Disparities in Reporting of Violent Events by Patients at a US Children's Hospital.

BACKGROUND: Violence toward health care workers is increasing, yet patient risk factors for having a reported violent event (VE) are unknown … white children. Many MBH conditions were significantly associated with increased aORs for VEs, with disruptive, impulse control, and conduct disorders (aOR 6.25; 95% CI, 4.17-9.36) conveying the highest risk. We saw similar patterns of disparities when analyzing just those children with an MBH diagnosis. CONCLUSION: Racial disparities in VE reporting highlight the need for applying an equity lens …

Development and Acceptability of Provider Training to Increase Treatment Engagement of Parents in Their Children's Behavioral Health Care Need.

BACKGROUND: Disruptive, Impulse-Control, and Conduct disorders (DIC) affect 5 million children in the United States and often require comprehensive and long-term behavioral health care for which sustained parental involvement is essential. Our research team is developing an intervention to improve parental engagement in the behavioral health care of their children with DIC. The intervention, which will be a modification of an evidence-based shared decision-making intervention called DECIDE, will include a parent component and a provider component. AIM: To determine the acceptability of the pro

Consensus report on impulsive aggression as a symptom across diagnostic categories in child psychiatry: implications for medication studies.

OBJECTIVE: To determine whether impulsive aggression (IA) is a meaningful clinical construct and to ascertain whether it is sufficiently similar across diagnostic categories, such that parallel studies across disorders might constitute appropriate evidence for pursuing indications. If so, how should IA be assessed, pharmacological studies designed, and ethical issues addressed? METHOD: Experts from key stakeholder communities, including academic clinicians, researchers, practicing clinicians, U.S. Food and Drug Administration, National Institute of Mental Health, industry sponsors, and patien

High sensitivity to punishment and low impulsivity in obsessive-compulsive patients with hoarding symptoms.

Recent factor-analytic studies involving over 2000 patients have reduced the symptoms of obsessive-compulsive disorder (OCD) into a few dimensions or potentially overlapping syndromes. Hoarding consistently emerged as a separate factor in all these studies. This study investigated the relationship between OCD symptom dimensions and normal personality traits in a sample of 56 OCD patients. They were administered the Sensitivity to Punishment and Sensitivity to Reward Questionnaire and the Eysenck Personality Questionnaire, derived from Gray's and Eysenck's personality models, respectively. The

The importance of callous-unemotional traits for extending the concept of psychopathy to children.

This study focused on the use of callous-unemotional (CU) traits to identify a subgroup of children with both attention deficit/hyperactivity disorder (ADHD) and a conduct problem diagnosis (oppositional defiant disorder [ODD] or conduct disorder [CD] who show characteristics similar to adults with psychopathy. In a clinic-referred sample of children aged 6 to 13 years (N = 154), those with diagnoses of both ADHD and ODD/CD were divided on the basis of teacher ratings of CU traits. Children high on these traits showed features typically associated with psychopathy, such as a lack of fearfulnes

Demographic and clinical features of 131 adult pathological gamblers.

BACKGROUND: This study was constructed to detail the demographic and phenomenological features of pathological gamblers. METHOD: One hundred thirty-one subjects with DSM-IV pathological gambling were administered a semistructured interview to elicit demographic data and information on the phenomenology, age at onset, course, associated features, treatment history, and response to treatment of the disorder, followed by the Structured Clinical Interview for DSM-IV. RESULTS: Seventy-eight female (59.5%) and 53 male (40.5%) (mean +/- SD age = 47.7+/-11.0 years) pathological gamblers were studied

Impulse control disorders in Parkinson's: Sleep disorders and nondopaminergic associations.

OBJECTIVES: Impulse control disorders (ICDs) are common among patients with Parkinson's disease (PD). Risk factors identified for developing ICDs include young age, family history, and impulsive personality traits. However, the association of these potentially disabling disorders with nondopaminergic drugs and sleep disorders has been understudied. Our objective was to examine the association between ICDs and nondopaminergic medications and sleep disorders. METHODS: We conducted an observational study of 53 patients with PD from the National Institute of Neurology and Neurosurgery. ICDs were

Empathic deficits and alexithymia in trauma-related impulsive aggression.

Our long term interest is to develop a developmental model of impulsive aggression based on a confluence of social, psychological and biological features. This approach incorporates neurobiological research, which has identified language processing deficits as a unique characteristic of impulsive aggressors and extends it to include emotional deficits. As an initial test of this hypothesis, we examined whether empathy and alexithymia were associated with impulsive aggression. Regressions were performed to explore the associations among impaired empathy, alexithymia, impulsive aggression, verba

How to resist temptation: the effects of external control versus autonomy support on self-regulatory dynamics.

The purpose of the present study (N=80 undergraduate students) was to examine two issues: first, does external control lead to an increase in resistance to temptation more than the use of autonomy support? Second, what are the long-term effects of these types of educational style? Based on the Personality Systems Interaction (PSI) theory, external control was expected to increase resistance to temptation for those participants who lack initiative and self-motivation (i.e., state-oriented participants). Consistent with expectations, resistance to temptation was greater for state-oriented partic

Inverse relationship between numbers of 5-HT transporter binding sites and life history of aggression and intermittent explosive disorder.

The objective of this study was to determine if platelet 5-HT transporter (5-HTT) sites vary as a function of aggression, and/or impulsiveness, and differ as a function of Intermittent Explosive Disorder (IED). Accordingly, the number of platelet 5-HTT sites was assessed in 100 personality disordered (PD) individuals with varying degrees of aggressiveness. The number of platelet 5-HTT sites was assessed by examining the Bmax of H(3)-Paroxetine Binding to the blood platelet. Life history of aggression was assessed by Life History of Aggression. Impulsivity was assessed by the Barratt Impulsiven

Early visual information processing deficit in depression with and without Borderline Personality Disorder.

Backward masking is a measure of early visual information processing usually abnormal in psychotic disorders. Previous studies of subjects with Borderline Personality Disorder have been inconsistent regarding their impairment or lack of impairment on backward masking. We examined visual backward masking performance in samples of unmedicated depressed patients with (n=12) and without (n=16) Borderline Personality Disorder, and healthy volunteers (n=18). Accuracy was poorer in depressed BPD patients, relative to both non-BPD depressed and healthy comparison subjects. As in previous studies, no d

Waiting impulsivity: a distinctive feature of ADHD neuropsychology?

Impulsivity is a core feature of attention-deficit hyperactivity disorder (ADHD). It has been conceptualized in a number of different ways. In the current article, we examine how the new concept of "waiting impulsivity", which refers to premature responding before a scheduled target appears, adds to our understanding of impulsivity in ADHD. Sixty children (8-12 years old; 30 ADHD; 30 typically developing controls) completed the 4-choice serial reaction time task, a measure of waiting impulsivity, alongside tasks measuring inhibitory control and temporal discounting and questionnaires measuring

Current issues in the treatment of OC-spectrum conditions.

Obsessive-compulsive (OC)-spectrum conditions consist of neurological-motoric conditions, impulse-control disorders, and disorders associated with bodily preoccupation. This article is a review of some understudied OC-spectrum conditions, with particular focus on phenomenology and overlap with obsessive-compulsive disorder, etiology, treatment outcome, treatment refractory issues, and new developments in treatment research. The focus will be on representative disorders from each related area, namely, Tourette's syndrome, trichotillomania, skin-picking, and body dysmorphic disorder. Similaritie

Family history of suicidal behavior predicts impulsive-aggressive behavior levels in psychiatric outpatients.

OBJECTIVES: Impulsive-aggressive traits have been proposed as mediators of the transmission of suicidal behavior in families. Our aim was to investigate the association between family history of suicidal behavior, and expressions of impulsivity and aggressive behavior, in a clinical population. METHODS: A sample of n=474 psychiatric outpatient subjects were assessed by means of standardized interviews for Axis I and II psychopathology, history of suicidal behavior in subjects and their family, and measures of impulsive-aggressive behaviors (IAB). RESULTS: A total of n=38 (8%) probands had a

[Anger regulation in boys with attention deficit hyperactivity disorder (ADHD)].

OBJECTIVES: We explored the reaction of boys with ADHD when they are angry at a friend. Due to their impulsivity and difficulty in focussing their attention, it may be expected that such boys be inclined to resort more frequently to confrontational and harmful strategies of anger regulation and less often to negotiation, reappraisal, or distancing strategies. METHODS: A sample of 23 boys diagnosed with ADHD (without co-morbid Oppositional Defiant Disorder or Conduct Disorder) between eight and thirteen years of age were compared to an age-matched sample of 23 boys with non-clinical behaviour.

A comparison of the Yale-Brown Obsessive Compulsive Scale for "heavy drinking" with a single item craving measure: construct validity and clinical utility.

The measurement of alcohol "craving" began with single-item scales. Multifactorial scales developed with the intention to capture more fully the phenomenon of craving. This study examines the construct validity of a multifactorial scale, the Yale-Brown Obsessive Compulsive Scale for heavy drinking (Y-BOCS-hd). The study compares its clinical utility with a single item visual-analogue craving scale. The study includes 212 alcohol dependent subjects (127 males, 75 females) undertaking an outpatient treatment program between 1999-2001. Subjects completed the Y-BOCS-hd and a single item visual-ana

The efficacy of ECT in adults with mental retardation experiencing psychiatric disorders.

There is a paucity of empirical data establishing the efficacy of electroconvulsive therapy (ECT) in patients with mental retardation and psychiatric disorders. This study examines the efficacy of ECT on specific symptoms and between psychiatric diagnoses in patients with mental retardation who are psychiatrically ill. A chart review was performed on 20 inpatients who had received ECT on a dedicated Mental Retardation-Dual Diagnosis Unit and were divided into 3 categories: mood disorders (n = 12), psychotic disorders (n = 6), and intermittent explosive disorder (n = 2). Ratings were performed
