Early to Mid-term Results of Coracoclavicular Ligament Reconstruction Using the Infinity-Lock Button System in the Management of Traumatic Acromioclavicular Joint Dislocations and Lateral End of Clavicle Fractures.

Background The management of distal clavicle injuries with disruption of the coracoclavicular ligaments (CCLs) is challenging. The aim of this study was to assess the functional results of reconstructing the CCLs with the Infinity-Lock Button System using validated outcome measures, patient satisfaction scores, return to pre-injury activity, and complications. Methods A total of 28 cases of CCL disruption were assessed over a seven-year period, which included 14 lateral end-of-clavicle fractures and 14 acromioclavicular joint (ACJ) dislocations. All patients underwent stabilisation of the CCLs…

Elastic stable intramedullary nailing for displaced pediatric clavicle midshaft fractures: a prospective study of the results and patient satisfaction in 24 children and adolescents aged 10 to 15 years.

BACKGROUND: Whereas displaced clavicle fractures in young children are treated nonoperatively, older children are more likely to suffer persistent pain and misalignments during a longer period of treatment. This study presents the outcomes of elastic stable intramedullary nailing for displaced clavicle fractures in children over the age of 10. METHODS: Prospectively, this study evaluates elastic stable intramedullary nailing for displaced clavicle midshaft fractures in children and adolescents, aged 10 to 15 years, from July 2004 to June 2010. We analyzed all complications, patient pain and l…

Biomechanical evaluation of fixation of the coracoclavicular stand-alone cow-hitch suture reconstruction in comparison to two established techniques for highly unstable distal clavicle fractures (Neer type V).

BACKGROUND: Treatment of displaced distal clavicle fractures with bony avulsion of the coracoclavicular (CC) ligaments often warrants surgical fixation, yet a gold standard surgical technique is to be defined. The purpose of this study was to compare the biomechanical fixation strength of a new fixation technique, the CC stand-alone cow-hitch suture reconstruction, and to compare this technique with a clavicle hook plate and a lateral locking plate with CC suture reconstruction. METHODS: Simulated Neer type V distal clavicle fractures of the clavicle were created in 18 cadaveric shoulders, wh…

Effects of Ultrasound-Guided Superficial Cervical Plexus Block With Clavipectoral Fascial Plane Block Versus Superficial Cervical Plexus Block With Interscalene Brachial Plexus Block in Clavicle Surgery: A Prospective Study.

Background Perioperative pain control in clavicle fixation is challenging due to complex innervation and limitations of conventional anesthesia. Regional techniques provide safer, opioid-sparing alternatives. This study compared the efficacy and safety of superficial cervical plexus block with clavipectoral fascial plane block versus interscalene brachial plexus block in clavicle fixation surgery. Methodology In this prospective randomized study, 60 patients scheduled for open reduction and internal fixation of clavicle fractures were allocated into two groups of 30 each. Group 1 received supe…

Distal Clavicle Autograft Versus Traditional and Congruent Arc Latarjet Procedures: A Comparison of Surface Area and Glenoid Apposition With 3-Dimensional Computed Tomography and 3-Dimensional Magnetic Resonance Imaging.

BACKGROUND: Limited studies have compared graft-glenoid apposition and glenoid augmentation area between the Latarjet procedure and distal clavicle graft in glenohumeral stabilization. Additionally, preoperative planning is typically performed using computed tomography (CT), and few studies have used 3-dimensional (3D) magnetic resonance imaging (MRI) reformations to assess graft dimensions. PURPOSE: The purpose of this study was 2-fold: (1) to compare bony apposition, glenoid augmentation, and graft width among coracoid and distal clavicle bony augmentation techniques and (2) to determine th…

Comparison of radiological and clinical outcomes, complications, and implant removals in anatomically pre-contoured clavicle plates versus reconstruction plates - a propensity score matched retrospective cohort study of 106 patients.

BACKGROUND: Plate fixation is frequently used to treat displaced midshaft clavicular fractures, however the ideal plate choice remains subject to discussion; reconstruction locking compression plates (RLCPs) are cheaper and can be easily contoured, whereas anatomically pre-contoured locking compression plates (ALCPs) are thought to provide better stability and therefore lower rates of mechanical failure. To compare the incidence of mechanical failures, functional and radiological outcomes in patients with midshaft clavicular fractures treated with ALCPs versus RLCPs. METHODS: A propensity sco

The comparison of Nice knots and traditional methods as an auxiliary reduction-fixation technique in pre-contoured locking plate fixation for comminuted Robinson type 2B clavicle fracture: A retrospective study.

Open reduction and pre-contoured locking plate fixation is a popular treatment option for displaced midshaft clavicle fracture. Lag screw and cerclage are 2 main intraoperative techniques to reduce and fix fragments. However, both lag screw and metallic cerclage have disadvantages. The doubled-suture Nice knot has been reported in many areas of orthopedic surgery for its effectiveness. This study aims to compare the outcomes of comminuted mid-shaft clavicle fractures reduced by Nice knots vs traditional techniques (lag screw or/and metallic cerclage) when bridged with pre-contoured locking pla…

Effectiveness of supraclavicular nerve and upper trunk of brachial plexus (SCUT) block versus clavipectoral fascia and supraclavicular nerve block (CPFS) in mid-shaft clavicle fracture surgery - A randomized clinical trial.

BACKGROUND AND AIMS: SCUT blocks the supraclavicular nerve (SCN) and upper brachial plexus trunk, and the clavipectoral fascia block (CPF) blocks the fascia-clavicle nerves. This study compares the SCUT block to the CPFS (CPF block and SCN block), which provides site-specific regional anesthesia (RA) for mid-clavicular surgery. MATERIAL AND METHODS: A prospective, randomized controlled trial with two parallel arms and 1:1 allocation is presented here. 18-60-year-old ASA class 1 or 2 adults scheduled for unilateral mid-shaft clavicular surgery were studied. Group A received SCUT block (4 mL of…

Which Technique is the Optimal Option for the Treatment of Displaced Far-Lateral (Cho Type IIC) Distal Clavicle Fractures? A Comparative Analysis of Coracoclavicular Fixation Versus Anatomical Locking Plate Fixation.

BACKGROUND: As achieving stable fixation in unstable far-lateral distal clavicle fractures is challenging, the optimal surgical treatment remains controversial. The aim of this study was to compare the clinical and radiological results of coracoclavicular fixation (CCF) versus anatomical locking plate fixation (ALPF) in patients with Cho type IIC fractures. METHODS: Thirty-eight patients who underwent surgical treatment for Cho type IIC fracture were included. Open CCF using a suture button device was performed in 25 patients and ALPF was performed in 13 patients. The mean follow-up period wa…

Neurovascular risks of anteroinferior clavicular plating.

Anteroinferior plating has been described for internal fixation of clavicular fractures, citing improved bicortical fixation, less hardware prominence, and safer drill trajectories compared with other plate configurations. This anatomic study defined structures at risk during anteroinferior clavicular plating. Four paired cadaveric specimens (8 clavicles) from ages 75 to 93 years were systematically dissected. Using the screw paths associated with an anteroinferior plate (anteroinferior to superoposterior), the distance from the posterior clavicle cortex to the nearest vital structure was meas…

Identification of a Novel Heterozygous De Novo 7-bp Frameshift Deletion in PBX1 by Whole-Exome Sequencing Causing a Multi-Organ Syndrome Including Bilateral Dysplastic Kidneys and Hypoplastic Clavicles.

INTRODUCTION: Congenital anomalies of the kidney and urinary tract (CAKUT) represent the primary cause of chronic kidney disease in children. Many genes have been attributed to the genesis of this disorder. Recently, haploinsufficiency of PBX1 caused by microdeletions has been shown to result in bilateral renal hypoplasia and other organ malformations. MATERIALS AND METHODS: Here, we report on a 14-year-old male patient with congenital bilateral dysplastic kidneys, cryptorchidism, hypoplastic clavicles, developmental delay, impaired intelligence, and minor dysmorphic features. Presuming a syn…

Age estimation from clavicular epiphyseal union sequencing in a Northwest Indian population of the Chandigarh region.

Medial clavicular epiphysis is the long bone epiphysis to fuse last and therefore, is useful for estimating age in the post-pubertal period. Age estimation was done from clavicles obtained from 343 cadavers (252 males and 91 females) of known age. The data was subjected to statistical analysis to see whether the difference obtained in the various ages was significant or not. Commencement of fusion was seen as early as 18 years of age in both male and female clavicles. No clavicle showed complete fusion until the age of 22 years. Complete fusion of the medial end of the clavicle was seen latest…

Dual mini-fragment plate fixation of midshaft clavicle fractures demonstrates fewer union complications but similar patient-reported outcomes compared to nonoperative management: a cohort study with mean 3.4-year follow-up.

BACKGROUND: Although open reduction and internal fixation is the gold standard treatment for displaced midshaft clavicle fractures, recent studies have advocated for nonoperative management, citing high rates of reoperation associated with operative intervention. However, no studies have compared nonoperative management to open reduction and internal fixation with dual-plate fixation, which may be associated with lower rates of reoperation compared to single-plate fixation. The purpose of this study was to compare the complications and patient-reported outcomes of dual mini-fragment plate fixa…
