10 000 people, Seine-et…

Should we remove the normal-looking appendix during operations for borderline mucinous ovarian neoplasms?: A retrospective study of 129 cases.

INTRODUCTION: The aim of this study was to determine how often the appendix is involved or the primary source of cancer in women undergoing surgery for mucinous borderline ovarian tumour (mBOT) or invasive mucinous ovarian tumour (IMOT) and to evaluate whether appendectomy is necessary. METHODS: The hospital database was searched for women who underwent surgery and whose final diagnosis was mBOT, IMOT or mucinous appendix carcinoma between 1998 and 2014. RESULTS: One hundred and twenty-nine cases were identified, including 69 mBOT, 51 IMOT and nine primary mucinous appendix carcinomas. Of 97…

Interval appendicitis.

There are a few studies that have focused on histopathologic findings in interval appendectomies compared to acute appendicitis and negative appendectomies. Our aim is to compare histopathologic features in interval appendectomies to a similar control group of stump appendectomies. A retrospective review study was conducted over a 10-year period. We found 21 (0.3 %) patients [age range: 10-70 years, average age: 40 years, male to female ratio: 1.3 to 1.0, average time interval: 6 weeks] who had interval appendectomies. Seventeen patients (81 %) showed three main patterns: xanthogranulomatous i

Medical Versus Surgical Management of Acute Appendicitis in a Regional Hospital in South Africa: A Six-Year Review.

BACKGROUND: Appendectomy is the current standard of care for acute appendicitis. However, accumulating international evidence suggests that medical antibiotic therapy is not inferior to appendectomy. Local factors may work for and against a medical approach. While prospective randomized trials are still to be performed in a South African setting, this study aimed to describe existing data within a geographically defined area. MATERIALS AND METHODS: A retrospective audit from 01/01/2018-31/12/2023 was performed at George Regional Hospital, South Africa. All adults and children over 12 years wi…

Effectiveness of protocol-based medical treatment in selected uncomplicated acute appendicitis: a retrospective cohort study.

OBJECTIVE: To describe the short-term treatment outcomes and intermediate-term appendectomy rates following protocol-based medical treatment in carefully selected patients with uncomplicated acute appendicitis without appendicolith. METHODS: This single-arm retrospective cohort study was conducted at a single tertiary center. Patients with uncomplicated acute appendicitis without appendicolith on computed tomography (CT) were treated following a standardized protocol consisting of intravenous antibiotics, a 48-hour in-hospital observation period, and systematic follow-up. The primary outcome …

Management for Acute Uncomplicated Appendicitis During Pregnancy: National Trends and Patient Outcomes.

OBJECTIVE: This study analyzes national trends in the management of uncomplicated appendicitis during pregnancy, comparing outcomes for nonoperative management (NOM) and appendectomy. BACKGROUND: In the nonpregnant population, several randomized controlled trials demonstrated noninferiority of NOM compared with appendectomy for acute uncomplicated appendicitis. However, it remains unclear whether these findings are generalizable to pregnant patients. METHODS: The National Inpatient Sample was queried for pregnant women diagnosed with acute uncomplicated appendicitis from January 2003 to Sept…

A Comparative Study of Raja Isteri Pengiran Anak Saleha and Alvarado Scores to Diagnose Acute Appendicitis.

BACKGROUND: Acute Appendicitis is a clinical diagnosis with atypical presentation in young, elderly, females, genitourinary and gynecological conditions. Delayed appendectomy increases the risk of appendicular perforation, sepsis morbidity and mortality. Literature reports as high as 20-40% negative appendectomy. Raja Isteri Pengiran Anak Saleha score has come with higher sensitivity and diagnostic accuracy than Alvarado score in Asian population. This study aims to compare RIPASA and Alvarado score for diagnostic accuracy. METHODS: Appendectomy patients at Patan Hospital from April to Septem…

Deciduosis of the appendix: a rare cause of acute abdomen during pregnancy (a case report).

Appendectomy is the most common non-obstetric surgical intervention in pregnant patients. In rare cases, deciduosis can develop during pregnancy in the appendix and cause inflammation through appendiceal occlusion by ectopic decidua tissue. We report a 28-year-old woman at 32 weeks of pregnancy, who presented at the emergency room with the diagnosis of an acute appendicitis. After successful appendectomy and histopathological examination of the appendectomy specimen, a diagnosis of appendiceal deciduosis with acute appendicitis was established. Here, we discuss the origin of appendiceal decidu…

One-Year Healthcare Utilization and Costs After Emergency Department Encounters for Uncomplicated Appendicitis: A Statewide Observational Study.

OBJECTIVE: Non-operative management with antibiotics has emerged as an alternative to appendectomy for uncomplicated appendicitis. This study compared one-year healthcare costs and utilization outcomes after operative versus non-operative management of uncomplicated appendicitis using statewide administrative data. METHODS: We conducted a statewide observational study using linked Healthcare Cost and Utilization Project (HCUP) databases from Maryland (2016-2021) including emergency department, inpatient, and ambulatory surgery encounters. Adults with uncomplicated appendicitis, identified by …

Endoscopic Management of Appendiceal Stump Lesions via Transcolonic Access: A Safe and Feasible Minimally Invasive Strategy.

BACKGROUND AND AIM: Transcolonic endoscopic appendectomy is an emerging minimally invasive alternative to traditional appendectomy, offering potential benefits such as reduced postoperative pain, faster recovery, and avoidance of external incisions. The variable anatomical location of the appendix, however, influences both the surgical approach and technical difficulty. The appendiceal stump-defined as the residual portion of the appendix after prior appendectomy-poses a unique challenge, as the main body of the appendix has already been removed. This study aimed to evaluate the feasibility, s…

The feasibility of bipolar tissue sealer for appendix stump closure.

BACKGROUND: Appendiceal stump closure is a highly important step in laparoscopic appendectomy, especially for post-operative complications. The aim of this study is to compare the effects of suture ligation and bipolar tissue sealer techniques on burst pressure using flesh appendectomy specimens. MATERIAL AND METHODS: Appendectomy specimens of 32 patients with grade I-II disease were included in the study. Perforated or necrotic appendices and specimens ineligible for pressure measurement were excluded from the study. Appendiceal stumps of 16 patients in group 1 were double-ligated with 2/0 p…

Appendicitis in the modern era: universal problem and variable treatment.

BACKGROUND: Acute appendicitis is the second most common gastrointestinal diagnosis mandating urgent operation in the U.S. The current state of adult appendectomy, including patient and hospital characteristics, complications, and predictors for complications, are unknown. METHODS: Retrospective review of U.S. Nationwide Inpatient Sample 2003-2011 for appendectomy in ≥18-year-olds was performed. Primary outcomes measures included postoperative complications, length of stay, and patient mortality. Categorical variables were analyzed by χ2, trend analyses by Cochran-Armitage. The Dilemma of Stump Appendicitis: A Case Report.

Stump appendicitis is an uncommon but clinically significant delayed complication of appendectomy, arising from inflammation of remnant appendiceal tissue after the initial procedure. Because most clinicians do not consider appendicitis in a post-appendectomy patient, stump appendicitis frequently results in diagnostic delay and increased morbidity. We report the case of a 27-year-old man from a northern state of India who presented with features of generalized peritonitis one year after undergoing open appendectomy for acute appendicitis. Radiological evaluation suggested inflammation but fai…

An incidental finding of ANET in a patient with perforated appendix: A case report.

INTRODUCTION AND IMPORTANCE: Appendiceal neuroendocrine tumors (ANETs) are incidentally found in 0.2-0.7% of appendectomies for suspected appendicitis. CASE PRESENTATION: A 29-year-old female presented with pain in the right lower quadrant of her abdomen for the past 48 h. On emergency appendectomy for suspected acute appendicitis, a perforated appendix was found. Histopathological examination showed grade 1 (low) ANET. CLINICAL DISCUSSION: Appendiceal neuroendocrine neoplasms (ANENs) are commonly found at the tip of the appendix and are treated with appendectomy alone. For few ANENs located…

Surgical management of cecal diverticulitis.

Ninety surgical cases of cecal diverticulitis at the University of Hawaii hospitals were reviewed from 1980 to 1991. Seventy-eight percent of the study group were of Asian descent, with a mean age of 41.7 years. Right lower quadrant pain and tenderness were the only constant findings, occurring in 86 and 87 of the 90 patients, respectively. The most common preoperative diagnosis was acute appendicitis, occurring in 73% of patients. A right colectomy or cecectomy was performed in 49 patients, an appendectomy in 29, and a diverticulectomy in 10. Seventeen complications occurred, only 1 of which …

The Clinical Relevance of an Inflamed Appendix in Crohn's Disease.

BACKGROUND AND AIMS: An appendectomy for appendiceal inflammation has been suggested to ameliorate the clinical course of patients with ulcerative colitis [UC]. In contrast, for Crohn's disease [CD] an inverse association has been suggested with a higher incidence of CD and worse prognosis after appendectomy. The aim of this study was to analyse the clinical relevance of an inflamed appendix in CD patients undergoing ileocaecal resection [ICR]. METHODS: All consecutive patients undergoing primary ICR between 2007 and 2018 were considered for inclusion. Microscopic data of available appendicea…

Incidentally discovered intestinal nonrotation at time of bariatric surgery: Which operation to perform?

BACKGROUND: An incidental finding of intestinal nonrotation at the time of bariatric surgery poses the following 2 dilemmas: (1) which operation to perform, and (2) whether an appendectomy should be performed concurrently. OBJECTIVES: To review the experience of 2 Bariatric Centers of Excellence with laparoscopic sleeve gastrectomy (LSG) in patients with intestinal nonrotation, and to perform a systematic review of the literature on this topic. SETTING: Two Bariatric Centers of Excellence as designated by the Ontario Bariatric Network. METHODS: A chart review of all LSG cases performed in p…

[Training in laparoscopy and appendicitis].

BACKGROUND: In order to improve laparoscopic skills, appendectomy is the most common procedure because of its high frequency and low difficulty. In spite of that, during the learning curve (each surgeon´s first 35 interventions) the incidence of complications may increase, so improvement in training means a bigger risk for some patients. METHODS: We retrospectively reviewed major complications (intra-abdominal abscess, intestinal occlusion, hemorrhage) of 1,710 appendectomies performed at our service between 1997 and 2013. We divided them in three groups: open appendectomy (OA, n= 1,258), lap…

[Laparoscopy useful in the diagnosis and surgery of acute appendicitis].

The value of diagnostic laparoscopy was evaluated in 74 patients who had clinical signs of acute appendicitis. The average age was 27 years (5-67). The laparoscopically determined degree of inflammation was compared with the results of histologic examination, and the results of laparoscopic appendectomy were evaluated. In 6 patients laparoscopic appendectomy was not performed, because of infiltrative disease (twice), twisted ovary, ovulatory bleeding, enteritis and partially incarcerated inguinal hernia. In 2 patients suffering of acute appendicitis conversion to laparotomy was necessary becau…
