Female stress urinary incontinence (SUI) is mainly due to an excessive mobility of the cervico-urethral complex. Synthetic tapes placed without tension underneath the urethra to correct this hypermobility have revolutionized the surgical treatment of female SUI for the past fifteen years. The retropubic approach, namely the "tension-free vaginal tape" (TVT), was designed in 1996. It generates high SUI cure rates but is associated with a risk of intrapelvic bleeding and bladder and intestine perforation. The inside-out transobturator approach, which was developed eight years ago in our Institut

OBJECTIVE: To investigate whether a preoperative pessary test could be used to identify patients with occult stress incontinence and if simultaneous anti-incontinence surgery with a midurethral sling could prevent postoperative incontinence.
STUDY DESIGN: Eighty-two patients with a positive pessary test and severe genital prolapse participated in the study: 43 patients underwent prolapse surgery and transobturator tension-free vaginal tape (TVT-O) insertion and 39 patients underwent prolapse surgery only.
RESULTS: The objective cure rate for TVT-O and prolapse surgery was 90.7% at three mont

INTRODUCTION AND HYPOTHESIS: We present a pure transvaginal approach to the removal of eroded mesh and a retained foreign body involving the bladder secondary to placement of transvaginal mesh for management of pelvic organ prolapse (POP) using a mesh kit.
METHODS: Transvaginal excision of mesh erosion involving the bladder after mesh placement using a prolapse kit was performed. A U-shaped incision was made in the anterior vaginal wall; mesh was identified, divided in the midline, and then dissected from the bladder. Additionally, a retained foreign body was removed from the bladder. The bla

BACKGROUND: Osteomyelitis associated with using synthetic mesh for laparoscopic sacral colpopexy is rare.
CASE: We present a patient who developed Staphylococcus sacral osteomyelitis after sacral colpopexy with synthetic mesh and titanium tack fixation to the sacral promontory in the absence of mesh erosion or fistula formation. The patient presented with low back pain 6 weeks postoperatively. Magnetic resonance imaging, bone aspirate, and culture confirmed sacral osteomyelitis and discitis 10 weeks after surgery. The patient underwent 8 weeks of outpatient antibiotic treatment. Six months af

BACKGROUND: Gynecare Prolift has been successfully used for pelvic floor repair with favorable objective and subjective outcomes. There have been, however, increasing reports of significant rates of postoperative dyspareunia and impairment of sexual function.
MATERIALS AND METHODS: We are presenting two cases of post Prolift dyspareunia. The patients underwent several vaginal revisions with excision of the apical bands. However, they returned soon with the same complaint. Ultimately, laparoscopic excision of the apical mesh was performed.
RESULTS: Patients reported significant improvement in

OBJECTIVES: To present a new minimal invasive suburethral tape device derivative of the classic TVT, to describe the technique of laying, to evaluate complications and results to short term.
MATERIALS AND METHODS: Prospective multicentric study of 110 patients presenting a stress urinary incontinence and benefiting from the laying of TVT Secur without associated operation. The tape is identical to that old-fashioned retropubic and obturator TVT, smaller, laying in "U" or in "hammock" without orifice of exit, to avoid complications due to crossed spaces of the other techniques. Post-traumatic urethral damage resulting in urethrovaginal fistulas or strictures, though rare, should be suspected in patients who have unexpected urinary incontinence or lower urinary tract symptoms after pelvic surgery, pelvic fracture, a long-term indwelling urethral catheter, or pelvic radiation. Careful physical examination and cystourethroscopy are critical to diagnose and assess the extent of the fistula. A concomitant vesicovaginal or ureterovaginal fistula should also be ruled out. The two main indications for reconstruction are sphincteric incontinence and urethral obstruction. Surg

Complications occurred in two women of 45 and 54 years of age who were treated with tension-free midurethral sling procedures. The first woman was treated with transobturator tape due to stress incontinence and an overactive bladder. The procedure resulted in a worsening of the overactive bladder. The second woman was treated with tension-free vaginal tape (TVT) due to stress incontinence and subsequently suffered from pain on urinating. In the first patient, the tape was found to be too tight and following adjustment of the tape tension, the overactive bladder symptoms and the stress incontin

BACKGROUND AND PURPOSE: Bladder-neck suspension has been used widely for the management of female stress incontinence. Despite high early success rates, the long-term results are poor. This in-vitro model was designed to simulate the tear forces affecting the sutures under standardized laboratory conditions.
MATERIALS AND METHODS: First, we determined the ultimate strength of various tissues (porcine vagina [PV], human vagina [HV], porcine colon, and chicken skin) in resisting standardized traction force and expansion. Subsequently, different types of suture fixation (U stitch, Z stitch, patc

A follow-up was done 7 and 38 months after tension-free vaginal tape (TVT) operation in 1,113 women with mixed urinary incontinence. Mixed incontinence and predominant bother were subjectively defined. The results were analyzed according to the women's predominant bother: stress incontinence, urge incontinence, or stress and urge incontinence equally. Across the groups, stress incontinence was cured in 87 and 83% of the women at 7 and 38 months, respectively, with no difference between the three groups. Women with predominant stress incontinence had significantly better results at both 7 and 3

Reports of the tension-free vaginal tape (TVT) procedure performed in combination with reconstructive vaginal surgery have been published, but none of these has looked specifically at the sub-group of TVT in combination with sacrospinous colpopexy. This report describes a series of 15 cases where TVT was performed in combination with sacrospinous colpopexy. There were no major complications. Five patients (33.3%) had postoperative urine retention for more than 1 week. Mild incontinence was reported in five cases at 12 months. We conclude that the results of TVT in treating stress incontinence

INTRODUCTION AND HYPOTHESIS: Our purpose was to verify whether extensive dissection toward the sacrospinous ligament (SSL) needed for mesh fixation during anterior compartment repair increases the risk of postoperative voiding difficulties.
METHODS: A total of 124 patients after anterior compartment mesh repair without simultaneous suburethral sling placement operated on in the period 2005-2012 were enrolled in this retrospective observational study. Patients with previous anti-incontinence surgery with normal urodynamics were not excluded; 30 patients with incomplete data, severe perioperati
