Chronic axonal-demyelinating polyradicular neuropathy associated with mycosis fungoides: a case report.

Case of a 58-year-old patient diagnosed as having mycosis fungoides (MF) who presented with sensory ataxia, limbs weakness and neurophysiological and laboratory signs indicative of autoimmune chronic mixed axonal-demyelinating sensory-motor polyradicular neuropathy. The possibility that MF may be associated with an immunomediated peripheral neuropathy, even in the absence of a direct invasion of the nerves and widespread visceral involvement, should be considered.

Trigger digits-associated carpal tunnel syndrome: relationship between carpal tunnel release and trigger digits.

Of 875 idiopathic carpal tunnel syndrome (CTS) cases, 101 (11.5%) required trigger digit release operations within three years before and/or after carpal tunnel release (CTR); these 101 cases were investigated, retrospectively. Trigger digit release (TDR) was performed most often after the CTR, especially within three months. Next most common was at the same time as the CTR. The TDR performance rate after CTR was 5.9%. The nerve conduction study (NCS) comparison between trigger digits-associated CTS and isolated CTS showed that pre-operative distal motor latency was significantly more delayed

[Riche-Cannieu anastomosis and a paradoxical preservation of thenar muscles in carpal tunnel syndrome: a case report].

A 72-year old woman had been suffered from a dysesthesia in the left median nerve distribution, followed by a dysesthesia in the right ulnar distribution. Neurological examination revealed weakness in the right intrinsic hand muscles with the ipsilateral thenar and hyothenar atrophy. Paradoxical preservation of the left thenar muscles was a clinical challenge. Nerve conduction studies disclosed bilateral carpal tunnel syndrome and the right cubital tunnel syndrome. In needle electromyography, however, the left abductor pollicis brevis and opponens pollicis muscles had normal motor unit potenti

[Multifocal motor neuropathy with conduction blocks and prurigo nodularis. A paraneoplastic syndrome in a patient with non-Hodgkin B-cell lymphoma?].

Multifocal motor neuropathy with conduction blocks (MMNCB) is a peripheral demyelinating neuropathy. The etiology of this disease is unknown, but an autoimmune origin is postulated. Prurigo nodularis (PN), a chronic dermatosis also having an unknown etiology and many peripheral neuropathies of different nature are associated to hematological tumors. We have found no cases in the literature in which MMNCB was presented as a paraneoplastic syndrome of a non-Hodgkin B-cell type lymphoma (NHL-B). We present the case of a 67 year old man who simultaneously developed PN and MMNCB in upper limbs and

Compression of the ulnar nerve at the elbow by an intraneural ganglion.

A rare cause of ulnar nerve compression at the elbow is presented in this report. A 42 year old right-handed mechanic developed subacute, progressive numbness, tingling and weakness in his right hand. Electrophysiologic studies demonstrated a severe conduction block affecting the ulnar nerve in the retrotrochlear groove but without any sign of major axonal loss. His hand functions were carefully studied prior to surgery. While fine motor tasks were not affected, the hand strength was markedly diminished. At surgery, a 1-cm diameter intraneural ganglion at the site of the conduction block was f

[Polyneuropathy index-revised in the evaluation of diabetic neuropathy].

The polyneuropathy index-revised(PNI-R), based on 8 electrophysiological parameters(conduction velocities and F-latencies), was constructed to obtain an overall estimation of peripheral nerve conduction in diabetic patients, taking PNI as a model. PNI was calculated as a mean percentage of the normal on 12 velocity or latency parameters on motor nerve conduction studies. PNI-R is composed of 8 parameters; motor nerve conduction velocities in the forearm or leg segment and F-wave latencies after wrist or ankle stimulation concerning to the median, ulnar, peroneal and posterior tibial nerves. F-

[Nerve conduction velocity of repeater F-waves is identical to that of M-waves].

F-wave normally varies in latency and waveform from one response to the next. But the number of identical responses in a series of F-waves may be increased with neurogenic atrophy consistent with a decreased number of motoneurons capable of responding to antidromic stimulation. They are called "repeater F-waves". We herein demonstrate some repeater F-waves observed in three patients with moderate or slight diabetic polyneuropathy. In their motor nerve conduction studies on the peroneal nerve the maximum conduction velocity was 33 m/sec in patient 1, 36 m/sec in patient 2 and 48 m/sec in patien

Chronic effects of the nucleus pulposus applied to nerve roots on ectopic firing and conduction velocity.

STUDY DESIGN: This study analyzed the effects of the autologous nucleus pulposus (NP) on nerve root function using electrophysiologic methods. OBJECTIVES: To investigate the chronic effects of the NP applied to nerve roots and dorsal root ganglions (DRGs) on ectopic firing and nerve conduction velocity. SUMMARY OF BACKGROUND DATA: It was reported that ectopic firing in DRGs is involved in abnormal sensation such as sciatica and that ectopic firing was increased shortly after application of the NP to nerve roots. It was also reported that various inflammatory reactions occur around herniated

Median and ulnar palm-wrist studies.

OBJECTIVES: Routine carpal tunnel electrodiagnosis frequently includes median (MPW) and ulnar (UPW) palm-wrist mixed nerve conduction latency determinations over 8 cm. Despite widespread use, normative palmar latency difference (PLD) and UPW values, and the relative utility of onset latency (OL) or peak latency (PL) measurements are controversial. The current study was conducted to determine normative values for these parameters. METHODS: MPW and UPW studies were performed unilaterally in 33 normal controls. The PLD-OL and PLD-PL were calculated. The mean, range, standard deviation, and upper

Carpal tunnel syndrome - electrodiagnostic aspects of fifty seven symptomatic hands.

Electrodiagnostic data of fifty seven symptomatic extremities with carpal tunnel syndrome (CTS) are described. Practice recommendations made by American Academy of Neurology, American Association of Electrodiagnostic Medicine and American Academy of Physical Medicine and Rehabilitation regarding electrodiagnostic studies were considered while confirming CTS diagnosis by electrodiagnostic studies. Median sensory nerve conduction studies were the commonest abnormalities noted. The median orthodromic sensory latencies were prolonged in 86% and sensory nerve action potential amplitude abnormalitie

Brain excitability in migraine: evidence from transcranial magnetic stimulation studies.

Central neuronal hyperexcitability is proposed to be the putative basis for the physiologic disturbances in migraine. Because there are no structural disturbances in migraine, only physiologic studies can provide insight into the underlying mechanisms. Recently, transcranial magnetic stimulation has been developed as a valuable research tool and can be used to study brain function noninvasively. This article is a review of the studies done in migraine using transcranial magnetic stimulation.

Generator sites of early scalp potentials evoked from the three trigeminal branches.

OBJECT: The aim of this study was to seek evidence about the generators of the first three components of the scalp's early trigeminal evoked potentials (TEPs) obtained by stimulation of the supraorbital (SW1, SW2, and SW3), infraorbital (W1, W2, and W3) and mental (MW1, MW2, and MW3) nerves. METHODS: Simultaneous scalp and depth recordings were measured during surgical procedures in which thermorhizotomy and microvascular decompression were performed. CONCLUSIONS: Direct evidence was found that the origin of MW1 lies in the mandibular nerve at the foramen ovale, whereas the origin of W1 in t

Clinical trials for polyneuropathy: the role of nerve conduction studies, quantitative sensory testing, and autonomic function testing.

Clinical trials to assess the treatment of diabetic and other forms of polyneuropathy are becoming increasingly common. Nerve conduction studies, quantitative sensory testing, and autonomic function testing are often used in these trials. This article reviews the sensitivity and reproducibility of these measures to detect change in peripheral nerve function during long-term trials. The attributes of nerve conduction studies that are likely to be most useful are summated or averaged sensory nerve action potential amplitudes and averaged motor nerve conduction velocities. Summated or averaged co

Relapsing chronic low-dose corticosteroid-responsive multifocal motor neuropathy with conduction block.

Multifocal Motor Neuropathy (MMN) is a newly described disorder that might mimics the Motor Neuron Disease (MND). MMN is considered by some authors to be an entity distinct from Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP), nevertheless because a pure-motor variant of CIDP is well recognized and CIDP can present Partial Motor Conduction Block (PMCB), several authors believe that MMN can be a variant of CIDP. We describe a 27-year old male with a relapsing chronic low-dose corticosteroid-responsive MMN with conduction block. Our case stresses that MMN includes a wide range o

Activation and entrainment mapping defines the tricuspid annulus as the anterior barrier in typical atrial flutter.

BACKGROUND: The importance of anatomic barriers in the atrial flutter reentry circuit has been well demonstrated in canine models. It has been shown previously that the crista terminalis and its continuation as the eustachian ridge form a posterior barrier. In this study we tested the hypothesis that the tricuspid annulus forms the continuous anterior barrier to the flutter circuit. METHODS AND RESULTS: Thirteen patients with typical atrial flutter were studied. A 20-pole halo catheter was situated around the tricuspid annulus. A mapping catheter was used for activation and entrainment mappin
