[Post-traumatic fracture and migration in the pulmonary artery of the catheter of a totally implantable venous access device. Unusual complication].

The case of a 59 year-old patient, who sustained a post-traumatic fracture of the silastic catheter of his totally implantable venous access device that migrated in the right pulmonary artery, is reported. The venous device was placed six months earlier for the treatment of metastatic spread of a primary unknown adenocarcinoma. The venous device was placed on the left side in consideration of a recent right supraclavicular node biopsy. The catheter was introduced through the left internal jugular vein and its peripheral end was positioned subcutaneously across the clavicle to be connected to t

[Dislocation into the cardial lumen of a PTFE prosthesis used in the treatment of voluminous hiatal sliding hernia, A case report].

The authors report a case in which a PTFE prosthesis, used to treat a voluminous hiatal sliding hernia, dislocated inside the cardial lumen. Within two months the prosthesis had become decubitant and penetrated the cardial lumen, causing dysphagia, inappetence and weight loss. The use of radiological and tomographic imaging failed to provide a clear diagnosis; endoscopy alone was conclusive. Surgery was required to remove the prosthesis by resecting the distal esophagus and reconstructing the continuity of the digestive tract using terminolateral esophagogastric anastomosis (front wall of the

Wear particle diffusion and tissue differentiation in TKA implant fibrous interfaces.

In the context of mechanical loosening, we studied the hypothesis that wear-particle migration in the fibrous membrane under tibial plateaus after total knee arthroplasty can be explained by the pumping effects of the interstitial fluid in the tissue. Further, as a secondary objective we investigated the possibility that interface-tissue differentiation is influenced by interstitial fluid flow and strain, as mechanical effects of interface motions. For comparative reasons, we analyzed a previously published simplified two-dimensional finite-element model, this time assuming biphasic tissue pro

Mesh plug migration into scrotum: a new complication of hernia repair.

Hernia repair is one of the most frequent operative procedures performed throughout the world. The technique has continued to evolve and we now are performing these repairs utilizing mesh as a patch and also as a plug. The mesh plug concept has been advocated by Rutkow and others. With this change in technique, we have seen a new complication of hernia repair - the migration of the mesh plug from the original hernia repair site into the scrotum. It presented as a large tender mass in the scrotum of a 45-year-old male who had had previous recurrent surgery. In addition, he again had a recurrent

Migration of the Duraloc cup at two years.

We carried out 71 primary total hip arthroplasties using porous-coated, hemispherical press-fit Duraloc '100 Series' cups in 68 consecutive patients; 61 were combined with the cementless Spotorno stem and ten with the cemented Lubinus SP II stem. Under-reaming of 2 mm achieved a press-fit. Of the 71 hips, 69 (97.1%) were followed up after a mean of 2.4 years. Migration analysis was performed by the Ein Bild Rontgen Analyse method, with an accuracy of 1 mm. The mean total migration after 24 months was 1.13 mm. Using the definition of loosening as a total migration of 1 mm, it follows that 30 ou

Paradoxical embolism of a shotgun pellet.

Paradoxical embolism of a projectile from the venous to arterial system is a rare occurrence, which can cause diagnostic confusion. We present a case of venous embolism of a shotgun pellet from the left upper extremity to the noncoronary sinus of the aortic valve across a secundum-type atrial septal defect. Prevention of distal embolism of the pellet was presumably a result of its containment by flow vortices created within the sinuses of Valsalva.

[A case of retained surgical sponge penetrated into the sigmoid colon].

A thread of surgical sponge was found by colonoscopy at the sigmoid colon wall in the tip of a granuloma. The patient was a 50-year-old woman who complained of diarrhea and had a history of Cesarean section and complete hysterectomy. A radiopaque thread that used to be part of the surgical sponge was demonstrated with plain pelvic X-ray films. Abdominal CT study revealed the gossypibioma including a radiopaque thread in front of the sacrum. The patient was admitted for endoscopic extraction. Upon endoscopic observation, no thread was found and the fistula had also disappeared. The surgical spo

An unusual cause of upper gastrointestinal haemorrhage--perforation of a vena cava filter into the duodenum.

Penetration of vena cava inferior filter spikes through the vessel wall is a frequent but only rarely symptomatic complication of these devices. We report a case of upper gastrointestinal haemorrhage 11 year after implantation of a Mobin-Uddin filter. At gastroduodenoscopy a filter spike protruding from the duodenal mucosa was identified as the cause of the haemorrhage. After removal of the filter the patient made an uneventful recovery. Indications for vena cava filter placement are discussed in the light of their early and late complications. Surgical intervention is strongly recommended for

[Percutaneous removal of displaced metal stents in TIPS].

PURPOSE: Basing on our experience with three dislocated or displaced stents (one Wallstent, two Plamaz stents) during or after TIPS-procedure, we describe our technique to reposition or remove the stents. METHOD: The stents were moved back into the inferior vena cava by means of a balloon catheter, an alligator forceps, or an Amplatz gooseneck snare. They were secured against further dislocation by a central guidewire. After insertion of a balloon catheter into each dislocated stent, they were coaxially grasped with an Amplatz gooseneck snare. Stent diameter was reduced by forceful closure of

[A case of surgical retrieve of migrated microcatheter].

This is a report of an accidental breakaway of a microcatheter during endovascular embolization for cerebral arteriovenous malformation occurring in a 48-year-old female. She had a sudden onset of cerebral herniated signs caused by a large hemorrhage in the region of the right parietal lobe. An angiogram showed a large arteriovenous malformation in the same region. 48 days after emergency evacuation of the hematoma and clipping of a feeding artery, we performed endovascular embolization using a microcatheter as assistant therapy for total removal of the nidus. However, accidentally, the microc

The intra-articular migration of the broken wire: a rare complication of circumferential wiring in patellar fractures.

Patellar fractures are frequently encountered in emergency situations. With proper treatment, the outcome is usually favorable. This study focuses on a rare complication of circumferential wiring in a case of patellar fracture treated by open reduction. A technical error which may have caused this complication is analyzed.

Thoracic complications of ventriculoperitoneal shunts: case report and review of the literature.

Thoracic complications of ventriculoperitoneal shunts are rare, but potentially serious. The authors report a case of a drainage of cerebrospinal fluid into the tracheobronchial tree through a peritoneal shunt catheter that migrated into the chest. After injection of contrast material into the shunt, a plain radiograph of the chest revealed a bronchogram. The symptoms resolved after a revision of the shunt. Published case reports of this and other thoracic complications of ventriculoperitoneal shunts are comprehensively reviewed. A classification of such complications into three types is propo

A rare case of IUD tubal migration. Case report.

The Authors describe a rare case of IUD tubal migration. The uterine device had been inserted twelve years before. A hysteroscopical removal of the IUD was tried unsuccessfully, because it was impossible to locate it either in the uterine cavity or at the fallopian tube orifice level. The patient underwent a minilaparotomy and the IUD was found out at the peritubal level. An ecographic control of all intrauterine devices insertions is recommended.

Foley feeding catheter migration into the small bowel.

Percutaneous endoscopic gastrostomy provides a non-surgical alternative to enteral feeding. The tube may deteriorate or be expelled accidentally from the stomach and require replacement. We report a patient in whom the replacement Foley catheter migrated into the jejunum, resulting in impaction of the hub into the abdominal wall with resultant localized infection and impaction at the stoma site. We review this problem and suggest a simple technique to prevent the complication.

Scapulothoracic penetration of a Beath pin: an unusual complication of arthroscopic Bankart suture repair.

The arthroscopic Bankart suture repair technique is an alternative to open procedures that control anterior shoulder instability. A case is presented in which a Beath pin traveled through the scapula and penetrated the scapulothoracic articulation during arthroscopic Bankart repair. The authors caution that strict adherence to Morgan's technique should minimize complications with this procedure.
