Minimum ten-year results of a porous acetabular component for Crowe I to III hip dysplasia using an elevated hip center.

We conducted a retrospective study of the placement of porous-coated acetabular components using screws at more than 20 mm above the teardrop without structural bone graft for dysplastic hips to determine long-term outcome. Thirty hips (29 patients) were monitored for a mean of 15.2 years (range, 10.4-18.3 years) after surgery. Compared with 12 contralateral normal hips, the distance of the hip center from the teardrop was significantly high (26.8 +/- 4.8 and 13.4 +/- 2.7 mm P < .001); however, it was not laterally different (31. 5 +/- 5.1 and 31.7 +/- 5.0 mm). No acetabular components showed

Effect of Er:YAG Pulsed Laser-Deposited Hydroxyapatite Film on Titanium Implants on M2 Macrophage Polarization In Vitro and Osteogenesis In Vivo.

In a previous study, we successfully coated hydroxyapatite (HAp) onto titanium (Ti) plates using the erbium-doped yttrium aluminum garnet pulsed-laser deposition (Er:YAG-PLD) method. In this study, we performed further experiments to validate the in vitro osteogenic properties, macrophage polarization, and in vivo osseointegration activity of HAp-coated Ti (HAp-Ti) plates and screws. Briefly, we coated a HAp film onto the surfaces of Ti plates and screws via Er:YAG-PLD. The surface morphological, elemental, and crystallographic analyses confirmed the successful surface coating. The macrophage

Pneumomediastinum, subcutaneous emphysema, and tracheal tear in the early postoperative period of spinal surgery in a paraplegic achondroplastic dwarf.

Achondroplasia was first described in 1878 and is the most common form of human skeletal dysplasia. Spinal manifestations include thoracolumbar kyphosis, foramen magnum, and spinal stenosis. Progressive kyphosis can result in spinal cord compression and paraplegia due to the reduced size of spinal canal. The deficits are typically progressive, presenting as an insidious onset of paresthesia, followed by the inability to walk and then by urinary incontinence. Paraplegia can be the result of direct pressure on the cord by bone or the injury to the anterior spinal vessels by a protruding bone. Su

Comparative outcome analysis of internal screw fixation and Kirschner wire fixation in the treatment of scaphoid nonunion.

In cases with difficult scaphoid screw placement due to small, fragile bone fragments, or transplants and insufficient space, the internal Kirschner wire fixation is a fallback option; however, controversy remains regarding its stability, safety, and outcome. Between 2001 and 2011, 95 patients were treated in our center (n = 80 with cannulated compression screws and n = 15 with Kirschner wires), and retrospectively analyzed. The outcome measurements included the analysis of patient data, union rate and analysis of functional measures, and quality of life. Bony reconstructions were performed wi

New concepts for bone fracture treatment and the Locking Compression Plate.

The operative treatment of bone fractures using plates and screws is a standard successful technique. Internal fixation with plates and screws leads to additional trauma and disturbance of the bone blood supply, which increases the risk of delayed union and infection. However, problems also are encountered in the fixation of osteoporotic bone. The locked internal fixator technique is an approach to optimize internal fixation. It aims at flexible elastic fixation to imitate spontaneous healing, including induction of callus formation. The technology supports what is currently called "minimally

Reduction and stabilisation of severe spondylolisthesis. A report of three cases.

A modification of a previously reported one-stage technique for reduction and stabilisation of severe spondylolisthesis using a posterior route is described. Reduction is obtained by inserting Harrington rods to lift L5 vertically out of the pelvis and two double-threaded screws to pull it backwards. After reduction the rods are taken away and stabilisation achieved by means of screws and a sacral bar. With this modified technique lumbar vertebrae above L5 are never immobilised, compared with the previous method where the retention of the Harrington rods resulted in more lumbar vertebrae being

Internal fixation with biodegradable high purity magnesium screws in the treatment of ankle fracture.

BACKGROUND: Metal plates and screws are widely used as internal fixations in the treatment of ankle fracture. However, there are many disadvantages such as "stress shielding" effect and the need for secondary surgical removal, which potentially affected the blood supply around the fracture site. In recent years, many studies confirmed that the biodegradable high purity magnesium (Mg) screws exhibited sufficient mechanical strength with adequate degradation rate for effective bone healing, avoiding the need for implant removal operations. METHOD: We conducted a prospective study on patients wi

Clinical and computed tomographic evaluation of safety and efficacy of facet screw fixation in the subaxial cervical spine.

DESIGN: A retrospective study comparing screw positioning and associated complications between 2 different facet screw placement methods. OBJECTIVE: To review the anatomic location, clinical safety, efficacy, and limitations of 2 facet screw placement techniques. BACKGROUND: Facet screw fixation in the subaxial cervical spine penetrates 4 cortical layers and affords better stability than lateral mass screws. Takayasu and colleagues recommended placing screws in the sagittal plane. We modified the trajectory to direct the screws laterally from the sagittal plane to place the root at less risk

Self-drilling screws for the prevention of dental and skeletal injuries during open reduction and internal fixation of pediatric mandibular fractures.

Facial trauma in pediatric population predisposes the child to injury of both the developing skeleton and dentition. This article aims to highlight the experience of the authors through a case report, in using self-drilling screws for fixation of mandibular fractures in pediatric age group. The use of self-drilling screws minimizes the complications such as thermal and/or mechanical damage to the developing dentition and the bone. They also provide significant advantages including ease of availability and technique, superior anchorage with primary stability, and minimizing or avoiding permanen

Recurrent locking of knee joint caused by intraarticular migration of bioabsorbable tibial interference screw after arthroscopic ACL reconstruction.

Two cases are reported in which, after ACL reconstruction with autologous hamstring grafts, tibial polylactide interference screws migrated into the knee joint. Clinically, both patients presented with recurrent locking of the joint. In one case, a broken 15 mm-long tip of the screw was found intra-articularly. In the other case, the whole screw had migrated into the joint cavity. The degradation process of polylactic acid, operative technique and bone quality are discussed as possible reasons for these complications.

Spine-specific robots may offer superior screw placement accuracy in spinal surgery procedures. A systematic review and network and proportional meta-analysis.

BACKGROUND: Owing to the common but reductive practice of aggregating heterogeneous robotic systems without accounting for navigation integration or technological iteration, existing classifications hinder the comparability and generalizability of screw placement accuracy assessments. To address this limitation, the present study proposes a four-type framework comprising Spine-Specific Robot A (SSR-A) and B (SSR-B), Orthopedic-Universal Robot A (OUR-A) and B (OUR-B), where A and B respectively denote the absence and presence of integrated navigation, and systematically assesses the relative pe

How to Avoid and Handle Problems in the Placement of Cement-Augmented Fenestrated Percutaneous Pedicle Screws?

BACKGROUND AND OBJECTIVES: Pedicle screws with a central cannula and fenestrations allow cement augmentation, providing lower risk for screw loosening and pullout, especially in these patients with poor bone quality. This study aims to offer suggestions for resolving issues and reducing complications associated with the use of cement-augmented fenestrated pedicle screws. METHODS: A retrospective study was conducted across multiple centers on patients who received fenestrated pedicle screws with cement augmentation (CAFPS). Using 2-dimensional fluoroscopy guidance, we placed over 800 screws in

Survivorship analysis of pedicle spinal instrumentation.

Between 1985 and 1989, the senior author performed 120 consecutive pedicle instrumentation cases--78 Steffee (VSP) procedures and 42 procedures using Cotrel-Dubousset instrumentation. Posterolateral or posterior fusions using autogenous iliac bone graft were performed across the instrumented vertebrae in all cases. Survivorship analysis was used to calculate a predicted cumulative rate of success for this series of patients over 10 years postoperative follow-up. The criteria of failure of pedicular instrumentation or "death" of an implant were defined as 1) screw bending, 2) screw breakage, 3)

A biomechanical analysis of intrascaphoid compression using the Herbert scaphoid screw system. An in vitro cadaveric study.

The purpose of this study was to evaluate the compressive capabilities of the Herbert scaphoid screw system. A transverse osteotomy was performed at the waist of eight scaphoids removing a 3 mm wafer of bone. A custom-designed load washer was inserted within this defect to measure intrascaphoid compression during Huene guide application, during screw insertion, after screw insertion and after guide release. The intrascaphoid compression was noted to peak during screw insertion; however it dramatically decreased without any clinical subjective evidence of decreased insertional torque by the sur

Additional Surgical Method Aimed to Increase Distractive Force during Occipitocervical Stabilization : Technical Note.

OBJECTIVE: Craniovertebral junctional anomalies constitute a technical challenge. Surgical opening of atlantoaxial joint region is a complex procedure especially in patients with nuchal deformity like basilar invagination. This region has actually very complicated anatomical and functional characteristics, including multiple joints providing extension, flexion, and wide rotation. In fact, it is also a bottleneck region where bones, neural structures, and blood vessels are located. Stabilization surgery regarding this region should consider the fact that the area exposes excessive and life-long
