Morphologic aspects of bone healing after third-degree open fractures. An experimental study.

The morphologic aspects of undisturbed and disturbed bone healing following third-degree open fractures is demonstrated by means of experimental investigations using animals. As an experimental model we used a canine thigh fractured by a shot. The data was obtained as part of bioballistic investigations of the effect of highvelocity bullets in living tubular bones. The shot fracture was immobilized by external fixation. Progressive radiological controls, as well as microangiographic, microradiographic and histological findings are used to show bone healing. Bone healing following shot fracture

Application of Machine Learning to Osteoporosis and Osteopenia Screening Using Hand Radiographs.

PURPOSE: Fragility fractures associated with osteoporosis and osteopenia are a common cause of morbidity and mortality. Current methods of diagnosing low bone mineral density require specialized dual x-ray absorptiometry (DXA) scans. Plain hand radiographs may have utility as an alternative screening tool, although optimal diagnostic radiographic parameters are unknown, and measurement is prone to human error. The aim of the present study was to develop and validate an artificial intelligence algorithm to screen for osteoporosis and osteopenia using standard hand radiographs. METHODS: A cohor

Is there a correlation between symptoms and bone scintigraphic findings in patients with complex regional pain syndrome?

BACKGROUND: Complex regional pain syndrome (CRPS) is characterized by pain in combination with sensory, vasomotor, sudomotor, trophic and motor abnormalities. The diagnosis of CRPS is based primarily on clinical criteria and the presence of distinct signs and symptoms. The role of bone scintigraphy in the diagnosis of these patients has been limited by its variable sensitivity. In this study, we aim to look if the presence of specific symptoms or symptom subgroups in patients with clinically diagnosed CRPS correlates with scintigraphic findings in bone scan. MATERIALS AND METHODS: We retrospe

The Effect of Distal Scaphoid Excision and Triquetrum Excision on Radioscapholunate Fusion: A Systematic Review.

PURPOSE: Radioscapholunate (RSL) fusion is a motion-preserving limited wrist fusion treatment option for symptomatic radiocarpal arthritis; however, there is no consensus regarding the need for excision of the distal scaphoid or triquetrum. This study aimed to determine the impact of distal scaphoid excision (DSE) and triquetrum excision (TE) in RSL fusion and compare the outcomes and complications. METHODS: A systematic review of the literature was performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and relevant clinical studies on RSL fusio

Biomechanical analysis of locking plates for fixation of metacarpal shaft fractures: A finite element analysis.

BACKGROUND: There appears to be a paucity of knowledge about the biomechanics of locking plates for the fixation of metacarpal shaft fractures. A thorough understanding of the biomechanics of locking plates is needed to apply them correctly, optimize outcomes, and avoid complications. The purpose of this study is to investigate the biomechanics of the fixation of metacarpal fractures using locking plate-screw constructs with different numbers of screws. HYPOTHESIS: The difference in the number of screws in the locking plate influenced the biomechanical outcome of the metacarpal fracture. MET

Double-Screw Osteosynthesis in an Unstable Scaphoid Fracture Model: A Biomechanical Comparison of Two Screw Configurations.

PURPOSE: Although there is evidence that a single headless compression screw is sufficient for fixation of most scaphoid fractures, double-screw osteosynthesis has been shown to result in higher failure strength and stiffness than a single screw. However, the biomechanical effect of different screw configurations has not been determined. METHODS: A standardized unstable fracture model was produced in 28 cadaveric scaphoids. Specimens were randomly allocated to 1 of 2 fixation groups using 2 internal compression screws positioned in either the sagittal or coronal plane. A specimen-specific 3-d

Radiological anthropometry of the hand in Turner's syndrome.

Metacarpal-phalangeal (M-P) lengths, metacarpal sign, and carpal angle were studied using 142 pairs of hand X-rays from 81 individuals with Turner's syndrome age 6 to 25 years. Left M-P lengths, grouped by bone age, were compared with normal female standards and Z-score pattern profiles calculated for each bone age. Differences between Turners and normals in most M-P lengths increased with age, particularly after puberty. Calculation of inter-individual and intra-individual variability yielded good evidence for a M-P pattern profile typical of Turner's syndrome, with increasing growth deficien

[Effects of ovariectomy on trabecular structures of rat alveolar bone].

To clarify the relationship between estrogen deficiency and tooth loss, we analyzed the trabecular structural changes of mandibular alveolar bone in ovariectomized rats. Two months after ovariectomy, the bone resorptive activity remarkably accelerated and caused high bone turnover. On the other hand, one year after ovariectomy, bone loss and trabecular fragmentation occurred in the alveolar bone, although the accelerated degree of bone resorptive activity was moderate. If elderly women could experience similar changes on their alveolar bone, we must pay more attention to the bone resorption pr

Pediatric Hand and Wrist Fractures.

Hand and wrist fractures are common in the pediatric population. Accurate diagnosis relies on the understanding of the physeal anatomy and carpal ossification. Treatment of these fractures is largely influenced by physeal biology and compliance with treatment. A majority have a favorable outcome with nonoperative treatment. Operative treatment should be considered in patients with clinical deformity, open fractures, and significant fracture displacement. Physeal-friendly surgical approaches and implants should be used to minimize the sequelae of physeal injury.

Partial Hand Transplant: Lessons Learned From Cadaveric Dissection Studies.

As the field of vascularized composite allotransplantation continues to expand, new upper extremity transplant candidates are being considered. We recently evaluated a bilateral amputee who had a mid-forearm amputation and a contralateral metacarpal hand amputation. In the latter limb, a "partial" hand transplant that preserved the majority of the patient's existing hand, including a partially severed thumb with intact thenar muscle function, was proposed. The feasibility of this partial hand transplant was studied in fresh-frozen cadaver limbs. This report details the proposed approach, the c

[Reduction of radiation dosage by using digital luminescence radiography on a hand phantom].

PURPOSE: How much can the radiation dose be reduced for hand radiography by using digital luminescence radiography (DLR)? METHODS AND MATERIALS: A hand phantom (3M) with a cyst, two fractures and an "amputation" was digitally exposed in an anterior-posterior orientation using DLR (ADC-70, Agfa). The tube current time product (mAs) was reduced gradually while keeping the voltage constant. The surface entrance dose was measured by a Dosimax sensor (Wellhöfer). Five investigators evaluated the images for characteristics and critical features, pathological findings, visual resolution and contrast

Giant cell reparative granuloma in the hand: report of three cases and review of the literature.

Occurrence of giant cell reparative granuloma (GCRG) outside the jaw, skull, or facial bones is unusual. Three recent cases of GCRG occurring in the hand are presented. This lesion appears on radiographs as a lucent, expansile defect in bone, similar in appearance to enchondroma, giant cell tumor of bone, aneurysmal bone cyst, or brown tumor of hyperparathyroidism. Histologically, GCRG is characterized by spindle-shaped, fibroblastic stoma with scattered, small, angulated giant cells and areas of osteoid formation around foci of hemorrhage. Establishing the pathologic diagnosis is frequently d

Determination of bone age in children with cartilaginous dysplasia (multiple hereditary osteochondromatosis and Ollier's enchondromatosis).

Cartilaginous dysplasias (multiple hereditary osteochondromatosis [MHO] and Ollier's enchondromatosis [OE]) are common pediatric orthopaedic conditions. Long bone growth deformities commonly develop in children with MHO and OE. The timing of procedures frequently used to treat these deformities is often dependent upon the bone age. It was the purpose of this study to investigate bone ages in a series of 40 children with MHO and OE. There were 6 girls and 9 boys with OE, and 9 girls and 16 boys with MHO. Each child's age, gender, race, and diagnosis were recorded. Hand-wrist radiographs obtaine

[Primary hyperparathyroidism--bone turnover and osteitis fibrosa assessed by x-ray (author's transl)].

1. In primary hyperparathyroidism an increased bone turnover is seen, accompanied by osteitis fibrosa (= fibroosteoclasia, FO) in severe forms of the disease. Both types of bone reaction may be detected by microradioscopy X-rays of the hand, extensive striation of metacarpal cortical bone indicating increased bone turnover and subperiosteal resorption of phalanges pointing to FO. 2. In the present study 65 patients with proven PHPT were evaluated before and 39 after operation. Microradioscopy was combined with biochemical assessment of hyperparathyroidism including alkaline serum phosphatase (
