Congruent vs. incongruent tasks in interdisciplinary stroke rehabilitation: a single-case report.

PURPOSE: Stroke survivors may experience challenges in multiple domains (e.g., speech-language, dexterity, mobility) and pursue services from multiple professionals. Clinicians typically provide rehabilitation services in back-to-back sessions (multidisciplinary). Alternatively, two or more clinicians can co-treat a stroke survivor in one session (interdisciplinary). This pilot project examined task congruency in interdisciplinary stroke care. METHOD: A stroke survivor chronically challenged by non-fluent aphasia and right hemiparesis completed spoken-naming and upper-limb tasks simultaneousl

Alternative splicing in addiction.

Addiction is a chronic and relapsing medical condition characterized by the compulsive use of drugs or alcohol despite harmful consequences. While transcriptional regulation has long been recognized for its role in addiction, recent genome-wide analyses have uncovered widespread alternative splicing changes that shift protein isoform diversity in multiple brain reward regions central to addiction. In this review, we discuss emerging research and evidence that alternative splicing is dysregulated in cocaine, alcohol, and opioid use disorders.

Long-term efficacy and adverse effects of outpatient procedural analgesia with nitrous oxide for a child with Epidermolysis Bullosa.

Epidermolysis Bullosa is a dermatologic condition characterized by skin fragility and the formation of painful blisters all over the body. The course of this chronic hereditary disorder involves multiple painful procedures for which adequate analgesia is an ongoing challenge. This case report follows a previously-described pediatric patient with the Dowling-Meara variant of Epidermolysis Bullosa who was treated with at-home nitrous oxide for daily procedural analgesia. We report on the long-term effectiveness of this treatment in addition to any side effects encountered as a result of this tre

Recurrent Inflammatory Myositis as an Extra-Intestinal Manifestation of Dormant Ulcerative Colitis in a Patient on Long-Term Mesalamine.

Ulcerative colitis (UC) is a chronic systemic inflammatory condition primarily involving the large bowel mucosa with relapsing and remitting episodes. It is also associated with multiple extra-intestinal manifestations [EIM], including skeletal muscle involvement which is rare. Review of the literature reported only a few cases of inflammatory myositis in association with UC. We report an unusual presentation of recurrent inflammatory myositis of lower extremities in a 28-year-old male with quiescent UC and on long-term mesalamine therapy.

Chronic obstructive pneumonia caused by a vertebral body osteophyte.

Osteophytes associated with spondylosis have been implicated as a cause of multiple extraspinal manifestations. Symptoms are more likely to occur with the large osteophytes associated with diffuse idiopathic skeletal hyperostosis. In the thoracic region, osteophytes have been reported infrequently as a cause of extraspinal complications. We report a case in which an anterior thoracic vertebral osteophyte was responsible for chronic obstructive pneumonia due to obstruction of the right main stem bronchus. The patient's condition improved considerably after surgical resection of the compressing

[A surviving case of disseminated tuberculosis complicated with severe respiratory failure, pancytopenia, DIC, drug induced fever and pneumothorax in an elderly patient].

We reported a survival case of a 78-year-old female with disseminated tuberculosis complicated with severe respiratory failure, pancytopenia, DIC, drug induced fever and pneumothorax. Atypical symptoms and presence of chronic illness make the early diagnosis of disseminated tuberculosis in the elderly difficult. In this report, we emphasized that both a high awareness of possible clinical diagnoses and the performance of some clinical procedures including bronchoalveolar lavage were useful. Disseminated tuberculosis in the elderly with multiple complications which is rare condition, was discus

Therapeutic stereotactic procedures on the thalamus for motor movement disorders.

The value of functional neurosurgery in the treatment of motor movement disorders is emphasized. The two methods of stereotactic procedures, namely a destructive one with small lesions centered on specific targets, and a non-destructive one with chronically inserted electrodes connected with an also implanted programmable neuropacemaker are described in detail. The results in Parkinsonian tremor, essential tremor, tremor of multiple sclerosis, post-traumatic tremor and in other involuntary movement disorders are reported and demonstrate that stereotactic neurosurgical treatment of these condit

Pulmonary Hypertension: Pharmacological and Non-Pharmacological Therapies.

Pulmonary hypertension (PH) is a severe and chronic disease characterized by increased pulmonary vascular resistance and remodeling, often precipitating right-sided heart dysfunction and death. Although the condition is progressive and incurable, current therapies for the disease focus on multiple different drugs and general supportive therapies to manage symptoms and prolong survival, ranging from medications more specific to pulmonary arterial hypertension (PAH) to exercise training. Moreover, there are multiple studies exploring novel experimental drugs and therapies including unique neuros

Integrative Approach to Somatic (Psychogenic) Cough in a Geriatric Female Patient: A Case Study.

Chronic cough, defined as a cough lasting more than eight weeks, is a common condition that can significantly impair one's quality of life. Chronic cough is often attributed to respiratory, gastrointestinal, or allergic causes; however, a subset of patients experience persistent symptoms without identifiable pathology. In such cases, somatic cough syndrome (formerly known as psychogenic cough) should be considered. This report describes the successful treatment of a chronic refractory cough in an elderly patient using an integrative East-West medical approach. Following assessments by multiple

Physical training in patients with congestive heart failure.

Multiple compensatory mechanisms operate to preserve exercise tolerance in patients with left ventricular failure. Exercise capacity of most patients with chronic heart failure is limited by dyspnea or fatigue, or both. Maximal stress testing with direct assessment of peak O2 uptake is an essential measurement in planning exercise conditioning programs, which are now attracting patients with chronic heart failure. The biochemical and histologic patterns of skeletal muscle changes seen in chronic heart failure patients are consistent with the effects of long-term exercise deconditioning in norm

Prostatitis.

Prostatitis is a poorly defined group of syndromes with multiple causes, some of which are not yet determined. Although treatment of acute prostatitis is well defined and almost uniformly successful, chronic prostatitis, either of bacterial or undetermined etiology, remains a perplexing problem in both diagnosis and treatment. Identification of the etiologic agent of chronic "nonbacterial" prostatitis, if one exists, has not yet been accomplished. Undoubtedly, more than one organism may be responsible for these cases. Treatment is ineffective for a high percentage of patients with chronic pros

Relation of age and self-reported chronic medical condition status with dietary nutrient intake in the US population.

OBJECTIVE: To examine the association of nutrient intake with age and self-reported chronic medical condition status in a large, nationally representative sample. METHODS: We used data from the Continuing Survey of Food Intakes by Individuals, 1989-1991. The analytic sample included subjects aged > or =25 years with 3 days of dietary data, and medical condition information (n=7,207). A positive response to having been informed by a doctor of having diabetes, heart disease, high blood pressure, cancer, osteoporosis, and stroke indicated the presence of chronic medical condition(s) (n=2,368). Care of children with medical complexity in the hospital setting.

Children with medical complexity are a subset of patients with special health care needs whose "health and quality of life depend on integrating health care between a primary care medical home, tertiary care services, and other important loci of care such as transitional care facilities, rehabilitation units, the home, the school, and other community based settings," according to Cohen et al. These children are characterized by (1) substantial health care service needs, (2) one or more severe chronic clinical condition(s), (3) severe functional limitations, and (4) high projected use of health

National guidelines for evaluating pain-Patients' legal right to prioritised health care at multidisciplinary pain clinics in Norway implemented 2009.

Background All nations are posed with the challenge of deciding how to allocate limited health care resources. A Patients' Rights Law from 1999 gives patients in Norway with a serious health condition, for which there is efficacious and cost-effective treatment, a legal right to receive health care from the National Health Care system. Methods Recently national guidelines have been produced for implementing these legal rights within 32 fields of specialist health care. One of these fields deals with serious chronic pain conditions. A task force established by the Directorate of Health, compris

The PROgnostic ModEl for chronic lung disease (PRO-MEL): development and temporal validation.

BACKGROUND: Patients with chronic lung diseases (CLDs), defined as progressive and life-limiting respiratory conditions, experience a heavy symptom burden as the conditions become more advanced, but palliative referral rates are low and late. Prognostic tools can help clinicians identify CLD patients at high risk of deterioration for needs assessments and referral to palliative care. As current prognostic tools may not generalize well across all CLD conditions, we aim to develop and validate a general model to predict one-year mortality in patients presenting with any CLD. METHODS: A retrospe

The Prevalence of Bullying Among Pediatric Orthopaedic Patients.

BACKGROUND: Bullying is destructive and pervasive. Although the literature suggests children with chronic health conditions are at higher risk of being bullied, there is minimal research regarding the prevalence of bullying among children with orthopaedic conditions. Our study aimed to assess the prevalence of bullying among pediatric orthopaedic outpatients and evaluate the association of orthopaedic conditions and use of orthopaedic devices with perceptions of bullying. METHODS: Patients in outpatient pediatric orthopaedic clinics, ages 10 to 17 and their parents were surveyed using the Chi

Ophthalmologic management in KID syndrome: Long-term clinical experience.

Keratitis-ichthyosis-deafness (KID) syndrome is a rare disease caused by mutations in the GJB2 gene. This gene encodes the protein connexin 26, which is essential for gap junctions in the epidermis, inner ear and corneal epithelium. Clinically, it is characterised by dermal hyperkeratotic lesions, sensorineural deafness and chronic keratitis that is difficult to manage. We describe two siblings diagnosed with KID syndrome who were followed up in our department for more than 15 years. Both developed numerous ocular complications associated with chronic keratitis and limbar insufficiency that re

A Case of Pancreatic Duct Leak Presenting as Lower Extremity Pain and Edema.

Pancreatic pseudocyst from pancreatic duct leak is one of the common complications of both acute and chronic pancreatitis. The presentation of such leaks can range from patients being completely asymptomatic to septic shock. Extra-abdominal collections of fluid due to pancreatic duct leaks are very rare. We describe a diagnostically challenging patient with acute chronic pancreatitis presenting with lower extremity swelling and pain, who was found to have a pancreatic fluid leak into the right iliopsoas, right gluteal, and thigh muscle compartment. Despite endoscopic stenting of the pancreatic
