Transverse testicular ectopia: a case report on preoperative misdiagnosis in two siblings.

BACKGROUND: Transverse testicular ectopia (TTE) is a rare congenital abnormality, with only approximately 35% of TTE cases being correctly diagnosed preoperatively. TTE is generally not considered a genetic or inherited disease, although it may occur in genetically related individuals. Here, we report two siblings who had a preoperative misdiagnosis of TTE and share our experience with their diagnosis and treatment to enhance the clinical literature on the appropriate management of TTE cases. CASE DESCRIPTION: In June 2021, a 1-year-old boy received preoperative diagnoses of right cryptorchid…

Genetic mutations in lymphocytic variant of hypereosinophilic syndrome: study of five siblings.

INTRODUCTION: Lymphocytic variant hypereosinophilic syndrome (L-HES) is a rare subtype of hypereosinophilic syndrome driven by aberrant T-cell clones that promote eosinophilia through interleukin-5 (IL-5) overproduction. While clonal T-cell receptor (TCR) rearrangements are a hallmark, the underlying genetic landscape remains poorly defined. METHODS: We report a familial case series involving five siblings, three symptomatic and two asymptomatic, with comprehensive clinical, immunophenotypic, and genomic evaluations. Whole-exome sequencing (WES) was performed to identify rare germline variant…

Hereditary Folate Malabsorption Presenting With Pancytopenia in Two Siblings: A Case Report.

Hereditary folate malabsorption (HFM) is a rare autosomal recessive disorder caused by mutations in the SLC46A1 gene, leading to impaired intestinal and central nervous system folate transport. We present two male siblings with clinical features suggestive of HFM. The first infant exhibited pancytopenia, diarrhea, hypogammaglobulinemia, and neurological regression due to delayed diagnosis and treatment discontinuation, resulting in a fatal outcome. The second sibling was diagnosed early based on clinical suspicion and family history and showed favorable progress after timely parenteral folinic…

TECPR2-related hereditary sensory and autonomic neuropathy in two siblings from Palestine.

Due to the majority of currently available genome data deriving from individuals of European ancestry, the clinical interpretation of genomic variants in individuals from diverse ethnic backgrounds remains a major diagnostic challenge. Here, we investigated the genetic cause of a complex neurodevelopmental phenotype in two Palestinian siblings. Whole exome sequencing identified a homozygous missense TECPR2 variant (Chr14(GRCh38):g.102425085G>A; NM_014844.5:c.745G>A, p.(Gly249Arg)) absent in gnomAD, segregating appropriately with the inheritance pattern in the family. Variant assessment with in…

Challenges of siblings with tuberous sclerosis showing various manifestations and severe complications.

Tuberous Sclerosis Complex (TSC) is a rare genetic disorder that primarily affects the central nervous system and various body organs. This case series describes the case history of 2 siblings from the same parents who were diagnosed with TSC. Case 1 is a 13-year-old girl with bilateral renal AML (angiomyolipoma), multiple fat nodules in the liver, and subependymal nodules with tubers revealed in the brain magnetic resonance imaging (MRI). Case 2 is her brother, a 6-year-old boy, who presented with manifestations of subependymal giant cell astrocytoma (SEGA) and renal AML. TSC must be managed …

Primary Ciliary Dyskinesia with Identical Genotype but Distinct Phenotypes in Two Siblings.

In this study, we report two cases of siblings diagnosed with primary ciliary dyskinesia (PCD) sharing an identical genotype yet exhibiting distinct phenotypes. A 13-year-old girl with acute pneumonia was admitted to our hospital. Chest and sinus radiography revealed situs inversus and bilateral maxillary sinusitis. Chest computed tomography revealed bronchiectasis. Her 6-year-old brother with acute bronchitis was admitted and was diagnosed with bronchial asthma due to recurrent wheezing. Unlike his sister, he did not have situs inversus. Both patients had a chronic wet cough and were diagnose…

Congenital diaphragmatic hernia in siblings with PIGA-related congenital disorder of glycosylation.

There are over 150 proteins involved in glycosylphosphatidylinositol (GPI)-anchored protein biosynthesis, a class within the larger category of congenital disorders of glycosylation (CDG). Pathogenic variants identified in phosphatidylinositol glycan class A protein (PIGA) are associated with X-linked PIGA-CDG, a GPI-anchor defect. The disease has primarily been characterized by hypotonia, epilepsy, and global developmental delay; however, only 89 known cases are reported, so the phenotypic spectrum has likely not yet been fully delineated. Congenital diaphragmatic hernia (CDH) has been report

Pathways driving tendinopathy and enthesitis: siblings or distant cousins in musculoskeletal medicine?

Tendinopathy and enthesitis share clinical, anatomical, and molecular parallels. However, their relationship is complex, presenting challenges in diagnosis and treatment. The biomechanics underlying these pathologies, together with relative immune and stromal contributions to pathology, are characterised by crucial comparative elements. However, methodologies used to study enthesitis and tendinopathy have been divergent, which could account for discrepancies in how these conditions are perceived and treated. In this Review, we summarise key clinical parallels between these two common presentat

Familial testicular torsion in siblings of different age groups: A case report.

The acute scrotum (AS) is one of the urologic emergencies, where testicular torsion (TT) represents 20-30% of it. In the peri-puberty period, TT is not uncommon, yet there are very few reports of familial testicular torsion worldwide. This case report highlights a man who experienced TT and had a family history of TT. TT should be considered one of the leading causes of AS. We avoid any delay in the diagnosis of TT, where the signs, symptoms, family history of TT, physical examination, and color Doppler ultrasound (CDU) are helpful in earlier diagnosis and management to salvage the twisted tes

Yellow Fever Vaccine-Associated Viscerotropic Disease among Siblings, São Paulo State, Brazil.

We describe 5 cases of yellow fever vaccine-associated viscerotropic disease (YEL-AVD) in 2 familial clusters during the 2017-2018 yellow fever (YF) vaccination campaign in São Paulo state, Brazil. The first case was that of a 40-year-old white man who died of icterohemorrhagic syndrome, which was confirmed to be YEL-AVD by using real-time reverse transcription PCR to detect 17DD YF vaccine in the liver. Ten years previously, his brother died of a clinically similar disease without a confirmed diagnosis 9 days after YF vaccination. The second cluster included 3 of 9 siblings in whom hepatitis …

Incestuous childhood sexual abuse between siblings resulting in pregnancy without sexual intercourse.

Victims of incest are usually women and children who are mostly girls. The abuse between siblings may remain hidden because of the family's fear of social stigma, though it may result in pregnancy. This article presents a case of incestuous childhood sexual abuse between siblings resulting in pregnancy without intercourse, an event rarely reported in the literature. An 11-year-old girl was brought to the hospital because of a stomachache and was detected pregnancy from examinations. Forensic medicine experts determined that the girl's hymen was still intact and non-elastic in the genital exami…

Cockayne syndrome without UV-sensitivity in Vietnamese siblings with novel ERCC8 variants.

Cockayne syndrome (CS) is a rare progeroid disorder characterized by growth failure, microcephaly, photosensitivity, and premature aging, mainly arising from biallelic ERCC8 (CS-A) or ERCC6 (CS-B) variants. In this study we describe siblings suffering from classical Cockayne syndrome but without photosensitivity, which delayed a clinical diagnosis for 16 years. By whole-exome sequencing we identified the two novel compound heterozygous ERCC8 variants c.370_371del (p.L124Efs*15) and c.484G>C (p.G162R). The causality of the ERCC8 variants, of which one results in a frameshift and the other affec…

Severity of Psychologic Stressors Reflects Course of Crohn's Disease in Two Siblings.

A significant rise in Crohn's disease (CD) cases amongst first-degree relatives strongly suggests that inflammatory bowel disease (IBD) has a genetic component. Adherence to medical management is at the forefront for preventing disease relapse. However, given the role that stress places on the immune system, it is imperative to implore an individual's psychologic stressors to prevent future disease complications. This case of CD in two siblings, male and female, highlights the unique stress experienced by each patient at the time of symptom onset, the severity of their inflammatory symptoms, a…

Prenatally diagnosed congenital pyloric atresia in consecutive three siblings: a case report.

BACKGROUND: Congenital pyloric atresia (CPA) is a rare gastrointestinal anomaly frequently associated with epidermolysis bullosa (EB). Although the complications of familial isolated CPA are minor, delays in diagnosis can increase the chances of morbidity. CASE PRESENTATION: Three female infants born to a Japanese mother presented with CPA at birth. There was no consanguinity between the parents, and the spacing between pregnancies was 2 years in each case. All 3 siblings had a prenatal diagnosis of CPA owing to polyhydramnios and a dilated stomach, without dilatation of the rest of the gastr…
