Investigations into the cause of vertigo in sudden sensorineural hearing loss.

In this light microscopic study of the temporal bone, an attempt has been made to find a morphologic correlate of vertigo associated with id … al degeneration. Gross morphologic alterations in the vestibular system, such as membrane ruptures, endolymphatic hydrops, etc., were also assessed, but no clear-cut pathology was identified in the vertiginous and nonvertiginous ears. The absence of a light microscopic morphologic correlate for vestibular disturbances associated with ISSHL suggests that the symptoms co …

Exposure to high-altitude hypobaric hypoxic environment induces low-frequency hearing loss in C57BL/6J mice: Mediated by slowing down the postsynaptic electrical signal transmission speed in the cochlear-inferior colliculus auditory signaling pathway.

OBJECTIVE: To reveal the pattern of effects of exposure to high-altitude hypobaric hypoxia environment on the auditory function of C57BL/6J … pothesized that the aforementioned abnormal changes in voltage or resistance may be associated with endolymphatic hydrops, suggesting that endolymphatic hydrops may be one of the potential key links through which the high-altitude hypobaric hypoxia environment affects auditory conduction function.

Disruption of a structural niche for otoconia maintenance may underlie common vestibular disorders.

Many falls and balance disorders in older adults originate in the otolith organs, the gravity sensors of the inner ear. These sensors mainta … naling to the roof epithelium, as a driver of otoconia and scaffold production. In guinea pigs with endolymphatic hydrops, fluid expansion of the organs ruptured the otoconia-trafficking scaffolds as roof and macula separated, followed by a decline in macular otoconial mass. We propose a new disease model for common vertigo and balance disorders in which disruption of …

[Electrocochleography with an extratympanic tympanic membrane electrode : Age-dependent reference values for vertigo diagnostics].

BACKGROUND: Electrocochleography (ECochG) records electrical potentials of the inner ear and is used, among other applications, in the diagnostic work-up of Ménière's disease. For extratympanic recordings, age-dependent reference values are currently lacking, which limits clinical interpretation. OBJECTIVE: This study aims to establish age-stratified reference ranges for the summating potential (SP)/action potential (AP) amplitude and area ratios for noninvasive diagnostics in patients with suspected endolymphatic hydrops. MATERIALS AND METHODS: A total of 47 ears from 26 normal-hearing volu…

Investigation of the ototoxicity of gadoteridol (ProHance) and gadodiamide (Omniscan) in mice.

CONCLUSION: In the mouse, when a tympanic perforation is present, gadoteridol does not seem to cause ototoxicity. Gadodiamide may cause mild ototoxicity other than toxicity to the outer hair cells of the cochlea. OBJECTIVES: Endolymphatic hydrops have been visualized through intra-tympanic injection of gadolinium-based contrast agents (GBCAs) and three-dimensional fluid-attenuated inversion recovery (3-D FLAIR) magnetic resonance imaging. However, reports on the safety of GBCAs are limited. This study aimed to assess ototoxicity of gadoteridol and gadodiamide. METHOD: In a prospective, rando…

Vestibular abnormality in patients with Meniere's disease and migrainous vertigo.

CONCLUSION: Vestibular abnormality was found in 84% of patients with Meniere's disease (MD) and 66% of those with migrainous vertigo (MV), even in the interictal period. Although MV does not have proven pathology like endolymphatic hydrops of MD, MV had high vestibular abnormality, suggesting that comorbid vestibular abnormality can be a cause of vertigo and needs to be pursued. OBJECTIVES: MD and MV are common disorders causing recurrent vertigo. We determined the vestibular abnormality rates using vestibular tests with objective measurements. METHODS: Results of caloric, head-shaking nysta…

Acoustic phase shift: objective evidence for intralabyrinthine pressure disturbance in Menière's disease provided by otoacoustic emissions.

OBJECTIVES: Still today, Menière's disease (MD) can be definitively diagnosed only on post-mortem findings of endolymphatic hydrops. Otoacoustic emission (OAE) phase has been shown to be highly sensitive to intracranial pressure. Preliminary analysis of OAEs in MD patients indicated high sensitivity to slight variations in intracranial pressure. The principal objective of the present study was to confirm this specific sensitivity of OAEs in MD. PATIENTS AND METHODS: In a prospective study of 32 consecutive cases of acute MD seen in consultation or hospital, 20 patients (23 ears) underwent aco…

Effects of gadolinium injected into the middle ear on the stria vascularis.

CONCLUSION: The concentration of gadolinium (Gd) used clinically showed no remarkable effects on the stria vascularis; however, a higher concentration had adverse effects. The concentration of Gd must be borne in mind when injecting Gd into the tympanic cavity. OBJECTIVE: Endolymphatic hydrops has been visualized using high resolution MRI with the intratympanic administration of Gd in patients with Meniere's disease. We attempted to investigate the effects of Gd on the stria vascularis. MATERIALS AND METHODS: Gd hydrate diluted eightfold with saline or non-diluted Gd or saline was injected i…

Assessment of auditory and vestibular functions after vestibular neurectomy for Menière's disease.

Fourteen patients (12 Meniere's disease, one delayed endolymphatic hydrops and one labyrinthitis) suffering from intractable vertigo were treated with retrosigmoid vestibular neurectomy (RSVN) in our institute. Dizziness was controlled completely in all patients. Hearing was preserved in 91.7% of the cases with pure tone threshold deteriorating by more than 10 dB in one case, remained unchanged in 10 cases and improved in one case during long term observation. Substantial decrease of tinnitus was observed in 78.6%. Vestibular compensation was chronologically analyzed with a stabilometer, ENG a…

Acoustic-distortion products: separation of sensory from neural dysfunction in sensorineural hearing loss in human beings and rabbits.

Because distortion-product otoacoustic emissions (DPOAEs) provide a noninvasive measure of outer hair-cell (OHC) activity, they should provi … onditions in rabbit models of several of the human disorders, including noise-induced hearing loss, endolymphatic hydrops, and cochlear neurectomy. Animal studies were performed to assess the proficiency of DPOAEs to track a developing sensorineural deficit as well as to compare patterns of DPOAE dysfunction between clinical and experimental forms of peripheral hearing …

Type II collagen mediated autoimmune middle ear disease: eustachian tube disease, otitis media with effusion and tympanosclerosis.

Intact Eustachian tube and tympanic cavity functions are essential for normal middle ear physiology. Type II collagen is an essential component of ear tissue. Autoimmune response to this type II collagen produces sensorineural hearing loss, vestibular dysfunction, endolymphatic hydrops, otospongiosis-like lesions, Eustachian tube inflammation and Eustachian tube chondritis. In addition, these autoimmune responses occasionally produce otitis media with effusion (not infectious). Tympanosclerosis, which is a frequent sequelae of otitis media with effusions, was also induced by type II collagen i…

A model of viscoelastoplasticity in the cochleo-saccular membranes.

INTRODUCTION: Variations in the distensile behavior of the cochleo-saccular vestibular membranes may contribute to lesion evolution of endolymphatic hydrops in Meniere's disease. Such variation may be mediated through membrane viscoelastoplasticity. This feature may provide insight into the distensile process at work in these membranes. HYPOTHESIS: A precipitated collagen matrix can provide a suitable in vitro model of viscoelastoplasticity in cochleo-saccular vestibular membranes. METHODS: An in vitro extra-cellular matrix of precipitated collagen was evaluated as a model of suspected visco…

Some Remarks on Imaging of the Inner Ear: Options and Limitations.

The temporal bone has a highly complex anatomical structure, in which the sensory organs of the cochlea and the vestibular system are contai … uctures, the confirmation of dehiscence of the semicircular canals and finally, the verification of endolymphatic hydrops in cases of Ménière's disease. However, the term 'high resolution' is very time dependent. Two milestones in this respect have been (in 1991) the 3D visualization of the inner ear by means of maximum-intensity projection (MIP) of a T2-weighted const …

The furosemide test and vestibular status in Meniere's disease.

A total of 62 ears of patients with typical Meniere's disease was examined by the furosemide test to detect endolymphatic hydrops. In 95% of the normal control group, the per cent change in the maximum velocity of the slow phase of caloric nystagmus (MVS) after injection of furosemide was under 10%. Therefore, a positive furosemide test was defined as a change in MVS of more than 10%. Thirty-five (56%) of the 62 ears with typical Meniere's disease showed a positive furosemide test. When the affected ears were divided into two groups according to vestibular symptoms, only 11 (38%) of 29 inactiv…
