A syndrome going with skin-bone signs and precancerous gastrointestinal polyposis is presented. Interest of our case is given by the extension of polyposis onto the whole digestive canal, early appearances of abdominal signs accompanying polyposis, and as a new symptom polycystic lung and extended superficial naevus lipomatosus.

An elderly man with non-familial gastrointestinal polyposis, malabsorption and progressive hypoproteinemia is reported. Associated alopecia, cutaneous hyperpigmentation and nail dystrophy with loss of nails were consistent with the diagnosis of Cronkhite-Canada syndrome. Hypothyroidism was present in this patient and the rare association of these two conditions is discussed.

The case of a 21-year-old woman presenting with acute gastrointestinal haemorrhage is reported. By means of Doppler-endoscopy, a solitary minute polyp located in the proximal ascending colon, which revealed arterial pulsation, was detected, and removed with the aid of forceps. The histological work-up revealed thick-walled, in part dilated, vessels located in the lamina propria and in the submucosa. Changes have taken place in the methods used for surgery of rectal carcinoma. The primary tumour is removed and the regional lymphatic drainage area left behind, when limited techniques are chosen, that is in cases in which the risk of lymph node metastasis is not significant. A blinded, prospective study was conducted to determine the sensitivity of double-contrast barium enema examination (DCBE) in detecting left-colon polyps. Metal clips were endoscopically placed on normal mucosa either adjacent to polyps (in 65 study subjects with 92 polyps) or not adjacent to polyps (in 50 control subjects). In an endoscopic screening study of rectosigmoidal polyps in a defined normal population aged 50-59 years, polyps 5 mm or larger in diameter were removed by diathermic snare resection for histological examination. Histological examination was possible in 50 of 55 polyps removed during colonoscopy from 27 men and 17 women. Five children presenting with chronic and intermittent rectal bleeding were diagnosed as having colorectal polyps by fibreoptic colonoscopy performed under sedation. Three of the children had had barium-enema films reported on as normal. Eight polyps were seen, of which six were proximal to the sigmoid colon. All were removed endoscopically (one by proctoscopy, one by snare-intussusception) without complication. Colonoscopic polypectomy is a safe and efficient procedure in children, and colonoscopy may be regarded as first-line management in those with rectal bleeding.

An unusual case of metastasis to two lymph nodes from an adenocarcinoma arising in an adenomatous polyp of the sigmoid colon is presented. Invasion of the stalk could not be detected, but penetration of the muscularis mucosae was observed. Immunoperoxidase staining was used to detect an antigen in the invasive and metastatic components of the tumor that reacted with an antiserum to a tumor associated antigen. A focus of intraepithelial carcinoma was also detected by this method. The relevance of these findings to the adenoma-carcinoma sequence is discussed.

Cold snare polypectomy (CSP) is widely considered a safe treatment for colorectal polyps. We report a rare case of intraperitoneal hemorrhage followed by delayed perforation after CSP in a 78-year-old man undergoing hemodialysis. CSP was performed for an 8-mm slightly elevated lesion in the ascending colon, requiring forced CSP to complete resection.
