Radiation-induced morphological changes in the vagina.

BACKGROUND AND PURPOSE: Treatment-induced chronic vaginal changes after definitive radio(chemo)therapy for locally advanced cervical cancer patients are reported as one of the most distressing consequences of treatment, with major impact on quality of life. Although these vaginal changes are regularly documented during gynecological follow-up examinations, the classic radiation morbidity grading scales are not concise in their reporting. The aim of the study was therefore to identify and qualitatively describe, on the basis of vaginoscopies, morphological changes in the vagina after definitive

Survivor of a traumatic atlanto-occipital dislocation.

Atlanto-occipital dislocation is a devastating ligamentous injury that most often turns fatal. However, because of on-site resuscitation improvements, the emergency teams are increasingly dealing with this condition. We report a rare case of atlanto-occipital dislocation (AOD) in a surviving patient with more than one-year follow-up. The mechanism of injury appears to be an extreme hyperextension applied to the head. This injury occurs more frequently in children since they are anatomically predisposed (flat articulation between the occiput and the atlas, increased ligamentous laxity). The dia

Osteochondromas of the cervical spine in atypical location.

In the current article we review the available English literature by pubmed search engine on the topic of osteochondromas and their location in the cervical spine. The focus is to investigate the location of the lesions in cervical spine and to analyze clinical presentations and symptoms of patients, the aetiology and histopathology examination of the masses. In addition we report a rare illustrative case of a 62 year-old man with an osteochondroma of the cervical spine. The mass developed at a very slow rate for many years and produced no clinical symptoms. The location of the extradural mass

Anterior retropharyngeal approach to C1 for percutaneous vertebroplasty under C-arm fluoroscopy.

BACKGROUND CONTEXT: Percutaneous vertebroplasty (PVP) has proven to be a valuable palliative treatment option for patients with medically refractory painful osteolytic metastases of the spine. Percutaneous vertebroplasty of the atlas has been reported in only seven articles and has been performed with different techniques and approaches. PURPOSE: To describe the technique we used to perform PVP of a lytic lesion of the lateral mass of C1 via anterior retropharyngeal approach guided by C-arm fluoroscopy. STUDY DESIGN: A technical report. PATIENT SAMPLE: It included a 75-year-old man with kno

Incidental findings arising with cone beam computed tomography imaging of the orthodontic patient.

Cone beam computed tomography (CBCT) of orthodontic patients is a diagnostic tool used increasingly in hospital and primary care settings. It offers a high-diagnostic yield, short scanning times, and a lower radiation dose than conventional computed tomography. This article reports on four incidental findings-that appear unrelated to the scan's original purpose-arising in patients for whom CBCT was carried out for orthodontic purposes. It underlines the need for complete reporting of the data set.

Endoscopic-assisted lateral transatlantal approach to craniovertebral junction.

BACKGROUND: The lateral approach to the craniocervical junction is directed along the atlantal and occipital condyles to the dens. The advantages of the lateral approach compared with the anterior transoral and transnasal approaches are that it provides a sterile field, and anterior decompression and postdecompression fixation can be performed in one procedure. OBJECTIVE: To examine the usefulness of endoscopy as an auxiliary tool during lateral transatlantal odontoidectomy. METHODS: Six cadaver heads, in which the vessels were injected with colored silicone, were dissected using a surgical

[Atlas fractures with paralysis of the caudal cranial nerves. A case report].

The fracture of the Atlas is a serious trauma, associated with important diagnostic traps. Whether operative or noninvasive treatment should be used depends on the type of fracture. Paralysis of brain nerves are exceptionally rare. We present a case in which fracture of the atlas was associated with paralysis of the 9th through 12th brain nerves, to which differential diagnostic considerations are discussed.

SERPINH1 is a Potential Prognostic Biomarker and Correlated With Immune Infiltration: A Pan-Cancer Analysis.

Background: Serpin peptidase inhibitor clade H, member 1 (SERPINH1) is a gene encoding a member of the serpin superfamily of serine proteinase inhibitors. The upregulated of SERPINH1 was associated with poor prognosis in breast cancer, stomach adenocarcinoma, and esophageal carcinoma. However, the role of SERPINH1 in pan-cancer is largely unexplored. Methods: SERPINH1 expression and the correlation with prognosis in human pan-cancer were analyzed by the Cancer Genome Atlas and the Genotype-Tissue Expression dataset. Pearson correlation analysis was applied to evaluate the role of SERPINH1 expr

C1 pedicle screws versus C1 lateral mass screws: comparisons of pullout strengths and biomechanical stabilities.

STUDY DESIGN: In vitro biomechanical study. OBJECTIVE: To compare the pullout strengths and the biomechanical stabilities afforded by C1 lateral mass screws and C1 pedicle screws using bicortical and unicortical fixation techniques. SUMMARY OF BACKGROUND DATA: Posterior screw fixation techniques in the atlas including C1 lateral mass screw and C1 pedicle screw. The shortcomings of C1 lateral mass screw technique and potential risks of bicortical fixation method were recently described; C1 pedicle screw technique with unicortical fixation might overcome these anatomic and clinical drawbacks.

Precision oncology in gynecologic cancers: molecular taxonomy, biomarker-guided therapeutics, and the challenge of therapeutic resistance.

INTRODUCTION: The clinical management of gynecologic malignancies endometrial (EC), ovarian (OC), and cervical (CC) carcinomas has been historically guided by histomorphology and staging. This paradigm fails to capture profound molecular heterogeneity, resulting in suboptimal outcomes. Precision oncology, through molecular taxonomy and biomarker-guided therapy, aims to address this gap. METHODS: This review synthesizes the current landscape of precision oncology in gynecologic cancers by analyzing established molecular classifications, the evidence for biomarker-guided therapies, mechanisms o

Posterior atlanto-axial fixation with polyaxial C1 lateral mass screws and C2 pars screws.

PURPOSE: C1-C2 instability or painful osteoarthritis are recognised indications for posterior atlanto-axial fixation. In the traditional trans-articular C1-C2 screw fixation, up to 20% of patients cannot have safe placement of bilateral screws in the event of a medially located vertebral artery and a straight screw trajectory in the sagittal plane. The more recently developed C1-C2 fixation technique with individual C1 lateral mass screws and converging C2 pars screws can be employed in case of a medially located vertebral artery and has comparable biomechanical strength. This is a prospective
