Use of a multiplanar distracter for the correction of a proximal interphalangeal joint contracture.

Proximal interphalangeal (PIP) joint contractures are common complications in hand injuries and conditions such as Dupuytren's contracture. Conventional treatment such as splinting and serial casting may result in inadequate improvement. Operative release of the contracture may be complicated by neurovascular overstretch with injury to the digital nerves or vascular compromise. Gradual distraction of the contracted joint may prevent this neurovascular injury. The multiplanar distracter was designed for three-dimensional distraction of the mandible. Distraction may be obtained in the X, Y, or Z

Unusual combined fracture dislocation of the wrist and metacarpophalangeal joints: a case report and review of the literature.

Volar perilunate fracture dislocation is an extremely rare carpal injury, but associated with metacarpophalangeal joint dislocation of both the ipsilateral index and middle finger has never been reported. We report one case of a 28-year-old man following a high-energy trauma. After performing closed reduction of the metacarpophalangeal joint injury, open reduction of the wrist injury through volar approach was done, and a K-wire fixation was used to stabilize the scaphoid fracture and lunotriquetral joint. After a 16 months?follow-up period, the wrist regained a full range of motion without sy

Ulnopalmar dislocation of the fifth carpometacarpal joint. A rare injury.

Volar dislocation of the carpometacarpal joint of the little finger is an uncommon injury. It is subdivided in ulnopalmar and radiopalmar dislocations. The injury can easily be missed on standard x-rays. Closed reduction and K-wire fixation has commonly been used in the treatment, although closed reduction and casting has been reported. Only 10 cases of an ulnopalmar dislocation were published previously. We report a case of a simultaneous dislocation of the 4th metacarpophalangeal joint and an ulnopalmar dislocation of the 5th carpometacarpal joint. The combination of these injuries has not b

Acute double flexor tendon ruptures following injection of collagenase clostridium histolyticum (Xiapex) for Dupuytren's contracture.

We report a case of acute (24 h) double flexor tendon rupture of the little finger after a single injection of collagenase clostridium histolyticum into a palmar Dupuytren's contracture cord which caused metacarpophalangeal joint contracture. Tendon surgery was performed 48 h postinjury with primary repair and standard rehabilitation but it resulted in poor active flexion due to adhesions. Previous papers have suggested that a needle inserted into the flexor tendon can be detected prior to the injection of collagenase by asking the patient to actively move the finger, but our test on an awake

Palindromic rheumatism.

A 27-year-old male presented with the complaints of recurring attacks of painful inflammation affecting finger joints of both hands for last one year. There were no constitutional features of weight loss, fever, anaemia, itching or burning sensation over the joints. It was diagnosed to be a case of palindromic rheumatism clinically and treated with hydroxychloroquine. The case is discussed in detail with reference to similar cases reported by others.

Surgical Techniques for Correction of Traumatic Hyperextension Instability of the Proximal Interphalangeal Joint: A Biomechanical Study.

PURPOSE: To compare the biomechanical strengths of 5 surgical techniques for treatment of traumatic hyperextension instability of the proximal interphalangeal (PIP) joint. METHODS: Thirty-six cadaveric fingers were randomly assigned to 6 groups: normal control, volar plate repair, flexor digitorum superficialis tenodesis (FDST), single lateral band transfer (SLBT), double lateral band transfer, and dual split lateral band transfer. For each experimental specimen, the volar plate and accessory collateral ligaments were transected, the PIP joint was hyperextended to 90°, and a PIP joint stabili

Distribution of finger nodes and their association with underlying radiographic features of osteoarthritis.

OBJECTIVE: To determine the distribution of clinically palpable hand interphalangeal (IP) nodes at each finger and thumb joint in a population with nodes, the influence of left or right hand dominance and sex on the development of nodes, and the association between nodes and underlying radiographic features of osteoarthritis (OA). METHODS: We performed a cross-sectional analysis of participants in the Genetics of Osteoarthritis and Lifestyle (GOAL) study who had ≥1 Heberden's nodes or Bouchard's nodes on clinical examination. Frequencies (%) of nodes were described for each IP joint in the ha

Tension Reducing Muscle Stretch Protocol to Obtain Full Distal Tendon Excursion: A New Rehabilitation Protocol after Flexor Tendon Repair.

Background: Flexor tendon rehabilitation protocols minimize repair tension by limiting range of movement to prevent tendon rupture. The resultant muscle contracture inhibits finger extension, increases resistance in tendon gliding distally, and progress to proximal interphalangeal (PIP) joint flexion contracture. This study describes our new rehabilitation protocol, the Tension Reducing Muscle Stretch (TRMS), designed to prevent flexor muscle contracture and obtain full distal tendon excursion. Methods: We reviewed retrospectively 14 fingers in 13 consecutive patients with primary repair of co

Surgical Fixation of Metacarpophalangeal Collateral Ligament Rupture of the Fingers.

BACKGROUND: Collateral ligament injury of the metacarpophalangeal joint of the fingers is underreported in the literature and widely underestimated by the medical community. Here, the authors present results from a large series of patients and review factors influencing success of surgery. METHODS: The authors performed a retrospective study of 46 patients who underwent surgical fixation of the metacarpophalangeal collateral ligament using bone anchor in an acute or chronic setting. The diagnosis was predominantly clinical, based on laxity testing of the joint. The authors collected demograph

Pollicisation: The Myth about Creating a Pseudo-Trapezium.

We have performed 14 pollicisations without creating a "pseudo-trapezium" from the metacarpal head. The entire metacarpal of the intended finger to be pollicised is removed and the proximal phalanx of the finger is inserted into the space left by the base of the excised metacarpal. The results are good to excellent in all 14 cases followed for 2-9 years. The advantages are that the operation is much simpler, thus also much quicker. The new thumb looks more like a thumb (not too long) and is more stable (ball-and-socket joint), with no long term complications often seen with the pseudo-trapeziu

Beekeeper' arthropathy.

An acute inflammatory arthritis of unknown cause has been described in beekeepers in relation to their work with the hives. We present the case of a beekeeper who, after a bee sting, developed arthritis of the interphalangeal joint of the first finger of his left hand. Although the subacute clinical course and the magnetic resonance imaging findings required the differential diagnosis with an infectious process, the rest of the laboratory tests. other imaging studies and the course, together with a history of a similar episode a few years earlier on a finger of the other hand after a bee sting

[Infections of the hand].

Tendon sheath infections and septic arthritis require early irrigation therapy in order to avoid permanent loss of function. In the thumb, infection of the sheath often leads to an acute carpal tunnel syndrome, necessitating open median nerve decompression. Differential diagnoses have to be taken into consideration: tuberculous synovialitis of the tendon sheath or finger joint is not yet extinct. The severely painful herpetic whitlow of the finger tip requires virostatic therapy. Operative measures are also not indicated in the acute calcium deposit, which as a rule resolves spontaneously with…

Mechanics of Metacarpophalangeal Joint Extension.

PURPOSE: It is a common belief that extension of the metacarpophalangeal (MCP) joint of the finger is achieved via the sagittal bands acting as a sling or lasso to attach the extensor tendon to the base of the proximal phalanx. The aim of this study was to test the hypotheses that (1) division of the sagittal bands reduces extension force or torque of the MCP joint, and (2) division of the extensor tendon distal to the sagittal band will not affect the extension force or torque of the MCP joint. METHODS: Ten cadaver limbs were secured to a jig to allow for testing of the extension force of th

Donor Finger Morbidity in Cross-Finger Flap: A Systematic Review and Meta-Analysis.

Background  The morbidity of donor finger in a cross-finger flap has not received as much importance as the outcomes of the flap itself. The sensory, functional, and aesthetic morbidity of donor fingers, reported by various authors, are often contradictory to each other. In this study, objective parameters for the sensory recovery, stiffness, cold intolerance, cosmetic outcome, and other complications in the donor fingers, reported in the previous studies, are systematically evaluated. Methods  This systematic review is reported using Preferred Reporting Items for Systematic Reviews and Meta-a

Free Functional Gracilis Muscle Transfer for Severe Pediatric Volkmann Ischemic Contracture After Traditional Splinting of a Supracondylar Humerus Fracture: A Case Report.

Volkmann ischemic contracture is a severe and largely preventable complication of untreated or delayed compartment syndrome. In children, it is most commonly associated with supracondylar humerus fractures, especially when external compression from casts or splints is not promptly recognized. We report the case of a six-and-a-half-year-old right-handed girl who developed severe left upper-limb dysfunction after a closed supracondylar humerus fracture treated in a rural setting with tight traditional splint immobilization, locally known as "Jbira." No fasciotomy was performed during the acute p

Fingertip Replantation With Palmar Venous Anastomoses in Children.

BACKGROUND: Fingertip amputation in children is difficult to manage using microsurgical replantation techniques and many salvage procedures have failed owing to the nonavailability of suitable veins for anastomosis in the fingertip. This study reviewed our experience of pediatric fingertip replantation involving palmar venous anastomoses and evaluated the clinical outcomes. METHODS: From October 2008 to May 2013, 21 pediatric fingertips that had been completely amputated at or distal to the distal interphalangeal joint of the finger, or at or distal to the interphalangeal joint of the thumb w
