The Role of Perinatal Palliative Care in Fetal Neurology.

OBJECTIVES: Many serious or life-threatening neurologic conditions are first diagnosed during the fetal period, often following a routine ultrasound or sonographic evaluation after an abnormal aneuploidy screen. Such conditions represent a worrisome or unexpected finding for expectant parents, making the perinatal period a critical time point to engage and empower families encountering complex neurologic clinical scenarios. This review covers the role of perinatal palliative care in these settings. STUDY DESIGN: This study is a topical review RESULTS:  The prenatal identification of structura

Managing 'shades of grey': a focus group study exploring community-dwellers' views on advance care planning in older people.

BACKGROUND: Community-dwelling consumers of healthcare are increasing, many aging with life-limiting conditions and deteriorating cognition. However, few have had advance care planning discussions or completed documentation to ensure future care preferences are acted upon. This study examines the awareness, attitudes, and experiences of advance care planning amongst older people and unrelated offspring/caregivers of older people residing in the community. METHODS: Qualitative descriptive research, which included focus groups with older people (55+ years) and older people's offspring/caregiver

One Clinician, Every Conversation: Palliative Care Delivery by Solo APRN Hospitalists in Critical Access Hospitals.

Advanced practice registered nurses (APRN) who practice as solo hospitalists in critical access hospitals are often underrecognized in their role in providing palliative and end-of-life care. Palliative and hospice care specialty services are often not readily available for consultations in a timely manner, especially in highly remote areas of the country. These hospitalists routinely work without an option for palliative care consultations, ethics committees, or structured advance care planning processes. These hospitalists must simultaneously manage admissions, discharges, inpatient deterior

Psychological experiences of midwives during their first involvement in care following intrauterine foetal death: A mixed methods study in Japan.

BACKGROUND: Providing care following intrauterine foetal death (IUFD) can be emotionally challenging for midwives, particularly during early professional practice. However, little is known about how midwives experience and cope with their first involvement in IUFD care. AIM: To examine midwives' retrospective accounts of their first IUFD care experience within the first 3 years of practice and explore their coping processes. METHODS: A convergent mixed methods design was used. Forty-one midwives in Japan completed questionnaires assessing emotional responses to their first IUFD care and prof

Palliative Care in the Management of Patients with Advanced Heart Failure.

Globally, there are 18-million individuals living with heart failure, a disease that is responsible for 12-15 million office visits and 6.5 million inpatient hospitalizations each year. As HF becomes advanced or end-stage, patients often live in a cycle of frequent transitions between care settings, and with unmet needs, including distress from inadequately managed symptoms. Prognostication in patients with heart failure can be challenging due to the unpredictable exacerbating-remitting illness trajectory that is associated with this progressive disease. Recurrent hospitalizations, worsening f

An ethically-justifiable, practical approach to decision-making surrounding conjoined-twin separation.

Conjoined twins present unique ethical and palliative care challenges. We present an ethically-justifiable, practical approach to decision-making with regards to surgical separation. These decisions must account for the short- and long-term prognoses for each infant prior to, and after, separation. Other considerations include the benefits and burdens of separation and the family's values and goals. Caregivers should recognize that decisions surrounding separation may be unduly influenced by social biases. The palliative care team aids in developing goals of care to guide decision-making by pr

Fasting at Life's End: Exploring the Spiritual and Medical Tensions in Palliative Care.

Voluntary fasting at the end of life raises complex spiritual, ethical, and clinical challenges for palliative care providers. This qualitative study explored healthcare professionals' experiences in managing patient-initiated fasting within a culturally and religiously diverse palliative care context in Saudi Arabia. Sixteen participants-including nurses, physicians, and allied health professionals-were interviewed using a phenomenological-hermeneutic approach and analyzed through reflexive thematic analysis. Four interrelated themes emerged: (1) between respect and responsibility, reflecting

Prolonged grief disorder in end-of-life care: current literature and future directions.

Prolonged grief disorder (PGD) is a recognized distinct mental health disorder in the DSM-5-TR and ICD-11, characterized by core symptoms such as longing for and preoccupation with the deceased that persist and worsen over time. Family members and caregivers of patients that receive end-of-life care are confronted with imminent bereavement, which emphasizes the importance of bereavement care as a preventive measure and the need for continued support after death beyond the hospital setting. This review discusses bereaved family members' and caregivers' risk factors of developing PGD, screening

Oceans Apart - at the extremes of personalised care in the new Cystic Fibrosis paradigm.

The advent of CFTR modulators has revolutionised Cystic Fibrosis (CF) care, improving life expectancy, physiological parameters and reducing exacerbations. These are matched by changes in life expectations and ambitions of people living with CF (pwCF). This case study follows Sophie, a pwCF who completed a 54-day transatlantic row in early 2025, posing a unique challenge to our multidisciplinary team (MDT) that required bespoke support before, during and after her historic achievement.

Acceptability and feasibility of the Home Support Needs Assessment in a forensic mental health service: An initial study.

INTRODUCTION: Identifying and addressing the support needs of forensic mental health patients in daily life occupations is critical yet complex due to the health comorbidities and environmental restrictions impacting their participation. Occupational therapists support forensic patient recovery by facilitating their performance in daily life occupations required at home and in the community. Standardised assessments of daily living are important for guiding interventions. The Home Support Needs Assessment (HSNA) is a standardised measure identifying unmet support needs in daily life occupation

Nutritional Care in Children with Cystic Fibrosis.

Patients with cystic fibrosis (CF) are prone to malnutrition and growth failure, mostly due to malabsorption caused by the derangement in the chloride transport across epithelial surfaces. Thus, optimal nutritional care and support should be an integral part of the management of the disease, with the aim of ameliorating clinical outcomes and life expectancy. In this report, we analyzed the nutrition support across the different ages, in patients with CF, with a focus on the relationships with growth, nutritional status, disease outcomes and the use of the CF transmembrane conductance regulator
