Reinnervated free gracilis flap with basal curtain barrier for tongue reconstruction: a case series and surgical technique.

OBJECTIVE: Conventional free flaps for tongue reconstruction often result in adhesion and limited mobility. This study presents a case serie … se 1 demonstrated significantly superior tongue mobility, higher swallowing efficiency, and clearer speech intelligibility. CONCLUSION: The basal curtain barrier design effectively prevents tethering of the reconstructed tongue to the floor of the mouth. It offers superior functional freedom and rehabilitation outcomes for hemiglossectomy defects compared to sole muscle …

Spasmodic dysphonia, perceptual and acoustic analysis: presenting new diagnostic tools.

In this article, we investigate whether (1) the IINFVo (Impression, Intelligibility, Noise, Fluency and Voicing) perceptual rating scale and (2) the AMPEX (Auditory Model Based Pitch Extractor) acoustical analysis are suitable for evaluating adductor spasmodic dysphonia (AdSD). Voice recordings of 12 patients were analysed. The inter-rater and intra-rater consistency showed highly significant correlations for the IINFVo rating scale, with the exception of the parameter Noise. AMPEX reliably analyses vowels (correlation between PUVF (percentage of frames with unreliable F0/voicing 0.748), runni

[Evoked otoacoustic emission in conduction of dehydration tests in patients with Meniere's disease].

We studied potentiality of the test of evoked otoacoustic emission (EOAE) for monitoring of dynamic changes in the internal ear in conduction of dehydration tests. In parallel to registration of tonal hearing thresholds and speech intelligibility in conduction of dehydration tests we registered two classes of EOAE: delayed EOAE (DEOAE) and distortion product otoacoustic emission (DPOAE). Of 36 examinees, we registered DEOAE prior to dehydration in 22 patients. By audiometry, these patients had hearing under 40 dB lower the hearing threshold at frequencies 250-4000 Hz. In registration of DEOAE …

Signaling of noncomprehension by children and adolescents with mental retardation: effects of problem type and speaker identity.

Previous research has demonstrated considerable within-individual and within-group variability in the signaling of noncomprehension by persons with mental retardation. The first purpose of this study was to determine whether within-individual variability in such signaling was related to differences in the nature of the inadequate message and the identity of the speaker. The second purpose was to evaluate the relationship between within-group variability in noncomprehension signaling and measures of cognition, receptive and excessive language ability, speech intelligibility, and social cognitio…

An evaluation of recent trends in preschool programming for hearing-impaired children.

A longitudinal study was conducted of 139 children with severe and profound hearing losses. The children were between 3 and 5 years of age at the commencement of the 4-year study. The relationship of several background and educational variables with the linguistic, academic, and social development of the children was investigated. Age, hearing loss, and intelligence were related to most of the dependent measures. Relationships involving communication mode (auditory/oral vs. total communication) varied with the measure being considered. The variable of program type (individual vs. group) intera

Rapid development of perceptual gaze control in hearing native signing Infants and children.

Children's gaze behavior reflects emergent linguistic knowledge and real-time language processing of speech, but little is known about naturalistic gaze behaviors while watching signed narratives. Measuring gaze patterns in signing children could uncover how they master perceptual gaze control during a time of active language learning. Gaze patterns were recorded using a Tobii X120 eye tracker, in 31 non-signing and 30 signing hearing infants (5-14 months) and children (2-8 years) as they watched signed narratives on video. Intelligibility of the signed narratives was manipulated by presenting

Ataxic dysarthria.

Although ataxic dysarthria has been studied with various methods in several languages, questions remain concerning which features of the disorder are most consistent, which speaking tasks are most sensitive to the disorder, and whether the different speech production subsystems are uniformly affected. Perceptual and acoustic data were obtained from 14 individuals (seven men, seven women) with ataxic dysarthria for several speaking tasks, including sustained vowel phonation, syllable repetition, sentence recitation, and conversation. Multidimensional acoustic analyses of sustained vowel phonati

A Binaural Cochlear Implant Sound Coding Strategy Inspired by the Contralateral Medial Olivocochlear Reflex.

OBJECTIVES: In natural hearing, cochlear mechanical compression is dynamically adjusted via the efferent medial olivocochlear reflex (MOCR). These adjustments probably help understanding speech in noisy environments and are not available to the users of current cochlear implants (CIs). The aims of the present study are to: (1) present a binaural CI sound processing strategy inspired by the control of cochlear compression provided by the contralateral MOCR in natural hearing; and (2) assess the benefits of the new strategy for understanding speech presented in competition with steady noise with

Lifetime leisure music exposure associated with increased frequency of tinnitus.

Tinnitus has been linked to noise exposure, a common form of which is listening to music as a leisure activity. The relationship between tinnitus and type and duration of music exposure is not well understood. We conducted an internet-based population study that asked participants questions about lifetime music exposure and hearing, and included a hearing test involving speech intelligibility in noise, the High Frequency Digit Triplets Test. 4950 people aged 17-75 years completed all questions and the hearing test. Results were analyzed using multinomial regression models. High exposure to lei…

The effect of talker and intonation variability on speech perception in noise in children with dyslexia.

PURPOSE: In this study, the authors aimed to determine whether children with dyslexia (hereafter referred to as "DYS children") are more affected than children with average reading ability (hereafter referred to as "AR children") by talker and intonation variability when perceiving speech in noise. METHOD: Thirty-four DYS and 25 AR children were tested on their perception of consonants in naturally produced CV tokens in multitalker babble. Twelve CVs were presented for identification in four conditions varying in the degree of talker and intonation variability. Consonant place (/bi/-/di/) and

Effects of Training on the Use of a Manual Microphone Shutoff on a BiCROS Device.

BACKGROUND: Bilateral contralateral routing of signals (BiCROS) hearing aids function to restore audibility of sounds originating from the side of the unaidable ear. However, when speech is presented to the side of the aidable ear and noise to the side of the unaidable ear, a BiCROS arrangement may reduce intelligibility of the speech signal. This negative effect may be circumvented if an on/off switch is available on the contralateral routing of signals (CROS) transmitter. PURPOSE: This study evaluated if the proper use of the on/off switch on a CROS transmitter could enhance speech recognit
