Novel treatments for myasthenia gravis.

Advances in immunology over the last several years have provided insights into the pathophysiology of autoimmune diseases such as generalized myasthenia gravis (gMG). This has translated into the development of effective, rapidly acting, and targeted novel immune therapies. The two categories of new therapies available at present include the complement C5 inhibitors and the neonatal Fc receptor (FcRn) antagonists. The place of these drugs in the algorithm of MG treatment continues to evolve. Simultaneously, drug development proceeds, with other complement inhibitors, new B cell inhibitor thera…

Exercise and myasthenia gravis.

The role of non-pharmacological management in MG is currently underdeveloped and underutilized. Exercise training has been shown to be both feasible and beneficial in improving function and strength in individuals with MG. Observational studies indicate that physical activity is associated with reduced perceived fatigue. Barriers to exercise persist in MG, and should be addressed by healthcare professionals. Maintaining or increasing muscle strength and endurance as well as cardiorespiratory fitness should be prioritized regardless of current level of function. In the general population, exerc

The clinical evaluation of myasthenia gravis.

The diagnosis of myasthenia gravis (MG) is strongly considered in a patient complaining of fatigable weakness and in whom muscle fatigability is demonstrated during bedside examination. This chapter will be dedicated to the history-taking and clinical examination of patients with MG and we will also touch upon other myasthenic syndromes and mimics. Most patients first experience fatigable ocular symptoms. The symptoms may remain isolated to the eyes (ocular MG) or later involve the limb, bulbar or respiratory muscles (generalized MG). The examination of the myasthenic patient serves to demonst…

Myasthenia gravis in women.

Myasthenia Gravis (MG) often affects young women, in whom reproductive issues are of importance, including the possibility of MG exacerbation during pregnancy, potential obstetric risks in pregnancy and treatment-related risks to the fetus. Apart from pregnancy, hormonal changes throughout a woman's lifespan may also affect MG symptoms. In this chapter, we discuss the role of sex hormones in MG and the disease burden in women. We also review the effects of pregnancy on MG, the evidence regarding pregnancy outcomes and current recommendations regarding MG treatments during pregnancy.

Clinical outcome measures.

The typical fluctuating skeletal muscle weakness in myasthenia gravis can make measuring disease severity challenging. Multiple measures have been developed to assess the signs and symptoms of myasthenia, from pure examination measures to fully patient-reported measures to combinations. There are also measures aimed at assessing quality of life and satisfaction. Electrophysiology and serology have also been used in some studies as surrogate outcomes. In this chapter, we review outcomes specifically developed to assess disease severity in myasthenia, as well as quality of life, and data on elec…

Survival Outcomes of Medicare-Covered Elderly US Population With Myasthenia Gravis.

INTRODUCTION/AIM: Survival outcomes have been inadequately studied among people with myasthenia gravis (MG) in the United States (US). We examined the impact of MG and comorbid conditions on longevity. METHODS: We performed a longitudinal study using Medicare claims data (2006-2019). Incident MG cohort was identified based on the following criteria: age ≥ 65 years; ≥ 1 month of fee-for-service Parts A and B coverage; no health maintenance organization coverage; initial and subsequent MG claims within 2010-2011 separated by ≥ 28 days. A non-MG cohort of five times the number of the MG group wa…

Robot-assisted extended thymectomy in juvenile myasthenia gravis: first case performed in Ecuador.

Myasthenia gravis (MG) is a chronic autoimmune disorder characterized by fatigable muscle weakness, typically caused by antibodies against acetylcholine receptors (AChR) or muscle-specific kinas. While it primarily affects adults, juvenile cases represent a significant minority and often pose greater therapeutic challenges. Extended thymectomy has proven beneficial in AChR-positive MG, reducing symptoms and immunosuppressive needs. Robotic-assisted thymectomy offers enhanced precision with lower morbidity. We report the first such procedure in Ecuador, performed on a 21-year-old woman with rec…

Coexistence of Acute Demyelinating Polyneuropathy and LRP4-Positive Myasthenia Gravis.

INTRODUCTION: Guillain-Barré syndrome (GBS) and myasthenia gravis (MG) are rare autoimmune disorders that may share overlapping features such as ophthalmoparesis, limb weakness, and bulbar symptoms, complicating the differential diagnosis. Coexistence of GBS and MG or chronic inflammatory demyelinating polyneuropathy and MG, particularly the low-density lipoprotein receptor-related protein 4 (LRP4) antibody-positive subtypes, is extremely rare. We present such a case to highlight diagnostic challenges. CASE PRESENTATION: A 46-year-old man presented with distal weakness, sensory loss, facial d…

Cost-Effectiveness Analysis of Efgartigimod vs Chronic Immunoglobulin for the Treatment of Myasthenia Gravis in Canada.

INTRODUCTION: Generalized myasthenia gravis (gMG) is a chronic neuromuscular disease that causes muscle weakness and fatigue, severely impairing quality of life. Efgartigimod is a novel drug that is recently approved for treatment of acetylcholine receptor antibody-positive (AChR-Ab+) gMG patients in Canada. In clinical practice, it is expected to be used in AChR-Ab+ gMG patients who continue to experience symptoms despite conventional therapy and primarily replace chronic immunoglobulins. METHODS: A Markov model was developed to estimate costs and benefits (measured as quality-adjusted life …

Rehabilitation for functional enhancement of myasthenia gravis: A systematic review and meta-analysis.

Myasthenia gravis (MG) is a rare neuromuscular disorder that causes muscle weakness and fatigue. This review evaluated the current evidence on the efficacy, tolerance and adherence of rehabilitative interventions. Based on the PRISMA guidelines, 445 articles were identified from major scientific databases. After applying the inclusion and exclusion criteria, 6 studies were included in the final analysis. Two primary rehabilitative strategies - physical and respiratory training - were identified. The results of the systematic review and meta-analysis showed that both interventions improved func…

Management of Myasthenic Crisis and Emerging Roles of Molecularly Targeted Therapies: A Narrative Review.

Myasthenia gravis (MG) is a chronic autoimmune disorder characterized by fluctuating skeletal muscle weakness. Myasthenic crisis (MCr), a severe and potentially life-threatening complication, presents with respiratory failure and requires intensive care and rapid immunomodulatory intervention. Conventional MCr treatments-such as plasma exchange (PLEX), intravenous immunoglobulin (IVIG), and intravenous methylprednisolone (IVMP)-remain standard treatments; however, they present significant limitations, including delayed onset of action, adverse effects, and inconsistent efficacy. Recent therape…

Olfactory Dysfunction in Patients With Acetylcholine-Receptor-Antibody-Positive Myasthenia Gravis.

BACKGROUND AND PURPOSE: Myasthenia gravis (MG) is an antibody-mediated disease characterized by fluctuating muscle weakness and fatigue due to impaired neuromuscular junction transmission. Although primarily considered a motor dysfunction disorder, there is emerging evidence that MG can also present with nonmotor symptoms, including olfactory impairment. However, the prevalence and clinical relevance of olfactory dysfunction in MG remain poorly understood. This study compared olfactory function between MG patients and healthy controls with the aim of identifying clinical factors associated wit…

Personalized Management and Timing of Thymectomy in Juvenile Myasthenia Gravis: Insights from Routine Clinical Scale Use in a Single-Center Retrospective Cohort-A Case Series.

Introduction: Juvenile myasthenia gravis (JMG) is a rare autoimmune disorder with a variable clinical course and limited pediatric-specific treatment guidelines. Objective clinical scales, such as the Quantitative Myasthenia Gravis (QMG) Score and the Pediatric Myasthenia Gravis Quality of Life 15 (PM-QOL15), may support individualized management, but their role in routine practice remains underexplored. Methods: We retrospectively reviewed 10 seropositive JMG patients followed at a single tertiary neuromuscular clinic between 2014 and 2024. All patients underwent a systematic assessment with …

Maintaining Surgical Principles While Transitioning From Multiport to Single-port Robotic Thymectomy.

Thymectomy is a treatment option for new-onset or treatment-resistant myasthenia gravis in patients with muscle-type acetylcholine receptor autoantibodies, and it is the standard treatment of early-stage thymomas. However, patients with severe forms, particularly those with titin and ryanodine receptor antibodies, may require long-term immunosuppressive therapy instead. The optimal approach to thymectomy remains debated, with minimally invasive techniques like video-assisted and robot-assisted thoracoscopic surgery offering better perioperative outcomes. Single-port robot-assisted thymectomy h…

Interferon regulatory factor 1 enhances T cell differentiation in patients with myasthenia gravis.

Interferon regulatory factor 1 is involved in many autoimmune conditions and is increased in patients with myasthenia gravis. However, its function in myasthenia gravis remains unclear. Herein, we explored the function of interferon regulatory factor 1 in myasthenia gravis, with an aim to understand the underlying mechanisms. Patients with myasthenia gravis who had acetylcholine receptor antibodies were included in the study. Peripheral blood lymphocytes were extracted from the included patients, and B lymphocyte subsets were isolated. Next, T and B cells from peripheral blood were co-cultured…
