Wrist and Hand Ultrasound: Reliability of Side-to-Side Comparisons of Very Small (<2-mm) Clinically Relevant Anatomic Structures.

OBJECTIVES: In ultrasound (US) examinations of clinically relevant very small structures of the wrist and hand, the healthy contralateral side can be used as a reference to identify subtle abnormalities. Intraindividual side-to-side variability must be minimal. The aim of this study was to assess the reliability of side-to-side US evaluations of very small structures of the wrist and hand. METHODS: Forty-one healthy volunteers were prospectively studied. Small structures of the wrist and hand were evaluated bilaterally by 2 musculoskeletal radiologists in separate sessions. The first annular

Structural and functional cardiac analyses using modern and sensitive myocardial techniques in adult Pompe disease.

The purpose of this study was to analyze comprehensively the heart using modern and sensitive myocardial techniques in order to determine if structural or functional cardiac alterations are present in adult Pompe disease. Twelve patients with adult Pompe disease and a control group of 187 healthy subjects of similar age and gender were included. Structural and functional cardiac characteristics were analyzed by conventional and 2D speckle-tracking echocardiography. In addition, in a subgroup of adult Pompe patients, we analyzed the myocardial and musculoskeletal features by means of cardiac an

Epistasis between polymorphisms in COMT, ESR1, and GCH1 influences COMT enzyme activity and pain.

Abnormalities in the enzymatic activity of catechol-O-methyltransferase (COMT) contribute to chronic pain conditions, such as temporomandibular disorders (TMD). Thus, we sought to determine the effects of polymorphisms in COMT and functionally related pain genes in the COMT pathway (estrogen receptor 1 [ESR1], guanosine-5-triphosphate cyclohydrolase 1 [GCH1], methylenetetrahydrofolate reductase [MTHFR]) on COMT enzymatic activity, musculoskeletal pain, and pain-related intermediate phenotypes among TMD cases and healthy control subjects. Results show that the COMT rs4680 (val(158)met) polymorp

Specific Overuse Injuries in Professional Athletes.

Overuse injuries are a major cause of pain, performance decline, and time loss in professional athletes. Unlike acute traumatic injuries, these conditions develop gradually because of repetitive submaximal loading and inadequate recovery. Recurrent mechanical stress may lead to microstructural damage involving tendons, bones, cartilage, ligaments, and surrounding soft tissues. Overhead athletes, runners, and jumping athletes are particularly susceptible to chronic overload syndromes, including rotator cuff tendinopathy, patellar tendinopathy, Achilles tendinopathy, and stress fractures. Lumbar paraspinal compartment syndrome with concomitant rhabdomyolysis and multifidus necrosis.

Acute lumbar paraspinal compartment syndrome is a rare, function-threatening emergency often misidentified as benign musculoskeletal strain. We describe a 29-year-old male who presented to the emergency department (ED) with acute low back pain following a touch football game. Despite an initial discharge for muscle spasm, he returned 24 h later with refractory pain and laboratory evidence of rhabdomyolysis (creatine kinase 14,399 U/L). Magnetic resonance imaging revealed edema of the left paraspinal musculature. Due to persistent pain and progressive laboratory abnormalities, emergent surgical

Application of Prenatal Whole Exome Sequencing for Structural Congenital Anomalies-Experience from a Local Prenatal Diagnostic Laboratory.

Fetal structural congenital abnormalities (SCAs) complicate 2-3% of all pregnancies. Whole-exome sequencing (WES) has been increasingly adopted prenatally when karyotyping and chromosomal microarray do not yield a diagnosis. This is a retrospective cohort study of 104 fetuses with SCAs identified on antenatal ultrasound in Hong Kong, where whole exome sequencing is performed. Molecular diagnosis was obtained in 25 of the 104 fetuses (24%). The highest diagnostic rate was found in fetuses with multiple SCAs (29.2%), particularly those with involvement of the cardiac and musculoskeletal systems.

A successful anesthetic approach in a patient with Schwartz-Jampel syndrome.

Schwartz-Jampel syndrome (SJS) is a rare genetic condition that is characterized by several musculoskeletal abnormalities, such as myotonia, joint contractures, and facial dysmorphisms. Patients with this syndrome can present an anesthetic challenge, due to an increased risk of developing malignant hyperthermia (MH) and the possibility of encountering a difficult airway. Several precautions must be taken when general anesthesia is required, such as the avoidance of potential triggers for MH, continuous core temperature, and end-tidal CO2 monitoring, assuring that dantrolene is readily availabl…

Role of Imaging and Cytogenetics in Evaluation of DiGeorge Syndrome - A Rare Entity in Clinical Practice.

DiGeorge syndrome is a congenital genetic disorder that affects the endocrine system, mainly the thymus and parathyroid glands. The syndrome produces different symptoms, which vary in severity and character between patients. It manifests with craniofacial dysmorphism and defects in the heart, parathyroid, and thymus. Patients can present with a palatal deformity and nasal speech. This rare entity is caused mainly due to deletion of chromosome 22q11.2. Radiographic evaluation of DiGeorge syndrome is necessary to define aberrant anatomy, evaluate central nervous system, craniofacial abnormalitie

[The diabetic hand].

Diabetes mellitus is a chronic metabolic condition characterized by persistent hyperglycaemia with resultant morbidity and mortality related to its microvascular and macrovascular complications. In addition diabetes is also associated with several musculoskeletal disorders of the hand, that can be debilitating. There is increased incidence of these abnormalities in patients with type 1 and type 2 diabetes compared with the general population, related to disease duration but not to the age or sex. Typical diabetes associated hand condition include the palmar flexor tenosynovitis, Dupuytren's co

Is there need for change of health examinations for sea pilots?

Sea pilots must be capable of carrying out their work in all situations. Thus, they must not have any disease or defect, that could impair their job performance. By periodic medical examinations attempts are made to ensure their working capacity. In most countries these examinations are carried out by a general practitioner and they include only few if any objective laboratory tests. The aim of the present investigation was to study the effectiveness of the periodic medical examinations to find out in the population of pilots examined persons with health risks, especially risks for cardiovascu

Effects of different husbandry systems and attitudes of cattle farmers on the behavior and welfare of cattle in Germany.

This study examines the relationship between the attitude of cattle farmers toward their animals and the occurrence of behavioral and health problems in cattle. Various factors, including animal behavior, husbandry conditions and the frequency of diseases, are taken into account. The results of an online survey of 1,242 German cattle farmers indicated that a range of factors, including the husbandry system, the number of cattle, grazing practices and the human-animal relationship, can influence the occurrence of abnormal behavior in cattle, such as tongue play, mutual sucking, horse-like sitti

Genu recurvatum in cerebral palsy--part B: hamstrings are abnormally long in children with cerebral palsy showing knee recurvatum.

Hyperextension of the knee in stance (knee recurvatum) is a common disorder in patients with spastic cerebral palsy (CP). A group 35 children with CP (47 lower limbs) was divided into two subgroups according to the timing of maximum knee extension during the stance phase of gait. Gait analysis and musculoskeletal modelling data were compared with a control group of 12 normally developing children. We observed no difference in kinematics between the CP groups who showed an equinus position of the foot at initial contact. Both groups showed increased external extensor moments across the knee. Four-dimensional quantitative analysis of the gait of mutant mice using coarse-grained motion capture.

To analyze an abnormal gait pattern in mutant mice (Hugger), we conducted coarse-grained motion capture. Using a simple retroreflective marker-based approach, we could detect high-resolution mutant-specific gait patterns. The phenotypic gait patterns are caused by extreme vertical motion of limbs, revealing inefficient motor functions. To elucidate the inefficiency, we developed a musculoskeletal computer model of the mouse hindlimb based on X-ray CT data. By integrating motion data with the model, we determined mutant-specific musculotendon lengths, suggesting that three major muscles were in

Ultrasonographic findings of shoulder disorders in patients with Parkinson's disease.

The prevalence of a history of shoulder complaints is higher in patients with Parkinson's disease (PD) than in healthy individuals. The aims of this study were to evaluate shoulder disease in PD patients with ultrasonography (US) and to identify the relationship between the US findings of shoulder disease and the motor signs in patients with PD. Thirty-three PD patients completed a shoulder disability questionnaire, a musculoskeletal examination, and US of the shoulder. Twenty-two patients had abnormal US findings. Tendon tearing was the most common abnormal US finding (22/22), and the suprasp

Late effects of gastric bypass for obesity.

We studied 41 patients who had gastric bypass for obesity from 1974-1979. The procedure was well received by patients and most achieved adequate weight loss, but most subjects consumed inadequate diets and many developed iron and/or vitamin B12 deficiencies. Ten were anemic and 13 had been treated previously for postbypass anemia. Severely vitamin B12-deficient subjects did not respond to 50 micrograms oral vitamin B12 tablets, but those with milder deficiencies usually did. Schilling tests were usually abnormal and corrected when intrinsic factor was given. Many subjects developed manifestati

Clinical efficacy of soft tissue micro-adjustment combined with traction in pediatric atlantoaxial subluxation: a randomised controlled study protocol using musculoskeletal ultrasound technology.

BACKGROUND: Atlanto-axial rotatory subluxation (AARS) in pediatric patients is characterized by abnormal or restricted motion between the atlas and axis vertebrae, typically presenting with neck pain, limited mobility, torticollis, and muscle stiffness. Cervical atlanto-occipital joint x-ray in open-mouth position or CT scan reveals an atlanto-occipital distance (AOD) of 2 mm < AOD < 5 mm, or a bilateral atlanto-occipital lateral distance (B-LAD) ≥ 2 mm. Although traditional manipulative and bone-setting techniques in Traditional Chinese Medicine have demonstrated clinical benefits, robust emp
