Following trauma, two Caucasian males presented with Dupuytren's contracture affecting the distal interphalangeal joint of the little finger and extending onto the dorsum of the digit to insert into the nail bed. Complete excision of the cords and appropriate physiotherapy helped the patients achieve excellent postoperative results.

Family physicians play a pivotal role in providing timely care for common fingertip injuries and minimizing long-term disability after injury. Subungual hematoma is diagnosed clinically and treated with observation or nail trephination. Nail bed lacerations are treated by removal of the nail and repair of the nail bed. Distal interphalangeal joint dislocations typically occur dorsally and result from hyperextension or hyperflexion during sports or because of accidental trauma. Management primarily involves closed reduction and splinting; however, surgical intervention might be necessary for co

The family physician often provides the first and only medical intervention for fingertip injuries. Proper diagnosis and management of fingertip injuries are vital to maintaining proper function of the hand and preventing permanent disability. A subungual hematoma is a painful condition that involves bleeding beneath the nail, usually after trauma. Treatment requires subungual decompression, which is achieved by creating small holes in the nail. A nail bed laceration is treated by removing the nail and suturing the injured nail bed. Closed fractures of the distal phalanx may require reduction

PURPOSE: To describe the use of a pedicled osteoarticular flap harvested from the base of the third metacarpal for the treatment of traumatic defects of the metacarpophalangeal (MCP) joints.
METHODS: From February 2006 to January 2008, we included in the study 15 patients with posttraumatic defects of the MCP joints. The mean age of the patients was 35 years. The injured MCP joints were located in the thumb (n = 6) and index (n = 4), middle (n = 4), and ring fingers (n = 1). Of the 15 patients, 10 presented with acute injuries and 5 with old injuries. At follow-up, we assessed active motion a

BACKGROUND: Central slip and volar plate injuries rarely occur in the same finger, with only two cases previously reported in published literature. In these two patients, both teenagers, two highly distinct protocols were used. Here, we report a third case involving a 51-year-old gentleman who closed a door on his left third finger.
METHODS: Because the fractures were noncomminuted and only minimally displaced, he was treated nonsurgically using a multi-step process of splinting to prevent impairment of either proximal interphalangeal joint extension or flexion, combined with active range of

Spinal cord injury at the C5 and C6 level results in loss of hand function. Electrical stimulation of paralyzed muscles is one approach that has demonstrated significant capacity for restoring grasp and release function. One potential limitation of this approach is that key muscles for stimulation may have lower motor neuron damage, rendering the muscles unexcitable. We have used surgical modification of the biomechanics of the hand to overcome this limitation. Tendon transfer of paralyzed but lower motor neuron intact muscles can compensate for potential function lost owing to muscles with lo

PURPOSE: The purpose of this study was to explore the strength required to extend the small finger using these techniques.
METHODS: Healthy adult participants without current hand injury or pathology were tested for small finger manipulation strength using a calibrated scale as the flexed small finger proximal interphalangeal joint was brought into extension with resistance. Both wrist flexion and quadriga effect were tested first, then quadriga effect alone, then wrist flexion, then without restrictions. This was to prevent fatigue as a confounding factor (weakest to strongest). Measurements

Sensorimotor dysfunction of the fingers and hands hinders the recovery of motor function post-stroke. Generally, hemiplegic patients are unable to properly control the dynamic friction generated between their fingers and objects during hand/finger muscle activity. In addition to sensory information, a sense of agency generated by the temporal synchronization of sensory prediction and sensory feedback is required to control this dynamic friction. In the present study, we utilized a novel rehabilitation device that transmits real-time fingertip contact information to a transducer in a case of st

The effects of different hand motions and positions used during early protected motion rehabilitation on tendon forces are not well understood. The goal of this study was to determine in vivo forces in human flexor digitorum profundus (FDP) and flexor digitorum superficialis (FDS) tendons of the index finger during active unresisted finger flexion and extension. During open carpal tunnel surgery (n = 12), flexor tendon forces were acquired with buckle force transducers, and finger positions were recorded on video while subjects actively flexed and extended the fingers at two different wrist an

BACKGROUND: This prospective study aimed to evaluate the outcomes of the use of dermal templates for lengthy volar soft tissue defects (1.5-4 cm) in the fingers.
METHODS: The volar soft tissue defects of 15 patients (19 fingers) were treated with Lando dermal template coverage between June 2022 and November 2022. We evaluated sensory recovery, scar formation, and overall appearance of the repair site at an average of 13 months (range, 12-17 months) of follow-up.
RESULTS: The defect healed in all cases. We found an average static 2-point discrimination of 7 mm (range 4 to 14 mm). Scar formati

This study is the review of 16 hands in the same number of patients (15 men, 1 female) in whom ring finger flexor digitorum superficialis opponensplasty was performed. The patients were operated on during the period of 6 years. The lack of thumb opposition was a result of traumatic nerve leasions (5 median, 11 median and ulnar). The operation was performed 7.2 months (on average) after the nerve reconstruction. The outcome was assessed by scoring the finger to which the thumb could obtain a pinch grip (Kapandji's test), measuring the gap between thumb and little finger MCP joints, the patient'

PURPOSE: This study aimed to assess the outcome of a modified two-stage flexor tendon reconstruction using silicone tubes as antiadhesion devices while performing simultaneous tendon grafting.
METHODS: From April 2008 to October 2019, 16 patients (21 fingers) with zone II flexor tendon injuries, who sustained failed tendon repair or neglected tendon laceration, were treated by a modified two-stage flexor tendon reconstruction. The first stage of treatment comprised flexor tendon reconstruction with interposition of silicone tubes to minimize fibrosis and adhesion around the tendon graft; the

PURPOSE: Traumatic defects of the condyles of the proximal phalanx (P1) are challenging injuries. Use of osteochondral grafts from the hamate had been described for defects of the base of the middle phalanx. Extending this concept, the purposes of this study were to see whether an osteochondral graft from the base of the little finger metacarpal was anatomically feasible to reconstruct a condyle of P1, and to determine whether the reconstructions performed were clinically successful.
METHODS: We measured the radius of curvature of the base of M5 and the condyles of P1 of the 4 fingers in 15 d

In multiple finger amputations, microsurgical reconstruction should concentrate on the thumb, index, and middle fingers. The patient in the case report presented in this paper initially sustained an amputation of all of the digits on the left hand and an amputation of the right thumb. Both thumbs were amputated at the metacarpophalangeal joint. Initial replantation of the left thumb, index, and middle fingers and of the right thumb was carried out. The right thumb replantation failed and, after healing, the right great toe was transferred to the right hand. Subsequent to this, the patient deve

Combined distal interphalangeal joint (DIP) arthrodesis with proximal interphalangeal joint (PIP) arthroplasty or arthrodesis presents unique challenges. Although less common than isolated surgery for the DIP and PIP joints, with an aging population, combined DIP and PIP procedures are an increasingly encountered occurrence. Anatomical and morphological studies have provided length and width measurement standards for the middle and distal phalanges, allowing for planning to assess the compatibility of strategies. Besides reviewing anatomical studies to provide length and width guidelines for h
