Superficial radial neuritis and De Quervain's disease. Report of three cases.

Three patients with De Quervain's disease developed superficial radial neuritis, a complication which seems not to have been considered in the literature. All 3 patients were women. One responded to conservative therapy and the other 2 required tenolysis of the abductor pollicis longus and extensor pollicis brevis tendon sheaths to relieve symptoms.

MRI of Hands with Early Rheumatoid Arthritis: Usefulness of Three-Point Dixon Sequences to Quantitatively Assess Disease Activity.

The use of efficient treatment with a treat-to-target strategy combined with early detection of the disease completely changed the imaging presentation and outcome of newly diagnosed rheumatoid arthritis (RA) patients. Magnetic Resonance Imaging (MRI) has become the reference technique in clinical research to detect and quantify inflammatory involvement of the soft tissues (synovitis and tenosynovitis) and bone marrow (osteitis) along with structural damages of the bone (erosions) in hands of patients with RA. Three-point Dixon MRI may be a valuable alternative to the currently recommended seq…

The Role of Ultrasound in Research and Clinical Practice in Psoriatic Arthritis: Highlights From the GRAPPA Ultrasound Workshop.

Ultrasound (US) is a valuable imaging modality that can accurately identify relevant features of psoriatic arthritis (PsA), such as synovitis, tenosynovitis, and enthesitis. The Group for Research and Assessment of Psoriasis and Psoriatic Arthritis (GRAPPA) Ultrasound Committee ran a workshop during the annual GRAPPA meeting that was held in July 2020. The group presented the following 3 topics: (1) the transition from psoriasis to PsA and the role of US; (2) the effect of biomechanical forces on the entheses in health and disease, and insight for PsA pathogenesis; and (3) differentiation of e…

Trigger Finger Treatment.

Trigger finger is a frequent condition. Although tenosynovitis and the alteration of pulley A1 are identified as triggering factors, there is no consensus on the true cause in the literature, and its true etiology remains unknown. The diagnosis is purely clinical most of the time. It depends solely on the existence of finger locking during active bending movement. Trigger finger treatment usually begins with nonsurgical interventions that are instituted for at least 3 months. In patients with initial presentation with flexion deformity or inability to flex the finger, there may be earlier indi…

[Sonography of the wrist and the hand].

Sonographic examination of the hand requires high-frequency linear transducers. As the relevant structures are located very close to the surface, water stand-off pads are mandatory. Owing to the high sensitivity of sonography in the detection of fluid, exudative synovitis, tenosynovitis and ganglia can be easily diagnosed. Sonographic information on muscle atrophy and alterations of the shape and echogenicity of the median nerve in patients with carpal tunnel syndrome may be useful in evaluating the extent of disease. Further indications for the sonographic examination of the hand include susp…

Anatomical Study of Stabilizing Structures of the Extensor Carpi Ulnaris Tendon Around the Wrist.

PURPOSE: The sixth dorsal extensor compartment is a relatively common site of stenosing tenosynovitis in the upper extremity, but the exact location of stenosis is not fully understood. The objective of this study was to investigate the detailed anatomy of structures surrounding the extensor carpi ulnaris (ECU) tendon around the wrist. METHODS: Fifty fresh human cadaveric wrists were used for gross observation and morphology measurements of the sixth dorsal compartment and the ECU subsheath. An additional 13 wrists were used for histological examination. We evaluated the morphology of support…

Atypical X-linked agammaglobulinemia diagnosed in adulthood with arthritis caused by a hypomorphic BTK splice-site variant: a case report and literature review.

X-linked agammaglobulinemia (XLA) is an inborn error of immunity (IEI) caused by loss-of-function variants in the Bruton tyrosine kinase (BTK) gene, classically presenting in early childhood with recurrent bacterial infections. Here, we report an atypical case of XLA diagnosed in adulthood during evaluation of inflammatory arthritis. A 42-year-old man with no history of severe or recurrent infections initially presented with monoarticular inflammation of the right elbow joint and later developed Doppler-positive synovitis and tenosynovitis of the hands, together with a methicillin-sensitive St…
