Impact of Digitalization on Performance of Dutch Hospitals: Longitudinal Quantitative Study.

BACKGROUND: Hospitals continue to invest heavily to increase their level of digitalization. While advanced digital maturity is assumed to improve hospital performance, empirical evidence remains mixed. This tension is mirrored by the productivity paradox of IT, whereby investments in digital technologies do not consistently translate into observable performance gains. OBJECTIVE: This study aims to examine the relationship between the Healthcare Information and Management Systems Society (HIMSS) Electronic Medical Record Adoption Model (EMRAM) score and hospitals' financial, operational, and w

Leveraging LLMs for Early Diagnosis in the Emergency Department: Comparing ClinicalBERT and GPT-4.

This study explored the potential of LLMs, such as ClinicalBERT and GPT-4, to identify potential diagnoses using early clinical notes from the MIMIC-III dataset. We compared these models across four conditions: circulatory system diseases, respiratory system diseases, septicemia, and pneumonia. ClinicalBERT consistently outperformed the GPT models, with its highest F1-score of 0.952 for respiratory system diseases. The GPT models, while showing high recall, had lower precision, with the highest F1-score of 0.784 achieved by the GPT binary voting method. ClinicalBERT demonstrated strong precisi

Development of Ward Visitor Management System in the COVID-19 Epidemic.

The purpose of this study was to design, develop, and deploy a visitor management system (VMS) to effectively manage visitors during COVID-19. The VMS was designed using the User-Centered Design (UCD) methodology. The iterative process of UCD includes 3 interviews and 5 usability tests and cognitive walkthrough cycles. This system comprised six parts: the WEB server provides visit scheduling service; the database server stores visit data and provides visit data services; the mobile application server provides security checks and scanning services; the electronic medical record (EMR) server pro

Evaluating the accuracy and efficacy of an electronic medical record alert to identify paediatric patients with low-risk febrile neutropenia.

BACKGROUND: Point-of-care decision support, embedded into electronic medical record (EMR) workflows, has the potential to improve efficiency, reduce unwarranted variation and improve patient outcomes. A clinical-facing best practice advisory (BPA) in the Epic EMR system was developed to identify children admitted with low-risk febrile neutropenia (FN) who should be considered for treatment at home after a brief inpatient stay. We evaluated the accuracy and impact of this BPA and identify areas for improvement. METHODS: The low-risk FN BPA was co-designed with key-stakeholders and implemented

A low-cost, endoscopic, digital, still and video photography system for ENT clinics.

Image capture systems that display and record endoscopic images are important for documentation and teaching. We have modified a universal serial bus microscope to couple with most clinical endoscopes used in our practice. This very economical device produces images suitable for teaching, and potentially for clinical use. The implications of this could be significant for teaching, patient education, documentation and the developing world.

Timing of singleton births by onset of labour and mode of birth in NHS maternity units in England, 2005-2014: A study of linked birth registration, birth notification, and hospital episode data.

BACKGROUND: Maternity care has to be available 24 hours a day, seven days a week. It is known that obstetric intervention can influence the time of birth, but no previous analysis at a national level in England has yet investigated in detail the ways in which the day and time of birth varies by onset of labour and mode of giving birth. METHOD: We linked data from birth registration, birth notification, and Maternity Hospital Episode Statistics and analysed 5,093,615 singleton births in NHS maternity units in England from 2005 to 2014. We used descriptive statistics and negative binomial regre

Visual hallucinations in the 19th century: research in a medical archive.

The authors have gathered and analyzed the visual hallucinations described in the mid- to late-19th century from archived medical records of the former psychiatric hospital "Osservanza" in Imola, northern Italy. Though the investigation was not intended as a statistical survey, the principal aim being to classify the hallucinations according to their outward characteristics, the authors have tried to locate the possible sources of these phenomena in folklore and religious iconography.

Randomised trial comparing the recording ability of a novel, electronic emergency documentation system with the AHA paper cardiac arrest record.

OBJECTIVE: To evaluate the ability of an electronic system created at the University of Washington to accurately document prerecorded VF and pulseless electrical activity (PEA) cardiac arrest scenarios compared with the American Heart Association paper cardiac arrest record. METHODS: 16 anaesthesiology residents were randomly assigned to view one of two prerecorded, simulated VF and PEA scenarios and asked to document the event with either the paper or electronic system. Each subject then repeated the process with the other video and documentation method. Five types of documentation errors we

One-year incidence of occupational injuries among teenagers in a Swedish rural municipality.

The aim of this study was to investigate occupational injuries among teenagers in a defined population and geographical area with regard to incidence and severity and to compare our registration system with that of the Swedish Occupational Injury Information System. All occupational accidents in the age group 15-19 years were registered at public health centres. We found a higher incidence compared to school accidents in the same age group as well as in relation to occupational accidents in the older population. Part-time work, lack of experience, temporary employment, unskilled work, and age-

Are women with major depression in pregnancy identifiable in population health data?

BACKGROUND: Although record linkage of routinely collected health datasets is a valuable research resource, most datasets are established fo … 80% of women dispensed an antidepressant did not have depression recorded as a comorbidity on their hospital records. A simple capture-recapture calculation suggests the prevalence of depression in this population of pregnant women to be around 16%. CONCLUSION: No single data source is likely to provide a complete health profile for an individual. For women with …

Systems failure in hospitals--using Reason's model to predict problems in a prescribing information system.

Both hospitals and hospital patients are bearing a massive cost as a result of the occurrence of medication prescribing errors in the public health system (Bates, D., et al. J. Am. Med. Inform. Assoc. 8(4):299-308, 2001; Lombardi, T. Medscape Pharm. Online J. 02(1), 2001; Roberts, M., and Stokes, J. eMed. J. Aust. 168:317-318, 1998). Consequently, it is important to examine and address the possible causes of this problem. In this paper we suggest that poor information systems may be a contributing factor in the occurrence of these errors. We research this issue in an HIV ward of a large public

Electronic operational classification of hospital-onset bacteremia and fungemia in Epic Cosmos: A retrospective observational study.

BACKGROUND: Hospital-onset bacteremia and fungemia (HOB) has been proposed as an electronic surveillance outcome, but deidentified electronic health record data require transparent organism handling when chart review is unavailable. METHODS: We analyzed deidentified HOB records from Epic Cosmos and retained the first positive blood-culture event per admission on or after hospital day 4. Organism text was standardized and linked to a locally developed workbook based on common-commensal organism logic and review by infectious diseases physicians and laboratory directors. Events were assigned to

Ambient Artificial Intelligence Scribes in the Emergency Department: A Scoping Review of Current Evidence.

BACKGROUND: Burnout affects nearly half of physicians in the United States, with emergency physicians (EPs) at particularly high risk. Documentation burden contributes to burnout. Artificial intelligence (AI) ambient scribes have emerged as a potential strategy to reduce documentation workload. While benefits have been reported in outpatient and inpatient settings, impact in the emergency department (ED) remains poorly characterized. OBJECTIVE: This scoping review aimed to identify reports of ambient AI scribe use in the ED, assess effects on documentation time, and evaluate implementation ch

The Impact of Clinical Documentation Integrity Programs on Diagnosis Documentation.

BACKGROUND: Clinical Documentation Integrity (CDI) programs aid in documentation of precise diagnoses. In this study, the authors assessed the rate at which certain diagnoses were present in the Pediatric Health Information System (PHIS) database and evaluated changes in the hospital-specific case mix index (CMI) pre- and post-CDI program inception at six children's hospitals. METHODS: Between 2011 and 2017, CDI programs were implemented at six children's hospitals. The authors queried the PHIS database to compare the following diagnoses in 2010 and 2021: acute respiratory failure, chronic re
