Overdrive pacing for supraventricular tachycardia: a review of theoretical implications and therapeutic techniques.

Rapid atrial pacing for treatment of supraventricular arrhythmias has been demonstrated to be safe and effective. Virtually any supraventricular tachycardia with the exception of atrial fibrillation, Type II atrial flutter, and probably sinus tachycardia can be treated successfully with pacing techniques. The recognition of the advantages of cardiac pacing over drug therapy or DC cardioversion has resulted in its widespread use, especially after open-heart surgery. Although the response to overdrive pacing may not reliably identify the underlying mechanism of supraventricular tachycardia, the

Demonstration of bidirectional dual A-V nodal pathways in the same patient.

In a patient with documented paroxysmal junctional tachycardia (PJT) electrophysiologic studies were performed using an extrastimulus technique. At an A1-A2 interval of 360 msec, atrial extrastimulus revealed sudden prolongation of an A2-H2 interval from 370 to 540 msec and PJT ensued. This finding was consistent with antegrade dual A-V nodal pathways. On the other hand, at a V1-V2 interval of 540 msec, ventricular estrastimulus showed a jump in ventriculo-atrial (V-A) conduction time with evidence of delay in the A-V node from 285 to 565 msec and a ventricular echo followed. This finding was

Mechanism of atrioventricular conduction: study on an analogue.

A simple analogue of the heart, consisting of neon relaxation oscillators, is presented. The analogue may display several disturbances of the A-V conduction, like normal atrioventricular (A-V) conduction, first-degree heart block, Wenckebach periods, Mobitz II type block, supernormal conduction, complete A-V block, the phenomenon of accrochage in complete A-V block and the absolutely arrhythmic response of the ventricles to a very high atrial rate. The analogue was constructed in the simplest possible way, i.e., using the least possible number of variables. The striking similarities between th

Demonstration of dual atrioventricular nodal pathways utilizing a ventricular extrastimulus in patients with atrioventricular nodal re-entrant paroxysmal supraventricular tachycardia.

In patients with atrioventricular (A-V) nodal re-entrant paroxysmal supraventricular tachycardia (PSVT), atrial extrastimulus technique frequently reveals discontinuous A1-A2, H1-H2 curves suggestive of dual A-V nodal pathways. To further test the hypothesis that these curves in fact reflect dual A-V nodal pathways, a ventricular extrastimulus (VS) was coupled either to A2 at a fixed A1-A2 interval which reliably produced an A-V nodal re-entrant atrial echo (E) with a constant A2-E interval in two patients, or to QRS complex (V) during sustained PSVT with a constant E-E interval in one patient

Congenital abnormalities of the conduction system in two patients with tachyarrhythmias.

Serial sections of the conduction system (CS) were performed in two patients with recurrent tachyarrhythmias. Case 1, a 34-year-old female who had dual atrioventricular (AV) nodal pathways with recurrent paroxysmal supraventricular tachycardia, committed suicide. Autopsy revealed an abnormally formed atrial septum with insertion of eustachian valve on the AV part of the pars membranacea. The intercuspid portion of the pars membranacea was muscular. The AV node was located adjacent to the membranous part of the ventricular septum rather than the central fibrous body. In addition, there was an a

Influence of heart rate and inhibition of autonomic tone on the QT interval.

To evaluate whether heart-rate-induced changes of the QT interval are dependent on autonomic tone, we studied 13 healthy subjects, mean age 67.5 years. The maximal uncorrected QT from leads I, II, V1 and V6 was determined during atrial pacing at 90 beats/min and 130 beats/min before and after i.v. administration of propranolol, 0.1 mg/kg, and atropine, 0.02 mg/kg. Significant reductions (p less than 0.01) of QT were induced by the paced increases in heart rate before drugs (10%), after propranolol (10%) and after the combination of atropine and propranolol (9%). Propranolol caused no significa

Clinical significance of prolonged sinoatrial conduction time.

Prolonged (greater than 152 msec) calculated sinoatrial conduction times (SACT) were found in 24 of 470 patients studied by the atrial extrastimulus technique, ranging from 155 to 220 msec (180+/-4.4; mean+/-SEM). There were 18 males and six females with ages of 29 to 85 (mean 65+/-2.6). Electrocardiographic monitoring revealed significant sinus or atrial dysrhythmias in 19 (79%) patients. Of these 19, 15 had persistent sinus bradycardia and/or sinoatrial block, three had sinus bradyarrhythmia with paroxysmal atrial tachycardia, and one had isolated atrial tachycardia. Additional electrophysio

Second degree block during reciprocal atrioventricular nodal tachycardia.

Of 67 patients with reciprocal atrioventricular (A-V) nodal tachycardia consecutively studied by programmed electrical stimulation of the heart, nine patients showed second degree block toward the ventricle and one patient toward the atrium during tachycardia. In four patients the occurrence of block was critically related to the prematurity of the test stimulus initiating the tachycardia. In three patients block developed following increase in rate of tachycardia. In two patients block could be elicited by introducing premature ventricular stimuli during tachycardia. Our observations indicate

Effectiveness and safety of AV node ablation after His bundle pacing in patients with uncontrolled atrial arrhythmias.

INTRODUCTION: In patients with uncontrolled atrial fibrillation, atrioventricular (AV) node ablation after permanent His bundle pacing (p-HBP) could be a therapeutic option for heart rate (HR) control. We aimed to demonstrate the advantages of AV node ablation with p-HBP, and to describe its effectiveness and safety. METHODS: This descriptive observational study included patients with uncontrolled permanent atrial arrhythmias who were candidates for HR control (January 2019 to July 2020) and underwent p-HBP and AV node ablation. RESULTS: A total of 39 patients were included. The median left

Sick sinus syndrome.

Sinus-node dysfunction is common in the elderly and, in most cases, does not cause any symptoms. Despite the high number of laboratory investigations, most diagnoses of sinus-node dysfunction are made by 12-lead electrocardiography, which shows severe sinus bradycardia, sinus arrest, or sinoatrial block. Continuous electrocardiographic monitoring, exercise testing, and electrophysiologic investigations (including pharmacologic interventions to cause complete autonomic blockade) are sometimes useful in detecting transient or latent sinus-node abnormalities. The term sick sinus syndrome should b

The OPTI-MIND study: a prospective, observational study of pacemaker patients according to pacing modality and primary indications.

AIMS: The OPTI-MIND study aims to collect 2-year clinical outcomes of pacemaker patients in real-world clinical practice, overall and according to patient characteristics and pacemaker settings. METHODS AND RESULTS: The present analysis of the OPTI-MIND study describes the programmed device settings after discharge from the pacemaker implant. The objective was to determine whether these settings fit recent guidelines for device-programmed physiological pacing based on the preservation of atrioventricular synchrony, avoiding unnecessary pacing, ensuring rate increase during exercise or prevent

Atrioventricular dromotropathy: evidence for a distinctive entity in heart failure with prolonged PR interval?

Heart failure (HF) is often accompanied by atrioventricular (AV) conduction disturbance, represented by prolongation of the PR interval on the electrocardiogram. Studies suggest that PR prolongation exists in at least 10% of HF patients, and it seems more prevalent in the presence of prolonged QRS duration. A prolonged PR interval may result in elevated left ventricular (LV) end-diastolic pressure, diastolic mitral regurgitation, and reduced LV pump function. This seems especially the case in patients with heart disease, in whom it is associated with an increased risk for atrial fibrillation,

Symptomatic atrioventricular dual pathway double responses: a role for slow pathway ablation.

Two patients with symptomatic fast/slow pathway double responses were evaluated with electrophysiology studies. Chronic palpitations were resistant or worsened by medical therapy. No reentry tachycardias were induced. A nonreentrant paroxysmal supraventricular tachycardia was documented. Radiofrequency ablation of the slow pathway was safely and successfully performed. Patients remain asymptomatic for 16-18 months. Ablation of the slow pathway for this substrate is a viable option.

Conversion of typical to "atypical" atrioventricular nodal reentrant tachycardia after radiofrequency catheter modification of the atrioventricular junction.

Typical atrioventricular (AV) nodal reentry tachycardia (AVNRT) is characterized by anterograde activation over a slowly conducting pathway and by retrograde activation through a rapidly conducting pathway. Preliminary reports suggest that radiofrequency catheter modification can eliminate typical AVNRT while preserving anterograde conduction. Radiofrequency catheter modification was used to treat 88 patients with typical AVNRT. After baseline electrophysiologic evaluation, the ablation catheter was positioned proximal and superior to the site of maximal His deflection. Radiofrequency energy w
