Engineering single-chain variable fragments to identify pathogenic antibodies in heparin-induced thrombocytopenia.

Heparin-induced thrombocytopenia (HIT) is characterized by the production of pathogenic antibodies that bind to complexes of platelet factor 4 (PF4) and heparin, causing platelet activation and a hypercoagulable state. A significant proportion of heparin-treated patients develop nonpathogenic anti-PF4/heparin antibodies that do not activate platelets but interfere with routine screening tests causing frequent false-positive results. We recently showed that pathogenic HIT antibodies are monoclonal and bind to an overlapping PF4 binding site, which could be exploited to identify clinically signi…

Heparin-binding enhances extracellular listeriolysin O activity, overcoming cholesterol inhibition and pH dependence.

The membranolytic toxin listeriolysin O (LLO), a member of the family of bacterial cholesterol-dependent cytolysins (CDC), is a key virulence factor of Listeria monocytogenes (Lm). Current evidence indicates that LLO is primarily a unique site-specific intracellular CDC, as it is highly active at the acidified pH of phagosomes but exhibits reduced activity and is prone to irreversible denaturation at physiological pH. Here, we demonstrate that LLO binds heparin and exploit this finding to affinity-purify the toxin in a single step. Heparin-binding increases LLO hemolytic activity at neutral pH…

Taking the 'HIT' (Heparin-induced thrombocytopenia) - Balancing Local Testing with Delayed Central Results.

AIM: To evaluate the frequency, turnaround time (TAT), and outcomes of Heparin-induced thrombocytopenia testing referrals from Cork University Hospital (CUH) and assess whether local specialised testing is justified. METHODS: Retrospective review of HIT referral forms and laboratory records (2021-2022). Data collected included demographics, referring specialty, platelet nadir, 4Ts score, HIT assay results, and TAT. RESULTS: Ninety-three patients were referred; 73 had complete data. Median TAT was 6 days (mean 31.9 days; adjusted mean 8.8 days after excluding six extreme outliers). Mean age w…

Alternative anticoagulants in heparin-sensitive patients undergoing carotid artery interventions: A scoping review.

BACKGROUND: Systemic unfractionated heparin (UFH) is routinely administered as procedural anticoagulation during carotid artery procedures. However, types I, II, and IV hypersensitivity to heparin can occur amongst patients with carotid artery disease who require carotid endarterectomy (CEA) or carotid artery stenting (CAS). Although rapid-acting nonheparin alternatives are available, their use for procedural anticoagulation in patients with suspected heparin hypersensitivity undergoing carotid intervention has not been thoroughly studied. We sought to evaluate the safety of nonheparin anticoa…

Heparin-grafted aligned chitosan/silk fibroin scaffolds with multifunctional bioactivities for cardiovascular applications.

Implantable cardiovascular biomaterials face critical clinical challenges, such as thrombosis, bacterial infection, and insufficient endothelial regeneration. In this study, aligned chitosan/silk fibroin (CS/SF) electrospun scaffolds were fabricated by heparin functionalization via parameter optimization, polydopamine-mediated surface modification, and controlled heparin grafting. A comprehensive characterization confirmed that the optimized scaffolds possessed well-aligned uniform nanofibers, desirable mechanical performance, adjustable hydrophilicity, and programmable degradation behavior. I…

Argatroban Anticoagulation for Heparin Induced Thrombocytopenia After Heartmate 3 Implantation: A Case Series.

BACKGROUND: Heparin-induced thrombocytopenia (HIT) is a rare but potentially fatal immune-mediated complication of heparin therapy. Its incidence is higher after left ventricular assist device (LVAD) implantation compared with other cardiac procedures. While bivalirudin is the most frequently reported alternative anticoagulant in this setting, evidence on argatroban use remains limited. We describe three cases of HIT following HeartMate 3 LVAD implantation, all managed with argatroban. RESULTS: Two patients were successfully transitioned to acenocoumarol (international normalized ratio 2-2.5)…

Clinical Challenges in Profound Thrombocytopenia Associated With Type 2 Heparin-Induced Thrombocytopenia (HIT2).

This case describes an unusually severe presentation of immune-mediated heparin-induced thrombocytopenia type 2 (HIT2), marked by an abrupt and profound decline in platelet count to levels below 2000/µL. Although HIT typically presents with moderate thrombocytopenia and thrombotic complications, such an extreme nadir in the absence of disseminated intravascular coagulation (DIC) or other hematologic disorders is highly atypical. The patient's course reinforces the need to consider HIT even when thrombocytopenia appears disproportionately severe and highlights that appropriate anticoagulation c…

Heparin prevents inflammation-induced pregnancy loss in mice through its anti-inflammatory effects.

Excessive inflammation at the feto-maternal interface is a major contributor to adverse pregnancy outcomes, including miscarriage and preterm birth. Heparin, which is widely used as an anticoagulant, has also been recognized as possessing anti-inflammatory properties; however, its role in inflammation-driven pregnancy complications remains unclear. In this study, we examined whether unfractionated heparin (UFH) prevents inflammation-induced miscarriage and preterm birth in mice. Lipopolysaccharide (LPS)-induced murine models were used to mimic inflammation-driven pregnancy complications. Pregn…

Emerging for non-invasive heparin delivery systems: recent advances, barriers, solutions, and applicability.

Nowadays, the use of unfractionated low molecular weight heparins through intravenous and subcutaneous routes has been limited by several delivery challenges. These include pharmacological activity fluctuations, bleeding issues, and numerous manufacturing restrictions. To address these issues, several efforts have been taken to find alternative routes for this medication. Unfortunately, the past and recent reviews were mainly explored the oral dosage forms of heparin and the other possible indications in practice. This review focuses on emerging efficient and non-invasive heparin options such …

Refractory delayed-onset heparin induced thrombocytopenia (HIT) without thrombosis, treated with intravenous immunoglobulin.

Heparin-induced thrombocytopenia (HIT) is a platelet activation syndrome characterized by the sudden onset of thrombocytopenia and/or thrombosis, induced by anti-platelet factor 4 (PF4)/heparin antibodies that cause platelet activation and destruction. Delayed-onset heparin-induced thrombocytopenia is a variant of HIT that occurs in less than 5 % of cases and is characterized by the onset of thrombocytopenia and/or thrombosis several days after heparin discontinuation. With treatment, the platelet count usually recovers within a week, but in up to 1 % of patients, thrombocytopenia persists for…

Fatal Pulmonary Embolism Secondary to Heparin-Induced Thrombocytopenia in Hemodialysis: A Case Study.

BACKGROUND: Heparin and low-molecular-weight heparin (LMWH) are first-line anticoagulants in hemodialysis but may trigger heparin-induced thrombocytopenia (HIT), a severe immune-mediated complication. Despite its clinical significance, HIT is often misdiagnosed early, increasing thromboembolic risks and mortality. CASE REPORT: We present a case of a 61-year-old male patient who developed HIT during initial hemodialysis. The patient exhibited recurrent catheter thrombosis, intermuscular venous thrombosis in the lower extremities, and progressive thrombocytopenia following LMWH administration d…

Optimized Cell-Based Platform for Platelet-Free Detection of Heparin-Induced Thrombocytopenia Antibodies.

Accurate detection of heparin-induced thrombocytopenia (HIT) antibodies is crucial for diagnosing and managing thrombotic events. Conventional immunoassays, however, often lack specificity and require confirmatory testing with fresh human platelets. To address this limitation, we optimized our previously developed cell-based enzyme-linked immunosorbent assay (ELISA) for improved HIT detection under various experimental conditions. Platelet factor 4 was immobilized on breast cancer cells (MDA-MB-231) to capture monoclonal HIT-like (KKO) and non-HIT (RTO) antibodies, which served as models to ev…
